A novel type of autosomal recessive syndactyly: clinical and molecular studies in a family of Pakistani origin.

Non-syndromic syndactylies have been classified into five major types (I-V), all showing autosomal dominant mode of inheritance. Later, the classification was extended and three additional variants (VI-VIII) were defined. Type VII, the Cenani-Lenz syndactyly, is the only non-syndromic, autosomal recessive type. It is characterized by fusion of all phalanges with metacarpal synostosis, dislocated and dysplastic carpals and infrequently, radio-ulnar fusion. Here, we present a Pakistani family with a novel non-syndromic autosomal recessive syndactyly manifesting a unique combination of clinical features. In both hands, reduction of certain phalanges is evident. Radiological examination shows synostosis of third and fourth metacarpals bearing single phalanges. The first three toes are webbed, with hypoplastic terminal phalanx in all the toes. Besides Cenani-Lenz syndactyly, the phenotype segregating in our family is the second well-documented autosomal recessive, non-syndromic syndactyly. A phenotype similar to our family was described in a Turkish kindred but was considered to be a homozygous expression of type I syndactyly. Since the clinical features in our family had minimal overlap with syndactyly types I, II, and III, we have performed microsatellite marker screening to look for the cosegregation of this phenotype with any of the known loci for these respective types. We show that the phenotype in our family is not linked to chromosomal regions 2q34-q36, 2q31, and 6q22-q23 encompassing loci for syndactyly types I, II, and III.

Diclofenac residues as the cause of vulture population decline in Pakistan.

The Oriental white-backed vulture (OWBV; Gyps bengalensis) was once one of the most common raptors in the Indian subcontinent. A population decline of >95%, starting in the 1990s, was first noted at Keoladeo National Park, India. Since then, catastrophic declines, also involving Gyps indicus and Gyps tenuirostris, have continued to be reported across the subcontinent. Consequently these vultures are now listed as critically endangered by BirdLife International. In 2000, the Peregrine Fund initiated its Asian Vulture Crisis Project with the Ornithological Society of Pakistan, establishing study sites at 16 OWBV colonies in the Kasur, Khanewal and Muzaffargarh-Layyah Districts of Pakistan to measure mortality at over 2,400 active nest sites. Between 2000 and 2003, high annual adult and subadult mortality (5-86%) and resulting population declines (34-95%) (ref. 5 and M.G., manuscript in preparation) were associated with renal failure and visceral gout. Here, we provide results that directly correlate residues of the anti-inflammatory drug diclofenac with renal failure. Diclofenac residues and renal disease were reproduced experimentally in OWBVs by direct oral exposure and through feeding vultures diclofenac-treated livestock. We propose that residues of veterinary diclofenac are responsible for the OWBV decline.

Sensitivity and spectrum of bacterial isolates in infectious otitis externa.

OBJECTIVE: To determine the spectrum and antibiotic sensitivity pattern of organisms from aural swabs taken from cases of infectious otitis externa. DESIGN: Descriptive study. PLACE AND DURATION OF STUDY: Department of ENT, Combined Military Hospital, Peshawar, from May 2002 to April 2003. SUBJECTS AND METHODS: One hundred and twenty-four patients, diagnosed clinically as infectious otitis externa without concomitant suppurative otitis media, perforated tympanic membrane or otomycosis, were selected for the study. Samples of pus from external auditory canal were taken on sterile cotton swabs and were cultured on blood agar and MacConkey Agar for 24 to 48 hours. Sensitivity was tested and interpreted by Kirby-Bauer disc diffusion method of grading zones of inhibition. RESULTS: One hundred and twenty four subjects were cultured. Sixteen samples showed no growth. One hundred and eight specimens yielded growth of bacteria. No specimen revealed multiple organisms. Staphylococcus aureus was found in 38%, and Pseudomonas aeruginosa was found in other 38%. Others included Proteus, Enterococci, Klebsiella, and E. coli. Majority of organisms were resistant to co-trimoxazole, amoxicillin and erythromycin, while 100% were sensitive to imipenem. Sensitivity to enoxacin was 96%, Ninety-two percent were sensitive to ciprofloxacin and ofloxacin. CONCLUSION: Pseudomonas aeruginosa and Staphylococcus aureus were the most common bacterial isolates. Both were sensitive to fluoroquinolones. The latter can be used empirically for treatment of acute infectious otitis externa.

Did Michelangelo (1475-1564) have high-functioning autism?

In this paper evidence is presented that Michelangelo met the criteria for Asperger's disorder, or high-functioning autism. The evidence relates to his single-minded work routine, unusual lifestyle, limited interests, poor social and communication skills, and issues of life control. Depression and various medical conditions, including gout, renal colic and renal stones, did not stop his obsessive working habits.

