Experimental cerebral vasospasm: resolution by iloprost.

A new stable analogue of prostacyclin, iloprost, was tested on the basilar artery of anaesthetized rabbits constricted by electrical stimulation and intracisternal injection of autologous blood. The vasospasm induced by these stimuli was effectively relieved (about 100%) by topical application of iloprost, a stable analogue of prostacyclin (PGI2), and no constriction occurred on further stimulation within one hour. The mechanisms of the action of iloprost and its possible therapeutic value in subarachnoid haemorrhage are discussed.

Effectiveness of radiofrequency thermocoagulation in recurrent trigeminal neuralgia after previous retrogasserian rhizotomy.

Between 1974 and 1984, 428 trigeminal neuralgia cases were treated by controlled radiofrequency thermocoagulation (RFTC). 29 had recurrent trigeminal neuralgia after intracranial surgery. 26 of the 29 patients were treated by retrogasserian rhizotomy and 3 by posterior fossa exploration. Among the 26 recurrent trigeminal neuralgia following retrogasserian rhizotomy, RFTC was effective in 23 cases (88.5%), and in 3 cases (11.5%) RFTC was effective for a short period. Repeated RFTC was unable to control the pain attacks which were later relieved by posterior fossa exploration and root section. Of the 3 recurrent trigeminal neuralgia following posterior fossa exploration, RFTC was effective in 2 cases (66.6%).

Symptomatology, evaluation and treatment of aquaductal stenosis.

Thirty-three surgical treated cases of aquaductal stenosis are presented. Beside the classical clinical representation of the disease, the symptomatology caused by anterior and posterior herniation of dilated third ventricle is discussed. The impotence in one male patient which is caused by anterior herniation was proved by means of NPT (Nocturnal Penile Tumescence) recording. In one patient bilateral partial deafness and Parinaud syndrome which was caused by posterior herniation was cured by third ventriculostomy. In the surgical treatment of aquaductal stenosis, the effectiveness of various surgical procedures are presented and the superiority of third ventriculostomy by microtechnique is emphasised.

Venous malformation of the aqueduct of Sylvius treated by interventriculostomy: 15 years follow-up.

A single venous shunt obstructing the aqueductus of Sylvius was treated by interventriculostomy. A passage for CSF was obtained between the third and fourth ventricles. The catheter which was left in the aqueduct collapsed the malformation, possibly preventing a future haemorrhage. A Simple rubber catheter in the aqueduct of Sylvius has been well tolerated by the patient for 15 years.

Brain tumour associated with intracranial arterial aneurysm.

Four cases of brain tumour associated with intracranial aneurysm are presented, and the available literature is reviewed. The association of brain tumours and arterial aneurysms is not well documented in neurosurgical literature. Reports of the association of giant aneurysms with brain tumours are rare. We could find only five such cases, and one of our cases is the sixth. The operative mortality of the patient with coexisting brain tumour and cerebral aneurysm is high. Two of our cases died postoperatively, one from pre-existing cardiac disease, and the other from hypothalamic vascular insuffiency related to surgery.

Third ventriculostomy by microtechnique.

The microtechnical approach to third ventriculostomy is presented. Opening of the lamina terminalis and basal membrane of the floor of the third ventricle is combined with opening of the membrane of Liliequist. The technique was used in five cases of aqueductal stenosis. We stress the importance of the microscope, and opening of the membrane of Liliequist to gain additional access.

Aneurysm and arteriovenous malformation of the spinal cord.

A case is presented in which microsurgical technique was used to excise an intramedullary vascular lesion of the spinal cord. This kind of vascular pathology is uncommon. In this case, a radical excision of the malformation was obtained using the microscope. What appeared to an associated intramedullary aneurysm was totally removed.

Meningiomas of the third ventricle.

Intraventricular third ventricle meningiomas are very rare. Fourteen cases have been previously reported. We present a case of third ventricle meningioma, and discuss aetiology, pathology, symptoms, signs, position in the ventricle, and surgical approaches.

Cerebral Ventricle Neoplasms

Pituitary pseudotumor.

Non-tumoral aqueductal stenosis with hydrocephalus causes dilatation of the third ventricle. This in turn may compress the hypothalamus and optic nerves and erode the clinoids and enlarge the sella turcica. In female patients, amenorrhea or oligomenorrhea is the predominant endocrinologic symptom. Symptoms usually disappear after the drainage of cerebrospinal fluid. Third ventriculostomy by microtechnique was used for operative treatment of our three cases. In all of these, the hydrocephalus was arrested.
