[The role of rheological factors in circulatory disorders in patients with arteriosclerotic lesions of major arteries].

The blood rheologic properties were studied in 49 patients with chronic arterial insufficiency. The values of systemic and regional circulation were studied in 25 patients. Changes in blood viscosity and aggregate condition were found to play an essential role in the occurrence of general and regional hemodynamic disorders. Reversal of the syndrome of hemorrheologic disorders led to normalization of central hemodynamic and regional circulatory values in patients with atherosclerotic lesions of the great vessels in the areas with maintained blood flow in the main channels and improved essentially the blood flow distal to the occlusion of the arterial trunk.

[Combined operations in cholelithiasis].

Experience in combined operations for cholelithiasis on 162 patients is discussed. In 98 patients, cholecystectomy was performed as a simultaneous stage of surgical treatment, the main stage of the operation was gastrectomy or resection of the stomach for cancer in 26 patients, formation of a small stomach for alimentary-constitutional obesity in 19, various types of vagotomy in 33, hemicolectomy, resection of the colon and sigmoid intestine for malignant tumors in 10, and reconstructive manipulations on the stomach, correction of intestinal obstruction, and other operations in 10 patients. Cholecystectomy was the main stage of the operation in 48 patients with hernias, benign tumors of the uterine appendages, breasts, and soft tissues. The authors insist that cholecystectomy must be carried out when cholelithiasis is a concurrent disease; they showed that increase in the extent of the operative intervention had no essential effect on mortality and frequency of postoperative complications (mortality rate, 1.2%). The authors substantiate the expediency of performing combined operations from economical considerations--the economical effect of the operations conducted by the authors came to some 51,000 roubles.

[Dynamics of regional blood circulation in normovolemic hemodilution in patients with arterial occlusive diseases].

Normovolemic hemodilution was used in 81 patients as preparation to reconstructive operation for the occlusion of main arteries of extremities. Regional hemodynamics was studied in 18 of them. It was established that acute hemodilution substantially increased the volume and tissue blood flow, regional systolic arterial pressure distal to the occlusion of the main vessel and reestablished the vessel tone. The oxygen transport became normal in the zones with preserved main blood flow and considerably increased in the places distal to the occlusion of the main artery.

[Effect of controlled normovolemic hemodilution on the microcirculatory bed and the incidence of postoperative complications in abdominal surgery].

Directed normovolemic hemodilution with the preoperative reservation of part of the patients' blood considerably reduced the pathologic effect of the operative aggression on the microcirculation system, allowed reduction of the frequency of postoperative complications and increased the efficiency of treatment of 93 patients with diseases of organs of the abdominal cavity.

[Hemodynamics in controlled normovolemic hemodilution in patients with obliterative vascular diseases].

The influence of acute controlled normovolemic hemodilution on the parameters of central hemodynamics was studied in 40 patients with obliterating diseases of lower extremities with eukinetic, hyperkinetic and hypokinetic types of blood circulation. Cardiac output was found to increase and general peripheral resistance to decrease after hemodilution. The greatest changes were noted in patients with the initial hypodynamic type of blood circulation. It was found that 24% increase of the oxygen transport function of the blood circulation was reached without an increase of the intensity of cardiac activity and hypoxia of the myocardium.
