[Application of laser-assisted in situ keratomileusis in glaucoma patients].

Should glaucoma patients receive laser-assisted in situ keratomileusis (LASIK) or not present a challenge to physicians. Corneal thickness and corneal curvature, which are modified during corneal refractive surgery, plays an important role in diagnosis and therapy in glaucoma patients because of its effect on the measured intraocular pressure. On the same time, patients undergo a transient but significant rise in intraocular pressure during the laser-assisted in situ keratomileusis procedure with risk of further optic nerve damage or retinal vein occlusion; steroids typically used after refractive surgery, can increase intraocular pressure in steroid responders, which is more prevalent among glaucoma patients. Glaucoma may not be a contraindication to LASIK, but it is a relative contraindication until its safety is studied more extensively.

Evidence-based management of glaucoma.

PURPOSE: The evidence-based medicine (EBM) is a new medical mode that is completely different from traditional experience-based medicine. The core of EBM is that decision-making during clinical practices must be based on objective research results. With the rapid development of modern ophthalmology, some former viewpoints according to experience-based medicine face challenges. Evidence-based ophthalmology (EBO) is imperative under the situation. Glaucoma, as a disease resulting in blindness, which can not be cured by operation, is thought more and more of by medical management officials and doctors. One challenge for ophthalmologists entering the 21st century will be to make clinical decisions based on valid information or evidence rather than intuition, hearsay, or peer practice. How can we do? Here reviews the application of EBM in medical management of glaucoma since EBM' s naissance and give a reference.

[Proteomics in ophthalmology].

Proteomics is the study of total expressed proteins of genome in certain cell, tissue or organ. It is the frontier of the research in life science in the new century, and the core of functional genomics. Proteomics provides a new way to study ocular cell behaviors and the mechanisms of ocular disease. Here we reviewed the applications of proteomics in ophthalmology.

The application of topical anesthesia combined with subconjunctival anesthesia for glaucoma surgery.

PURPOSE: To evaluate the feasibility, reliability and analgesia effect of topical anesthesia combined with subconjunctival anesthesia in anti-glaucomatous surgery. METHODS: Two hundred and four cases (357 eyes) underwent anti-glaucomatous surgeries under topical anesthesia with 0.5% Alcaine eye drops combined with subconjunctival anesthesia with 2% Lidocaine. The analgesic effect was analysed with visual analogue pain scale. RESULTS: Among all of 357 eyes, 62 eyes underwent peripheral iridectomy, 67 eyes underwent simple trabeculectomy, 167 eyes underwent compound brabeculectomy and 12 eyes nonpenetrating trabecular surgery. The effects of anesthesia were as follows: 304 eyes (85.2%) were painless (Grade I), 50 eyes (14.0%) were slight painful (Grade II), and 3 eyes (0.8%) were more painful (Grade III) during surgery. And no severe complications were observed in all the cases during surgery and postoperatively. Amaurosis fugax was not observed in the glaucoma patients at the late stage with narrow visual fields and poor visual ability. CONCLUSION: Topical anesthesia combined with subconjunctival anesthesia is effective, safe and simple anesthesia alternative in routine anti-glaucomatous surgery, especially for the glaucoma patients at the late stage with narrow visual fields and poor visual ability. It is worthy of being applied widely.

[Application of endoscopic operation in ophthalmology].

The application of endoscopic operation in ophthalmology has gotten great progression in recent years. It has showed many advantages such as more effectiveness, reliability and less damage, complications. Ophthalmic laser endoscopic operations can be applied in the management of glaucoma, vitreoretinopathy and so on. Lacrimal endoscopy in the treatment of lacrimal apparatus disease and nasal endoscopy in the Ophthalmo-Rhinol associated surgeries also has got great development. The application of ophthalmic endoscopic operation has indicated that the period of mini-wounded surgery in ophthalmology began. In this paper, we will describe the basic principle, status of application and prospect of endoscopic operation in ophthalmology.

The management of secondary glaucoma in nanophthalmic patients.

PURPOSE: To investigate the clinical characteristcs, management of secondary glaucoma in nanophthalmos, and the prevention of its complications. METHODS: Retrospectively, 9 cases (17 eyes) with nanophthalmic glaucoma were studied. RESULTS: The axial length of the eyes ranged (14.36 - 19.33) mm; All of the cases combined with hyperopia ranged (+7.00 +/- 16.00) D. All 17 eyes had the manifestation like angle-closure glaucoma. The glaucoma was controlled in 9 of 17 eyes at the early stage, which underwent laser iridotomy (4 of 9 eyes also underwent laser iridoplasty). 1 eye underwent ciliary photocoagulation because its visual acuity was lost and the patient complained of pain. The other 7 eyes underwent filtration surgery and 3 of them had permanent loss of vision caused by disastrous complications after the surgery. CONCLUSIONS: Management of secondary glaucoma in nanophthalmos is complicated. The laser iris surgery is safe and effective in glaucoma at the early stage. Vortex vein decompression, sclerotectomy or anterior sclerotomy may be performed to reduce disastrous complications.
