[Psychological mechanisms of refusal reactions in schizophrenic patients].

The psychological study is based on the clinical typology of refusal reactions (RR) in schizophrenic patients--"capitulation", "avoidance", "absenteism". Seventy patients with RR, mean age 27 years, 25 women, 45 men, were examined with the use of a number of psychological methods. The findings prove the hypothesis on the disturbances of motivation as a psychological mechanism of RR in schizophrenic patients. Deficiency is related to functions of the motivational process: incentive function, goal achievement, sense making and strengthened by trait anxiety and desize to avoid danger. The main factor promoting RR formation is basic motivation deficiency caused by endogenous process (schizophrenia). A mechanism of capitulation reaction formation is closely related to expectation of failure and refusal from acting as an opportunity to preserve self-estimation; avoidance reaction is characterized by significant disturbance of the motivation at the stage of incentive, goal achievement and sense making. Motivation deficiency in patients with absenteism reactions is predominantly associated with an specific individual's sense.

[The background for development of refusal reactions in schizophrenics].

Eighty-five patients (52 male, 33 female, mean age 28.9 years) with slowly progressive schizophrenia with predominantly positive (pseudopsychopathic type, ICD-10 item F21.4)--79 patients and/or predominantly negative ("poor symptoms" schizophrenia, F20.6)--6 patients symptoms, were studied. All patients referred to the psychiatric clinic with refusal reactions. Depending on the psychological and behavioral mechanisms developing on the base of negative changes the refusal reactions have been divided into capitulation, avoidance and absenteeism subtypes. Their clinical heterogeneity was determined by the ratio of structure of schizophrenia-related personality disorders and cognitive deficit. Negative disorders in capitulation refusal reaction manifest in patients as "psychosthenic-like" personality with accentuation of dependent personality features and operational cognitive deficit. Avoidance subtype was observed in patients with schizoid (autistic or eccentric) personality changes and predominantly dynamic cognitive deficit. Absenteeism subtype was found in weak-will individuals and mainly motivational cognitive deficit.

[Spectrum of therapeutic efficacy and safety of Lorafen (lorazepam) use for anxiety disorders].

Presented here, are the results of 4-weeks lorafen monotherapy in 31 in-patients of psychiatric and cardio-surgical clinics divided, respectively, into 2 groups: 16 patients with chronic anxiety disorders and 15--with acute developed before an coronary artery bypass surgery in ischemic heart disease. A dynamics of the patients' state was evaluated with Hamilton Rating Scale for Anxiety and the Hospital Anxiety and Depression Scale. Lorafen proved to be highly effective, with percentage of responders being about 77% (24 patients out of 31). Therapeutically effective dosage was 3.7 +/- 1.1 mg daily for chronic anxiety disorders and 2.1 +/- 0.6 mg--for situation-induced anxiety. Lorafen was shown to be tolerable and safety.

[Clinical aspects of "folie du doute"].

"Folie du doute" related to obsessive disorders manifests with doubts about managing habitual actions, reality of what is happening and existence of surrounding subjects; ruminations, indecision emerged as incapability to choose a definite behavior line. Sixty-six patients, 27 men and 39 women (mean age 31.3 years) have been examined. Three clinical "folie du doute" variants were distinguished: "repeated control" obsessions (doubts are realized by re-control of the actions committed); obsessive doubts defined by contrastive ideas of expedience for choosing the approach contrastive to the taken decision; anxious ruminations (lack of assurance in the past actions and obsessive doubts of the variants supposed-to-be). "Folie du doute" emerged in different psychic disorders, the first variant manifesting in the scope of neurotic-like schizophrenia; the second one--in psychogenic states or psychogenically-provoked exacerbations of psychopathic-like schizophrenia; the third one being regarded in the structure of cyclothymiac's depressive phases. In the clinical contest, "folie du doute" manifestation proves to be unfavorable prognostic marker.

[The psychopathology of depersonalization depression].

