[Use of perfluorocarbon emulsion in kidney transplantation].

Intravenous infusion of perfluorocarbon emulsion (PFCE) during autotransplantation of kidneys which had been stored for 24 hours led to a rapid normalization of intraorganic hemodynamics in dogs, and restoration of ionic homeostasis, energy potential, and functional activity of the transplants. It was shown that the protective effect of PFCE is associated with inhibition of free-radical lipid peroxidation, and the establishment of a stable protein-lipid conformation of the membrane structures of the cells of ischemic organs. It was established that the use of PFCE in the stage of stored kidney transplantation may be an effective method for the prevention of reperfusion damage to the transplant.

[Treatment of severe hepatic insufficiency by perfusion of patients' blood through a suspension of cryopreserved hepatocytes].

The results of hemoperfusion through a suspension of cryoconserved hepatocytes in 21 patients with severe hepatic insufficiency are analysed. It is shown that the manipulation had a beneficial effect on the clinical and biochemical indices, which was manifested by diminished jaundice and skin pruritus, increased physical activity, improved appetite; reduced levels of bilirubin, transaminase, alkaline phosphatase, bile acids, and cholesterol. The information gained is evidence that the method can be included in the complex treatment of patients with severe hepatic insufficiency.

[Optimal conditions for preservation of isolated heart using creatine phosphate and vitamin E (an experimental study)].

Functional and structural safety of the myocardium following 6-hour preservation at 4-6 degrees C in ion-balanced cardioplegic solution alone or in combination with pharmacological corrective was evaluated in rats. Langendorff and Neely model of isolated heart perfusion was used. The addition of creatine phosphate in a conservation of 10 mM/l to the solution or donor pretreatment with 70 mg/kg vitamin E was found to potentiate the protective properties of the solution under deep hypothermia. The combined use of the agents proved to warrant the initial intact structural and functional status of the rat myocardium.

[Ion-balanced cardioplegic solutions in cardiac surgery].

A comparative analysis of effectiveness of anti-ischemic defense of the myocardium performed in operations on the heart in 128 patients has shown that use of ion-balanced solutions is preferential since it allows reducing intrahospital lethality related with acute cardiac insufficiency from 13.3% to 6.3%.

[Changes in the local blood flows of the single kidney with partial impairment of the autonomic innervation].

The changes of cortical and intramedullary blood flow of the remaining after nephrectomy kidney under the condition of guanethidine sympathectomy and continuous pharmacological blockade of parasympathetic mediation by atropine were studied in chronic experiments on Wistar line rats in dynamics at 4, 7, 15, 30, 60, 90 and 120 days. Decrease in cortical and increase of intramedullary peritubular blood flow were noted in both experimental series, that is both blockade of sympathetic and parasympathetic nerves after 15 days. The conclusion was made that blood flow redistribution is a universal mechanisms of single kidney adaptation to its neurogenic impairments.

[Dynamic activity of the splenic lymphocytes during the modelling of chronic denervation-delymphatization of the kidneys in mice].

In experiments on mice CBA the activity of spleen lymphocytes was investigated in sham-operated mice and in mice with denervation-delymphatization of the both kidneys. The activity of lymphocytes was determined by registration of amounts of colony-formed elements by the method of Till and McCulloch. Periodic increase of the lymphocytes activity has been observed in mice with denervation-delymphatization kidneys. It is supposed that the periodic activation of lymphoid system, induced by denervation-delymphatization, may be one of triggering mechanisms of transplant rejection.

[Potentiation of the anti-ischemic effect of cardioplegic solutions by phosphorylated metabolites].

An experimental study in 307 isolated rat hearts was concerned with variations in myocardial resting tension, resting membrane potential, ATP levels and myocardial heat production in conditions of cardioplegic heart arrest by an ion-balanced cardioplegic solution with and without phosphorylated metabolites (ATP, phosphocreatine, inosine-F, glucose-6-phosphate) and dephosphorylated metabolites (glucose, inosine, and inorganic phosphates). Phosphorylated metabolites, incorporated in the solution, had a marked protective effect due to the activation of substrate regulation mechanisms at different stages of ATP synthesis and facilitation of cardiomyocyte adaptation to ischemic, hypothermic and cardioplegic exposure.

[Effect of perfluorocarbon emulsions on the protective properties of cardioplegic solutions].

The influence of ischemia and conventional and emulsion cardioplegia on the dynamics of resting tension, Ca2+ concentration, the level of membrane potential and oxygen tension in the myocardium was studied. It was shown that emulsion cardioplegia prolonged the duration of anoxic protection due to the additional oxygen supply of tissues and the decrease in Ca2+ cytoplasm accumulation.

[Use of balloon catheters for intraoperative control of resting myocardial pressure during cardioplegia].

Twenty-two patients operated on for aortal, mitral and multivalvular prosthesis under pharmacological cardioplegia were examined for the purpose of monitoring myocardial resting tension by the tensometric method and by using modified balloon catheters. The results demonstrated that the use of the balloon catheters permits making not only qualitative and quantitative assessments of myocardial lesions and a prompt evaluation of the function of the arrested heart but also the initial myocardial contractility and the restoration of myocardial contractility after cardioplegic arrest by controlling the contraction and relaxation myocardial rate.

[Ways of increasing the protective effect of cardioplegic solutions].

An experimental study on rat hearts using continuous recording of myocardial tension showed that the addition of various groups of drugs to the solution had negative effects on heart function. Positive effect was achieved when cardioplegia was combined with the pretreatment of animals with anti-ischemic drugs (alpha-tocopherol, dimethylsulfoxide) as well as preliminary oxygenation of the medium. Possible mechanisms of these myocardial effects are discussed.
