Oral sodium cromoglycate treatment of atopic dermatitis related to food allergy.

Thirty-two children and two adults with chronic atopic dermatitis related to food allergy entered this double-blind crossover study comparing oral sodium cromoglycate (200-1600 mg/24 h) with placebo. Each treatment period comprised 6 weeks: 4 weeks on elimination diet, and 2 weeks on a normal, i.e. unrestricted diet. The diagnosis of food allergy was made after clinical improvement with elimination diet and relapse after challenge. Overall analysis of skin symptoms evaluated by means of clinical assessments and diary cards, opinions of treatment, and use of concomitant medication gave no evidence of any difference between sodium cromoglycate and placebo. Unusual symptoms were reported by 18 patients. In one case the patient was withdrawn during the sodium cromoglycate period because of side effects. The majority of symptoms with both treatments were stomach problems. Overall analysis of the laboratory data gave no significant differences between treatments.

A clinical comparison of aerosol and powder administration of beclomethasone dipropionate in childhood asthma.

Forty children with childhood asthma were conducted through a double-blind cross-over study to compare the effect of 400 micrograms beclomethasone dipropionate administered as aerosol and powder (Rotacaps). Children with severe asthma derived more benefit from the steroid administered as aerosol judged from their peak flow performance in the morning, although neither evening peak flow nor symptom scores revealed any difference. It is concluded that the Rotacaps are an advantageous supplement to available medication for the treatment of asthmatic children, although it should be observed that, using equal amounts of BDP, powder administration seems less effective than aerosol.

Pretreatment of exercise-induced asthma in children using disodium cromoglycate and fenoterol inhalation powder.

The effect of pretreatment with inhalation powder containing fenoterol, sodium cromoglycate, fenoterol plus sodium cromoglycate and placebo, in the prevention of exercise-induced asthma was compared in a randomized, double-blind, cross-over investigation including 17 asthmatic children with exercise-induced asthma. Six minutes of treadmill running was carried out in a climate chamber 30 minutes after pretreatment on the four test days. The post-exercise fall in PEF after pretreatment with fenoterol was 7%, with sodium cromoglycate 20%, with fenoterol plus sodium cromoglycate 6% and after placebo 27%. Both sodium cromoglycate and fenoterol provided protection against exercise-induced asthma (p less than 0.05 and p less than 0.001). Fenoterol gave significantly better protection than sodium cromoglycate (p less than 0.001), and it is concluded that the treatment of choice for the prevention of exercise-induced asthma is the inhalation of a beta 2-agonist.

Comparison of isocapnic hyperventilation and treadmill exercise in children with exercise-induced asthma.

The degree of post-exercise airway obstruction (Exercise-Induced Asthma (EIA] in 14 children was compared to the degree of airway obstruction following isocapnic hyperventilation. EIA was provoked by 6 min of treadmill running. Isocapnic hyperventilation was performed sitting during 6 min. The total ventilation (Vtot) during the two provocations was identical. The temperature of the inspired air was also identical during the two provocations, and the relative humidity was 40% during treadmill-running and 15% during hyperventilation. The decrease in peak expiratory flow after treadmill-running was 29%. After hyperventilation a fall on 19% was seen. These figures are statistically different. It is concluded that although there is a significant difference in airway obstruction after the two provocations the ventilation is greater importance for EIA than is the work load.

The effect of sodium cromoglycate delivered by Spinhaler and by pressurized aerosol on exercise-induced asthma in children.

Sixteen children completed a randomized placebo-controlled double-blind study comparing the efficacy of pre-treatment with (1) 20 mg sodium cromoglycate (SCG) powder delivered by Spinhaler, (2) 2 mg SCG delivered by pressurized aerosol and (3) placebo. The drug was administered 15 min before the 6 min of treadmill-running performed at the same time of day on 3 consecutive days in a climate chamber with a temperature of 23 degrees C and a relative humidity of 40%. All the patients, none of whom were taking systemic steroids, had shown a post-exercise fall in peak expiratory flow (PEF) of more than 15% from baseline when tested one day prior to the study. On the day with no pre-treatment the average post-exercise fall in PEF was 46% (+/- 4) (mean +/- s.e. mean). After pre-treatment with SCG powder the average fall in PEF was 21% (+/- 3), after pre-treatment with SCG aerosol the fall was 22% (+/- 3) and after placebo a post-exercise fall of 31% (+/- 5) was seen. Both forms of SCG were significantly better than pre-treatment with placebo. Comparing no pre-treatment with pre-treatment with placebo revealed a statistically significant difference (P less than 0 . 001) indicating a placebo effect. It is concluded that this study design has a marked placebo effect. Both pre-treatment with 2 mg SCG delivered by pressurized aerosol and 20 mg delivered by Spinhaler is superior to placebo in conferring protection against exercise-induced asthma (EIA). No bronchodilating effect was seen in the first 15 min after administration of the drug.

Fat cell size and blood lactate in humans.

Twelve patients randomly selected as regards obesity were investigated. Morphological fat tissue parameters (size and number of adipose cells) were measured together with metabolic parameters in blood (K-value, fasting glucose, lactate, triglyceride, and free fatty acids). A high, positive correlation (0.86) between blood lactate and fat cell size was found. Also serum triglyceride and fat cell size showed a positive correlation (0.64). No correlations were found between fat cell number and the measured metabolic parameters.

Fat cell number and weight gain in lithium treated patients.

Total amount of fat, fat cell size, and fat cell number were determined in 33 patients under long-term lithium treatment. The weight gain during the treatment was estimated both by the patients themselves (questionnaire) and from their records. A positive correlation between fat cell number and weight gain was found whereas the fat cell size showed no correlation to weight gain.

B, T and null lymphocytes in newborn infants and their mothers.

Estimation of B, T and null cells were performed on 29 newborn healthy babies and 16 mothers. The lymphocytes were isolated from peripheral venous blood, which is considered to be more representative of the immune state in the newborn than the cord blood. B lymphocytes were estimated by cytofluorometric measurements, T lymphocytes by sheep red blood cell rosette technique, (SRBC-R). Combined immunofluorescence and SRBC-R technique revealed the null cells. In the newborn babies the amount of B and T cells were found to be diminished. In the mothers the amount of B lymphocytes were low compared with normal adults. The rather high null cell percentage found in the babies might represent immature precursor cells. Mothers seem to be immuno-depressed as reflected in the low amount of B cells.

Transplantation of human adipose tissue to nude mice.

Human adipose tissue was transplanted to the mouse mutant nude (nu/nu). All the grafts were accepted and contained fat cells easily distinguishable from those of the mouse. No detectable relation between the histological pictures before and after grafting was found. In some transplants nerve tissue, and in others macrophages containing fat droplets, were found. The fat tissue graft might be useful for investigation of the influence of various hormones on human fat cells.

The relationship between pregnancy, HCS and B lymphocytes.

The quantitative distribution of the lymphocyte subpopulations (B, T and null) and the serum concentrations of Human Chorionic Somatomammotropin (HCS), immunoglobulins IgG, IgM and IgA, complement component C4 and C1 inactivator, were estimated in venous blood samples from 32 women at various stages of pregnancy and compared with a control series of 7 non-pregnant normal women. A significant decline in the B cell percentage during pregnancy was seen. The fall in the percentage of the B lymphocytes was found to be concomitant with the rise in HCS concentration. No significant changes in the other parameters studied were present.
