Treatment planning for the layer-stacking irradiation system for three-dimensional conformal heavy-ion radiotherapy.

We have upgraded a heavy-ion radiotherapy treatment-planning system to adapt for the layer-stacking irradiation method, which is to conform a variable spread-out Bragg peak to a target volume by means of dynamic control of the conventional beam-modifying devices. The biophysical model, the beam-setup logic, and the dose-calculation algorithm implemented for the layer-stacking method are described and the expected clinical usability is discussed. The layer-stacking method was integrated in perfect accordance with the ongoing conventional treatments so that the established protocols, which are the clinically optimized dose fractionation schemes, will still be valid. On the other hand, a simulation study indicated a substantial improvement of dose distribution with the layer-stacking method though the significance may depend on the size, shape, and location of the tumor. The completed treatment system will provide an option for improved conformal radiotherapy without interfering with the conventional method and we expect a gradual expansion of the clinical cases applicable to the layer-stacking method.

Characterization of a class II defective transposon carrying two haloacetate dehalogenase genes from Delftia acidovorans plasmid pUO1.

The two haloacetate dehalogenase genes, dehH1 and dehH2, on the 65-kb plasmid pUO1 from Delftia acidovorans strain B were found to be located on transposable elements. The dehH2 gene was carried on an 8.9-kb class I composite transposon (TnHad1) that was flanked by two directly repeated copies of IS1071, IS1071L and IS1071R. The dehH1 gene was also flanked by IS1071L and a truncated version of IS1071 (IS1071N). TnHad1, dehH1, and IS1071N were located on a 15.6-kb class II transposon (TnHad2) whose terminal inverted repeats and res site showed high homology with those of the Tn21-related transposons. TnHad2 was defective in transposition because of its lacking the transposase and resolvase genes. TnHad2 could transpose when the Tn21-encoded transposase and resolvase were supplied in trans. These results demonstrated that Tn Had2 is a defective Tn21-related transposon carrying another class I catabolic transposon.

[Analysis of IgE reactivities of purified allergens from Candida albicans and Malassezia furfur among patients with atopic dermatitis].

We analyzed the reactivities of a series of purified allergens from Candida albicans (C. albicans) and Malassezia furfur (M. furfur) with IgE antibodies in sera from patients with atopic dermatitis. We compared the specific IgE antibody levels to manganese superoxide dismutase (Mn SOD), cyclophilin, enolase, secretory aspartic protease (SAP 2) and type A mannan from C. albicans and Mn SOD, cyclophilin and Mal f 2 from M. furfur in 21 sera from patients with atopic dermatitis and 20 sera from patients with asthma without atopic dermatitis. The prevalence of IgE antibodies and the mean IgE antibody levels to all of the allergens tested were higher among patients with atopic dermatitis than among those with asthma without atopic dermatitis. More than 50% of patients with atopic dermatitis were IgE antibody-positive to Mn SOD, cyclophilin and type A mannan from C. albicans, and Mn SOD and cyclophilin from M. furfur. The availability of these purified allergens will facilitate studies on the contribution of fungal allergens to the development of atopic dermatitis.

[The role of transmyocardial laser revascularization in congestive heart failure].

Transmyocardial laser revascularization (TMLR) has been approved as an indirect coronary revascularization measure through angiogenesis around created channels in patients who are not amenable to direct revascularization methods such as coronary artery bypass grafting (CABG) or percutaneous transluminal coronary angioplasty (PTCA). TMLR is less invasive and there are no contraindications in terms of left ventricular function. Therefore all patients who have untreatable lesions by CABG or PTCA with reversible ischemia are candidates for TMLR therapy. In a Japanese clinical trial, improvement of left ventricular function associated with relief of persistent angina and improved perfusion was seen in 54% of patients. Significant reduction of operative risk in adjunctive TMLR (combination of TMLR with CABG) compared with isolated CABG has been also demonstrated in a randomized trial. These results indicate the usefulness of adjunctive TMLR in multivessel-disease patients with left ventricular dysfunction. Because TMLR is a simple and less-invasive technique, combined use of TMLR with off-pump CABG or MIDCAB is also an attractive revascularization strategy in ischemic cardiomyopathy patients.

[Improvement of dose distributions at NIRS's 70 MeV proton eye treatment beam course]

In order to improve dose distributions at NIRS's 70 MeV proton eye treatment beam course, we introduced finer bar ridge filters, and examined the effects of range compensators. The pitch of new bar ridge filters was 5mm in contrast to 15mm pitch of old ones. A NC-machine recently available enabled this refinement. The spread out Bragg peak (SOBP) widths were 10, 15, 20 and 30 mm. The new ridge filters improved the field uniformity considerably. In the ridge filter design we assumed parallel beam condition in which the mono-energetic proton should proceed in parallel with the central axis, and bar ridges only changes the proton ranges. We searched empirically for the optimum wobbler radius in view of field flatness and depth dose distribution. Range shifter and compensator did not affect the field flatness and depth dose distribution at the optimum condition thus searched. We measured dose distribution in a phantom using a compensator of stairs-shape, which fairly modulated the beam. A 50% isodose line almost coincided with the compensator shape, and these results suggested that improvements of dose distributions should be possible using compensators. However width between 50% and 80% isodose lines depended on the thickness of phantom. This might be due to scattering in the compensator and suggests that it is necessary to calculate dose distribution taking account of such effects.

