Multiple pulse generator malfunctions with a dual chamber pacemaker.

The aim of this study was to evaluate the dual chamber uni/bipolarpacemaker Minidual 50, manufactured by Sorin Biomedica. Between 1995 and 1998, 66 Minidual 50 models were implanted at the Heart Institute. During the follow-up period of 33 +/- 12.8 months (range 0-50 months), total function loss in seven (10.6%) units and false threshold measurement of sensing and pacing in three (4.5%) patients were observed. Average time from implantation to malfunction was 37 months (range 28-42). Malfunction was unrelated to battery status and could not be predicted by any measures obtained during the pacemaker follow-up period. Kaplan Meyer survival curve predicted a 70% 4-year malfunction-free survival of that pacemaker model. Given this high rate of total malfunction and the unpredictable nature of its occurrence, the authors recommend the replacement of all remaining Minidual 50 units at risk, at least in dependent patients.

The area of the pressure-flow loop for assessment of arterial stenosis: a new index.

This study describes a novel method for assessing stenotic severity, based on simultaneous pressure and flow wave measurements. Pressure and flow measurements were performed in latex and rubber tubes, and in a clinically-used vascular graft. Pressure waves were recorded at several degrees of stenosis and at different distances proximal to the stenosis. Pressure wave versus flow wave was plotted. Internal pressure-flow loop area (PFLA), loop slope and pressure-axis intercept were calculated. Values of these three indices significantly increased with increasing degrees of stenosis P < 0.001). Similar phenomenon was observed during in-vivo experiments. Polynomial functions were fitted, resulting in an excellent PFLA variable/ percent stenosis correlation, independent of distance between sensor and stenosis (R2 > 0.96). In addition, tube compliance was measured and found to correlate with the polynomial coefficients (/R/ > 0.9). This innovative approach could significantly contribute to detecting and evaluating arterial stenoses, and to characterize the elasticity of the artery.

Effect of basic fibroblast growth factor on left ventricular geometry in rats subjected to coronary occlusion and reperfusion.

BACKGROUND: Previous studies have demonstrated myocardial salvage by basic fibroblast growth factor administration following chronic myocardial ischemia or acute myocardial infarction. OBJECTIVES: To study the effect of bFGF on left ventricular morphometry following coronary occlusion and reperfusion episode in rats. METHODS: bFGF (0.5 mg) or placebo was continuously administered for a period of one week using an implanted osmotic pump. Animals were sacrificed 6 weeks after surgery and myocardial cross-sections were stained with Massontrichrome and with anti-proliferating cell nuclear antigen antibody. RESULTS: LV area, LV cavity diameter, LV cavity/wall thickness ratio, and injury size were unchanged compared with control animals. Proliferating endothelial cells were significantly more abundant in injured compared with normal myocardium, but with no differences between animals treated or not treated with bFGF. CONCLUSIONS: One week of systemic bFGF administration following coronary occlusion and reperfusion had no additional effect on LV geometry or cellular proliferation in rats.