Biodegradation kinetics of endosulfan by Fusarium ventricosum and a Pandoraea species.

Endosulfan, classified as an organochlorine pesticide, is rated by the U.S. EPA as a Category 1 pesticide with extremely high acute toxicity. This study describes the biodegradation kinetics of endosulfan and the metabolic pathway utilized by Fusarium ventricosum and a Pandoraea sp. Complete disappearance of both alpha- and beta-endosulfan was observed during 12 days of incubation with F. ventricosum in flasks containing 100 mg L(-)(1) of endosulfan. The rate constants (k) for biodegradation of alpha- and beta-endosulfan by F. ventricosum using zero-order kinetics were 14.22 and 6.60 mg L(-)(1) day(-)(1), respectively. The Pandoraea sp. degraded about 95 and 100% of alpha- and beta-endosulfan, respectively, in 18 days of incubation in flasks spiked with 100 mg L(-)(1) of endosulfan. The rate constants (k) for biodegradation of alpha- and beta-endosulfan by the Pandoraea sp. were 8.19 and 3.78 mg L(-)(1) day(-)(1), respectively. Both fungal and bacterial strains formed less toxic endosulfan diol and endosulfan ether as metabolites during metabolism of endosulfan. The results of this study suggest that these novel strains may be used for the bioremediation of endosulfan-contaminated sites.

Pentose phosphate pathway coordinates multiple redox-controlled relaxing mechanisms in bovine coronary arteries.

Pentose phosphate pathway (PPP) inhibitors, 6-aminonicotinamide (6-AN) and epiandrosterone (Epi), were employed to examine whether changes in NADP(H) redox regulates contractile force in endothelium-removed bovine coronary arteries (BCAs). 6-AN (0.01-5 mM) or Epi (1-500 microM) elicited dose-dependent relaxation in BCAs contracted with 30 mM KCl, 0.1 microM U-44619, and endothelin-1 but not with phorbol 12,13-dibutyrate, a protein kinase C activator that causes Ca2+-independent contraction. Relaxation to PPP inhibition was associated with oxidation of NADPH and glutathione (GSH). Relaxation to 6-AN was not mediated by H2O2, because it was not altered by hypoxia or the peroxide scavenger ebselen (100 microM). The thiol reductant DTT (3 mM) attenuated the relaxation to 6-AN and Epi by 30-40%. Inhibition of glycolysis or mitochondrial electron transport did not elicit relaxation in BCAs contracted with 30 mM KCl, suggesting these pathways may not be involved in relaxation elicited by PPP inhibition. High doses of K+ channel blockers [e.g., TEA (10 mM) and 4-aminopyridine (10 mM)] only partially inhibited the relaxation to 6-AN. On the basis of changes in the fura-2 fluorescence ratio, 6-AN and Epi appeared to markedly reduce intracellular Ca2+. Thus PPP inhibition oxidizes NADPH and GSH and appears to activate a novel coordination of redox-controlled relaxing mechanisms in BCAs mediated primarily through decreasing intracellular Ca2+.

A novel autosomal recessive non-syndromic deafness locus (DFNB35) maps to 14q24.1-14q24.3 in large consanguineous kindred from Pakistan.

Autosomal recessive nonsyndromic deafness is one of the most frequent forms of inherited hearing impairment. Over 30 autosomal recessive nonsyndromic hearing loss loci have been mapped, and 15 genes have been isolated. Of the over 30 reported autosomal recessive nonsyndromic hearing loss (NSHL) loci, the typical phenotype is prelingual non-progressive severe to profound hearing loss with the exception of DFNB8, which displays postlingual onset and DFNB13, which is progressive. In this report we describe a large inbred kindred from a remote area of Pakistan, comprising six generations and segregating autosomal recessive nonsyndromic prelingual deafness. DNA samples from 24 individuals were used for genome wide screen and fine mapping. Linkage analysis indicates that in this family the NSHL locus, (DFNB35) maps to a 17.54 cM region on chromosome 14 flanked by markers D14S57 and D14S59. Examination of haplotypes reveals a region that is homozygous for 11.75 cM spanning between markers D14S588 and D14S59. A maximum two-point LOD score of 5.3 and multipoint LOD score of 7.6 was obtained at marker D14S53. The interval for DFNB35 does not overlap with the regions for DFNA9, DFNA23 or DFNB5.

Temperature and pH effects on biodegradation of hexachlorocyclohexane isomers in water and a soil slurry.