Computed tomographic findings of syphilitic aortitis.

We describe the computerized tomographic (CT) findings of the aortic wall in a case of acute-phase syphilitic arteritis. The delayed phase of the contrast-enhanced CT shows a double-ring configuration of the thick thoracic aortic wall, which is similar to CT findings previously reported for Takayasu arteritis. We speculate that the resemblance of the CT findings for these two diseases accounts for their similar histopathological features.

Increased susceptibility of the left compared to the right ventricle to remote ischemia/reperfusion injury in human C1-inhibitor-overexpressing transgene mice.

Acute myocardial injury has been demonstrated as a remote sequela of severe lower torso ischemia-reperfusion (I/R) due to proinflammatory events. In a model of I/R injury, administration of C1 esterase inhibitor (C1-Inh) reduces myocardial necrosis. We investigated the susceptibility of the left (LV) versus right ventricle (RV) and the protective effect of transgenic C1-Inh-overexpressing mice. Two groups of mice (n = 6) underwent a 2-h lower torso ischemia followed by 3 h of reperfusion: transgenic and wild type with sham-operated controls. Animals were then injected with (125)I bovine albumin. Heart was removed and samples from right and left ventricular free wall were harvested, weighted, and radioactivity was determined. Permeability index for wild-type animals in the RV was 0.22 +/- 0.04, compared to 0.17 +/- 0.07 in controls (NS), and in the LV 0.36 +/- 0.08, compared to 0.21 +/- 0.05 in controls (p <.01). The LV showed a significantly higher value compared to the right (0.22 +/- 0.04 vs. 0.36 +/- 0.08, p <.01). No difference was seen in the RV between transgenic and wild-type mice; however, in the LV the values decreased significantly in transgenic animals (p <.015). Thus, remote myocardial injury after lower torso I/R is present in both ventricles; however, the LV seems to be more susceptible as assessed by albumin permeability. Inhibition of the classic complement cascade may be a promising therapeutic approach for myocardial protection in reperfusion injury.

The influence of granisetron on the pharmacokinetics and pharmacodynamics of docetaxel in Asian lung cancer patients.

BACKGROUND: Docetaxel, which undergoes hepatic metabolism via cytochrome P450 3A4, is a promising anticancer agent. Toxicity is serious problem, however, because it is difficult to predict the cytochrome P450 3A4 activity of the drug. Moreover, drug-drug interactions involving cytochrome P450 3A4 enzymes are important. Granisetron, a selective antagonist of the 5-hydroxytryptamine3 receptor, also undergoes hepatic metabolism via cytochrome P450 3A4. In this study, we investigated the influence of granisetron on the pharmacokinetics and pharmacodynamics of docetaxel in Asian patients with lung cancer. METHODS: Six patients with advanced lung cancer were treated with doses of docetaxel (60 mg/m2). In the first course of treatment, no antiemetic agents were administered. In the second course, all patients received 3.0 mg of granisetron before 30-minute administration of docetaxel. In each of the treatment courses, blood samples (5 mL) were obtained for pharmacokinetic study at the following times: 0, 0.5, 1.5, 2.0, 3.0, 5.0, 8.0, and 24 hours after the start of the docetaxel infusion. RESULTS: Six patients were enrolled in this pharmacokinetics study. The mean +/- SD systemic clearance of docetaxel administered alone or in combination with granisetron was 32.9 +/ - 8.3 and 28.2 +/- 5.9, respectively. The area under the concentration-versus-time curve of plasma docetaxel (alone or in combination with granisetron) ranged from 1.355 to 2.773 and 1.647 to 3.079 microg x h/mL (mean +/- SD: 1.936 +/- 0.541 and 2.219 +/- 0.510 microg x h/mL), respectively. There was no significant difference in mean residence time (or invariance of residence time) between the single dose of docetaxel and the combination of docetaxel and granisetron. DISCUSSION: We found no significant difference in the pharmacokinetic and pharmacodynamic parameters of docetaxel between the single dose of docetaxel and the combination of docetaxel and granisetron. However, a wide interindividual variation existed in cytochrome P450 3A4 activity. It is clear that the results of the present study should be confirmed in a population study involving a larger number of subjects addressing the genetic variations of drug metabolizing enzymes, drug receptors, and drug transporters.

Volumetric cine imaging for cardiovascular circulation using prototype 256-detector row computed tomography scanner (4-dimensional computed tomography): a preliminary study with a porcine model.

This is a preliminary demonstration of volumetric cine imaging of cardiovascular circulation in domestic pigs using a prototype 256-detector row computed tomography (CT) scanner. The scan range is approximately 120 mm in the craniocaudal direction, with a 0.5-mm slice thickness. The thin sections can be used to create cine loops in multiple planes. Thus, the 256-detector row CT scanner overcomes some of the limitations of present helical CT methods.

