Factor XIII-mediated cross-linking of NH2-terminal peptide of alpha 2-plasmin inhibitor to fibrin.

The NH2-terminal 12-residue peptide of alpha 2-plasmin inhibitor, Asn-Gln-Glu-Gln-Val-Ser-Pro-Leu-Thr-Gly-Leu-Lys-NH2 . AcOH, was found to be a good substrate for plasma transglutaminase (activated blood coagulation factor XIII) and rapidly incorporated into fibrin by the enzyme. A high concentration of the peptide inhibited the enzyme-mediated cross-linking of alpha 2-plasmin inhibitor to fibrin probably by competing with the inhibitor for the same site of fibrin alpha-chain.

A pilot study on the anticonvulsive effects of a thyrotropin-releasing hormone analog in intractable epilepsy.

In 1981, Inanaga et al reported on the efficacy of DN-1417 (a TRH analog) in the treatment of degenerative myoclonus epilepsy and other forms of intractable epilepsy. Following this report, we studied the efficacy and safety of DN-1417 treatment in 10 intractable epileptic children ranging in age from 6 months to 11 years (mean 4 years), including 7 with Lennox syndrome (LS), 2 with West syndrome (WS) and 1 with degenerative myoclonus epilepsy (DME). The daily dose was from 0.02 to 0.05 mg/kg, initially, and then was increased to 0.05 mg/kg. Complete control of seizures was achieved in 2 patients with LS, a 50% or greater decrease in seizure frequency was observed in one patient each with LS and DME and a less than 50% decrease in 1 with LS and 2 with WS. There was no change in 2 LS cases, and 1 LS case became worse. Activation of psychic activities, such as psychomotor activity, facial expression and motivation, was also noted in 7 of the 10 patients. Furthermore, improvement of motor function was noted in 5 patients. Electroencephalographic abnormalities improved in 2 completely seizure free patients with LS in which EEG ameliorated along with clinical seizure control.

Statistical analysis of Japanese Thorotrast-administered autopsy cases--1980.

In 193 cases autopsied between 1945 and 1980, all persons who had been intravascularly injected with Thorotrast in life, the authors found 131 malignant hepatic tumors, 20 liver cirrhoses, 6 myeloid leukemias, 4 erythroleukemias, 5 aplastic anemias, 4 lung cancers, 1 mesothelioma and 1 osteosarcoma. The causes of death in the Thorotrast-administered autopsy group (193 cases) were compared with those of a non-Thorotrast-administered autopsy group (95,000 cases) of the same sex and age at death as recorded in the Annals of Japanese Pathological Autopsy cases from 1958 to 1978. This comparison revealed that the frequencies of malignant hepatic tumors, liver cirrhosis, erythroleukemia, and aplastic anemia were significantly higher in the Thorotrast-administered group than in the non-Thorotrast-administered group.

Undifferentiated carcinoma of the parotid gland with bizarre giant cells. Clinicopathologic report with ultrastructural study.

A rare autopsy case of undifferentiated carcinoma of the parotid gland with bizarre giant cells is reported. Grossly, the tumor was fist-sized, firm, solid, grayish-white in color, and was not encapsulated, with a bean-sized ulceration on the skin. Microscopical examination revealed undifferentiated carcinoma of the parotid gland composed of bizarre giant cells, large tumor cells, and spindle cells in part. Ultrastructural examination, recovered from formalin-fixed material, disclosed filamentous structure in some large tumor cells. It is strongly suggested that myoepithelial cells played an important role in the development of the undifferentiated carcinoma of the parotid gland. The clinical and histopathological characteristics of this case is described, and the literature concerning undifferentiated carcinoma of the parotid gland is reviewed.

Treatment of DIC with antithrombin III concentrates.

We administered AT III concentrates to 21 patients with DIC who failed to respond initially to heparin therapy. About 60% of these 21 patients were effectively treated with AT III concentrates by enhancing the effect of heparin and alleviating the burden of excessive plasma transfusions on the cardiovascular system.

Epidural hematoma communicating with cephalhematoma in a neonate.

The author reports a unique case of a neonate with a depressed skull fracture detected immediately below a cephalhematoma complicated with an epidural hematoma. The epidural hematoma communicating with the cephalhematoma through a depressed skull fracture disappeared after the cephalhematoma was punctured for aspiration, and the depressed fracture reduced spontaneously 6 months thereafter.

[Histopathological evaluation of minute stomach cancer].

We histologically examined 17 lesions from minute gastric cancer (less than 5 mm in diameter). Flat or IIb type was frequently seen in minute cancer (47%); the depressed type was predominant in ordinary early gastric cancer (72%). Differentiated adenocarcinoma was noted more often in minute cancer (88%) than in ordinary early gastric cancer (61%) or advanced cancer (45%). Differentiated type minute cancer was frequently associated with severe atrophic gastritis (93%) and intestinal metaplasia (73%). There was no evidence for the preexistence of chronic peptic ulcer, because no fibrosis was present in the gastric wall beneath the minute cancerous lesions.

Fibrin-associated plasminogen activation in alpha 2-plasmin inhibitor deficiency.

The clot formed from the plasma of a patient with congenital deficiency of alpha 2-plasmin inhibitor underwent a spontaneous extensive fibrinolysis. Radiolabeled fibrinogen was added to the plasma before clotting, and the whole process of the fibrinolysis was followed by measuring the release of radiolabels. Plasminogen activation was also followed by measuring the amidolytic activity that developed. There was an initial latent period, followed by an exponential increase of fibrinolytic activity. During the latent period, there was little or no release of radiolabels and no development of amidolytic activity. During the latent period, the clot was washed thoroughly to remove unbound proteins from fibrin and was incubated in buffered saline. The washed clot still underwent fibrinolysis, similar to the original plasma clot, suggesting that the plasminogen/plasminogen activators bound to fibrin during the initial latent period are responsible for fibrinolysis. The amount of plasminogen bound to fibrin during the latent period was close to the amount of plasminogen activated during the whole process of fibrinolysis. When the amount of plasminogen bound to fibrin was decreased by epsilon aminocaproic acid, the extent of fibrinolysis was decreased in parallel with the decrease of the amount of the bound plasminogen. This suggests that the amount of plasminogen bound to fibrin is one of the determinants of the rate of the fibrinolytic process.

[Hypertensive basal ganglionic hemorrhage with hemianopsia as the sole symptom--report of 2 cases].

The majority of patients with sudden onset of homonymous hemianopia are caused by occlusion of the posterior cerebral artery. Apart from a subcortical hemorrhage in the occipital lobe, no previous cases of the hypertensive intracerebral hemorrhage presenting isolated homonymous hemianopia have been reported. Since the introduction of computed tomography (CT), the authors have experienced two cases of hypertensive basal ganglionic hemorrhage (including a thalamic hemorrhage) showing homonymous hemianopia alone as the neurological manifestation. Both patients (age 40 and 45) are right-handed male with known hypertension. The CT scans demonstrated a hematoma laterally and posteriorly along the thalamus on the left side. The cerebral angiography were unremarkable. The authors discussed the mechanism presenting homonymous hemianopia without association of pyramidal signs. These two cases described above are unique, hypertensive basal ganglionic hemorrhage, presenting a localized damage in the optic peduncle with sparing the pyramidal tract. In addition, they are worthwhile to be paid attention, because of showing homonymous hemianopia as a single, neurological manifestation, as well as being able to be diagnosed only by CT.
