Anterior urethral valves.

Two cases of anterior urethral valves are reported, one occuring in a child with renal osteodystrophy and the second being the first recorded case of anterior urethral valves in an adult. These 2 cases represent the thirteenth and fourteenth cases of anterior urethral valves reported in the literature.

Post-obstructive diuresis.

Massive diuresis after relief of urinary tract obstruction has been divided into 3 categories: 1) salt, 2) urea and 3) water. Measurements of the urinary electrolytes and urine osmolality establish the character of the diuresis and facilitate in the fluid management of these patients. A clinical example of the 2 most common varieties, salt and urea, is presented in conjuction with the method of fluid replacement.

A comparative evaluation of Avitene and gelfoam for hemostasis in experimental canine prostatic wounds.

Microcrystalline collagen has been reported to be an effective hemostatic agent in brain, liver, kidney, etc. This experimental study in dogs and rats shows that microcrystalline collagen is a more effective hemostat in prostatic hemorrhage than is purified gelatin solution. No gross or histologic evidence of tissue damage or calcification was induced by microcrystalline collagen or purified gelatin solution. Wafers of microcrystalline collagen that are placed in rat bladders dissolve and do not induce calculogenesis.

Putting the process of care into practice.

"Putting the process of care into practice" provides an interactive, visual model of outpatient resources and processes. It illustrates an episode of care from a fee-for-service as well as managed care perspective. The Care Process Matrix can be used for planning and staffing, as well as retrospectively to assess appropriate resource use within a practice. It identifies effective strategies for reducing the cost per episode of care and optimizing quality while moving from managing costs to managing the care process. Because of an overbuilt health care system, including an oversupply of physicians, success in the future will require redesigning the process of care and a coherent customer service strategy. The growing complexities of practice will require physicians to focus on several key competencies while outsourcing other functions such as billing and contracting.