This study was conducted to monitor the biodegradation of alpha-, beta-, gamma-, and delta-hexachlorocyclohexane (HCH) isomers in liquid culture by a Pandoraea species and determine the influence of pH and temperature on the biodegradation of alpha- and gamma-HCH in liquid as well as in soil slurry cultures. The Pandoraea species degraded 79.4% delta-HCH and 34.3% gamma-HCH in liquid culture at 4 weeks of incubation. alpha- and beta-HCH exhibited almost identical rates (41.6 and 42.4%, respectively) of degradation. The highest degradation of alpha- and gamma-HCH (67.1 and 60.2%, respectively) was observed at an initial pH of 8.0 in liquid; 58.4 and 51.7% rates of degradation of alpha- and gamma-HCH, respectively, at an initial pH of 9.0 were found in soil slurry cultures. An incubation temperature of 30 degrees C was optimum for effective degradation of alpha- and gamma-HCH isomers (62.5 and 57.7%, respectively) in liquid culture, and 54.3 and 51.9% rates of degradation of alpha- and gamma-HCH isomers, respectively, were found in a soil slurry. Increasing the soil/water ratio decreased the extent of degradation of both HCH isomers. Degradation of HCH isomers occurred concomitant with bacterial growth. Byproducts of growth from Pandoraea species significantly decreased the pH of the liquid and the soil slurry during the growth on HCH isomers. The results of this study suggest that this bacterial strain may effectively be used for remediating polluted sites and water contaminated with different HCH isomers over a range of environmental conditions.

Reduced duration of muscle relaxation with rocuronium in a normocalcemic hyperparathyroid patient.

PURPOSE: To report a case of reduced duration of action of rocuronium in a patient with normocalcemic hyperparathyroidism (HPT). CLINICAL FEATURES: A 56-yr-old patient with primary HPT, who had had surgical resection of three and a half parathyroid glands nine months previously, was referred to our institution for further investigation of a persistent increase in parathyroid hormone. Preoperatively, the patient had a normal serum ionized and total calcium. The patient was diagnosed with a persistent parathyroid adenoma and was scheduled for an elective parathyroidectomy. General anesthesia was induced with iv propofol, fentanyl and succinylcholine. Intraoperatively, anesthesia was maintained with nitrous oxide in oxygen, and isoflurane. Neuromuscular blockade was attained using incremental doses of rocuronium. The average duration of 0.15 mg x kg(-1) incremental doses of rocuronium was 5.9 min (expected: 13-18 min), and that of 0.2 mg x kg(-1) was ten minutes (expected: 19-23 min). CONCLUSION: Primary HPT even in the absence of hypercalcemia may result in resistance to competitive blockade by rocuronium. It suggests that primary HPT may cause acetylcholine receptor up-regulation resulting in hyposensitivity to non-depolarizing muscle relaxants.

Enrichment and isolation of endosulfan-degrading microorganisms.

Endosulfan (6,7,8,9,10,10-hexachloro-1,5,5a,6,9,9a-hexahydro-6,9-methano-2,3,4-benzo-dioxathiepin-3-oxide) is a cyclodiene organochlorine currently used as an insecticide all over the world and its residues are posing a serious environmental threat. This study reports the isolation and identification of enriched microorganisms, capable of degrading endosulfan. Enrichment was achieved by using the insecticide as either the sole source of carbon or sulfur in parallel studies. Two strains each of fungi (F1 and F4) and bacteria (BF2 and B4) were selected using endosulfan as a sole carbon source. A Pandoraea species (Lin-3) previously isolated in our laboratory using lindane (gamma-HCH) as a carbon source was also screened for endosulfan degradation. F1 and F4 (Fusarium ventricosum) degraded alpha-endosulfan by as much as 82.2 and 91.1% and beta-endosulfan by 78.5 and 89.9%, respectively, within 15 d of incubation. Bacterial strains B4 and Lin-3 degraded alpha-endosulfan up to 79.6 and 81.8% and beta-endosulfan up to 83.9 and 86.8%, respectively, in 15 d. Among the bacterial strains isolated by providing endosulfan as a sulfur source, B4s and F4t degraded alpha-endosulfan by as much as 70.4 and 68.5% and beta-endosulfan by 70.4 and 70.8%, respectively, after 15 d. Degradation of the insecticide occurred concomitant with bacterial growth reaching an optical density (OD600) of 0.366 and 0.322 for B4 and Lin-3, respectively. High OD600 was also noted with the other bacterial strains utilizing endosulfan as a sulfur source. Fungal and bacterial strains significantly decreased the pH of the nutrient culture media while growing on endosulfan. The results of this study suggest that these novel strains are a valuable source of potent endosulfan-degrading enzymes for use in enzymatic bioremediation.