77 patients (29 men, 48 women) at the age of 17-51 years with depersonalisation depression (DD) of nonpsychotic level with duration 6 months and longer. Such depressions were psychopathologically characterized by close connection between affective and depersonalizing components of the syndrome. The depressive component manifested with a mild hypothymia with dissociation between subjective severity and objective self-estimation, vulnerability to provoking factors, inversion of somatic symptoms that permit to evaluate affective disorders as "atypical depression" (according to DSM-IV). Peculiarities of depersonalizing component were relatively low degree of the alienation phenomena and their psychopathic (hysteric) colour. Personal status of the patients before disease's manifestations was characterized by some signs of accentuation of a self-consciousness sphere and corresponded to borderline disorders. The conclusion was made about the existence of a particular type of DD in which symptom complexes, relatively independent (affective and depersonalizing), had a mutual affinity; so they united according to mechanism of pathologic synergism. This type of depression is only one of variants of DD.

[The syndrome of depressive depersonalization].

Nineteen patients (9 men, 10 women aged 22--38 years) with depressions were examined. The phenomena of anesthesia of ideatoric functions dominated in clinical picture of such depression. This depression is defined as the depression of estrangement. Psychopathologic differentiation of mental estrangement in the structure of depression and in similar disorders of self-consciousness which were formed in depressions under conditions of "transitional syndrome" (according to G.Gross) is outlined. This syndrome is characterised by irreversible negative disorders of "defective depersonalization". The following signs permit to distinguish depression of estrangement from "transitional syndrome": partiality of the estrangement's phenomenon; connection between psychopathologic formations and pathology of imagination (figurative expressiveness, demonstrativity, lability to psychogenic and medical actions); conformity of the syndrome's structure and characteristics of premanifested personality's structure (combination of hyperthymic features with histrionic and/or narcissic ones). "Apperceptive anesthesia" is suggested for designation of the variant of the depression described.

[Visceral manifestations of Wilson-Konovalov disease (analysis of 22 cases)].

Twenty-two patients with Wilson-Konovalov disease (WKD) had not only hepatic but also visceral symptoms: renal affection, cutaneous vasculitis, arthralgias, endocrine disorders, etc. In 8 of these 22 patients extrahepatic symptoms appeared 10 years before hepatic signs. The latter were distinguished by the frequency of edematic-ascitic syndrome, hemolytic episodes, hemorrhagic syndrome; low activity of serum aminotransferase. Initial neurological symptoms were registered in 6 patients only. 12 patients benefited from copper-eliminating therapy. 4 patients had advanced process which caused death. To detect WKD early, special diagnostic tests should be conducted in all subjects under 45 with hepatic symptoms or WKD-specific visceral manifestations. Examination of the patients' sibs is also desirable.

[Mitochondrial pathology].

Three adult patients with mitochondrial disease underwent clinical-morphological studies which demonstrated a diffuse and marked defect in mitochondria. The characteristic clinical picture of "ophthalmoplegia plus", a slow progression of the disease, the quantitative and structural changes in the mitochondria of muscle fibers justified the diagnosis of mitochondrial disease.

[Clinical and genetic heterogeneity of the Ehlers-Danlos syndrome].

A clinical description of Type I and II Ehlers-Danlos syndrome in a 3-year-old girl and a 19-year-old boy is presented. The syndrome is characterized by skin hyperelasticity, articular overextensibility, hemorrhagic manifestations and also by excessive vulnerability of tissues. Clinical descriptions are supplemented with roentgenologic, morphologic and electron microscopic findings as well as with the results of amino-acid analysis and of dermatoglyphic examination of the patient and her parents.

[Sporadic distal myopathy].

The presented description of two cases of sporadic distal myopathy represents the first report on this disease published in the national literature. The clinical picture of the disease was consistent with that of five patients with this disease entity described in the world literature and was characterized by the disease onset in young age expressed in the form of distal muscular atrophies, a dramatic increase in the activity of serum creatine kinase, with electromyographic, histologic and electron microscopic findings indicating the presence of the myopathic process. A differential diagnosis from other diseases manifesting themselves in distal muscular atrophies is also outlined.