Real-time volumetric imaging of human heart without electrocardiographic gating by 256-detector row computed tomography: initial experience.

OBJECTIVE: The feasibility of human cardiac imaging using a prototype 256-detector row cone-beam computed tomography (256CBCT) scanner without electrocardiographic gating was examined. METHODS: Two healthy male volunteers were examined by contrast-enhanced 256CBCT. The number of detectors was 912 x 256, each measuring approximately 0.5 mm x 0.5 mm at the center of rotation. The craniocaudal coverage was approximately 100 mm after reconstruction by the Feldkamp-Davis-Kress algorithm. The effective time resolution was 500 milliseconds using a half-scan algorithm. RESULTS: Serial enhancement of the left ventricular myocardium was detected. The right and left coronary arteries at proximal and distal segments were depicted without significant blurring. Although the left ventricular wall motion on cine images was not smooth over time, it was possible to measure ventricular volume and ejection fraction. CONCLUSIONS: Using the 256CBCT, it was possible to visualize the coronary arteries, myocardial perfusion, and ventricular contraction simultaneously during a single acquisition.

Characteristics of hypertrophic obstructive cardiomyopathy refractory to medical treatment and selection of surgical methods.

BACKGROUND AND AIM OF THE STUDY: Treatment for hypertrophic obstructive cardiomyopathy (HOCM) has been reported; however, there has been no report on the characteristics of medication-responsive and -refractory hypertrophic obstructive cardiomyopathy (HOCM). Using the classification of systolic anterior movement (SAM) which has been previously reported, we tried to identify the characteristics and use them to treat HOCM appropriately. METHODS: The clinical, echocardiographic, catheterization, and surgical data of 29 hospitalized patients with HOCM during 1980 to 1999 were analyzed retrospectively. We classified SAM in all patients by echocardiography. Nineteen patients improved with medical treatment (medical group), and 10 patients underwent surgical treatment because of ineffectiveness of medication (surgical group). We studied the relation between types of SAM and medical/surgical groups, and examined the relation between types of SAM and the surgical methods. RESULTS: Type I SAM was significantly more frequent in the medical group, while type II SAM was more frequent in the surgical group (p = 0.047). Patients in the surgical group underwent mitral valve replacement (MVR), myectomy, or a combination of MVR and myectomy. Left ventricular outflow gradient (LVOG) of over 100 mmHg was recognized in almost all patients with type II SAM. CONCLUSIONS: It was suggested that patients with medication-responsive HOCM tended to have type I SAM and those with refractory HOCM tended to have type II SAM. We consider that in type I SAM, if the position of the papillary muscles changed with medication or myectomy, shift of the chordae and type I SAM were reduced or disappeared. However, in type II SAM, even if the position of the papillary muscles changed, SAM did not disappear because lifting of the mitral leaflets remained. It is therefore suggested that patients with type II SAM should undergo at least MVR.

Chronic expanding intrapericardial hematoma after pericardial paracentesis.

Chronic expanding intrapericardial hematoma (CEIH) is rare. A thorough search of literature revealed a few cases after open heart surgery, chest trauma, or epicardial injury. We report the case of a 72-year-old man presenting with a large CEIH and constrictive pericarditis, who had no past history of the above conditions but had undergone pericardiocentesis four years earlier. The cause of the initial hemorrhage of the hematoma might be due to a scratch by a needle during pericardial paracentesis. The hematoma had expanded during a course of four years. The hematoma was removed surgically, and there has been no sign of recurrence 17 months after the operation.

Four-dimensional computed tomography (4D CT)--concepts and preliminary development.

Four-dimensional computed tomography (4D CT) is a dynamic volume imaging system of moving organs with an image quality comparable to that of conventional CT. 4D CT will be realized by several technical breakthroughs for dynamic cone-beam CT: (1) a large-area two-dimensional (2D) detector; (2) high-speed data transfer system; (3) reconstruction algorithms; (4) ultra-high-speed reconstruction computer; and (5) high-speed, continuously rotating gantry. Among these, development of the 2D detector is one of the main tasks because it should have as wide a dynamic range and as high a data acquisition speed (view rate) as present CT detectors. We are now developing a 4D CT scanner together with the key components. It will take one volume image in 0.5 sec with a 3D matrix of 512 x 512 x 512. This paper describes the concepts and designs of the 4D CT system, as well as preliminary development of the 2D detector.

Volumetric perfusion CT using prototype 256-detector row CT scanner: preliminary study with healthy porcine model.

This is a preliminary demonstration of volumetric perfusion CT of the brain in domestic pigs by using a prototype 256-detector row CT. Scan range is approximately 100 mm in the craniocaudal direction with 0.5-mm section thickness. The 256-detector row CT is an easily available imaging technique that can provide volumetric cine imaging, CT angiography, and perfusion CT in a wide craniocaudal coverage simultaneously.
