[Problems of diagnostic origin in tumors of the small intestine developing in the smooth muscle. Report on 7 cases].

The smooth muscle tumors of the small intestine are particularly interesting for the difficulty and the delay of their diagnosis. This usually happened for the poor and indefinite symptomatology and the problematic exploration of the organ. Starting up from the observation of 7 patients between 1986 and 1989 the Authors critically examined the literature about this problem to value the diagnostic opportunities of the principal clinical exams, suggesting as resolutive investigation, if indicated, the angiographic selective study of the visceral abdominal arteries.

Syphilitic aneurysm of the abdominal aorta: report of two cases.

We have described two histologically confirmed syphilitic abdominal aortic aneurysms arising below the renal arteries. Serological tests were negative and the patients had in the past been treated with penicillin and bismuth. We consider that there may be a correlation between atherosclerosis and syphilis in the natural history of some abdominal aortic aneurysms and the hazard of indiscriminate use of antibiotics for the treatment of other inflammatory vessel disease may allow syphilis to develop insidiously.

Malignant mesothelioma in non-asbestos textile workers in Florence.

By means of a review of histological diagnosis in the Pathology Department of the University of Florence, suspected cases of malignant mesothelioma, diagnosed in the period 1979-1984, were identified. Study of histological specimens permitted the selection of 13 cases of malignant mesothelioma resident in the Province of Florence. To these cases, referents were matched for age, sex, and year of hospital admission, with residence weighted for the general population of the Province. Both cases and referents (or their next of kin) completed an occupational questionnaire detailing possible occupational exposures. Out of the 13 cases of mesothelioma, 6 were textile workers and 5 of these were "rag-sorters". There were only 5 textile workers among the 52 controls. No asbestos cloth production plants have been in operation in the area from which the cases and referents are derived. Possible sources of exposure to asbestos in the textile industry of this area are discussed.

An asbestos hazard in the reprocessed textile industry.

Epidemiologic studies have identified an excess risk of lung cancer and mesothelioma among workers in the reprocessed textile industry in Prato, Italy. These studies suggested that there may have been asbestos hazard in this industry although exposure was not known to exist. An industrial hygiene investigation was conducted to determine whether there was previous or current asbestos exposure in the industry. Walk-through surveys, environmental sampling, process documentation, and management and worker interviews were conducted in 13 textile reprocessing establishments. Polypropylene bags that once contained asbestos were found in 2 of the 13. Asbestos bags were cut open and used to cover bales of rags which were then distributed throughout the world. Workers were exposed to asbestos while handling the bags which were contaminated with chrysotile, amosite, and crocidolite. Additional sources of asbestos exposure that may have existed in the past in the industry are also discussed.

[Immunodiagnosis and immunotherapy of allergic forms].

The Authors outline the immunological and immunopathogenic bases of atopy, pointing out the possible mechanisms involved in the impaired control capacity on IgE synthesis (hyperreactivity of NK cells, histamine-H2 receptors interaction, adenylcyclase stimulation, etc.). The in vivo and in vitro techniques employed in the allergological diagnosis are examined, with particular reference to the more recent tests (RAST, RAST Spot Test, Histamine radioenzymatic assay, in vitro IgE spontaneous synthesis, etc.). The latest findings in the allergological immunotherapy are then surveyed, including both the antigen non specific immunotherapy and the relevant possibility of inducing a non specific suppressor mechanism of the IgE response, and the antigen specific immunotherapy, which, besides the classical (aqueous, retard) vaccines, exploits the monovalent allergenic extracts, the allergoids (formaldehyde and glutaraldehyde) and the modified allergens (urea, PEG/PVA, D-GL, polysaccharides, photooxydation).

Iron status and HFE genotype in erythrocyte pyruvate kinase deficiency: study of Italian cases.

We evaluated the iron status and searched for mutations C282Y and H63D in the hereditary hemochromatosis gene (HFE) in 34 pyruvate kinase (PK)-deficient patients from 29 unrelated families. Nine had received multiple transfusions. Thirteen of the 25 nontransfused patients displayed increased serum ferritin concentration, in the absence of conditions known to raise this parameter. HFE genotype was abnormal in 9 of 34 patients. The allele frequency was 1.8% for mutation 845G--> (C282Y) and 16.1% for mutation 187C-->G (H63D). Nontransfused subjects with abnormal genotype had serum ferritin and transferrin saturation values significantly higher than those with wild-type genotype. Of the 12 adult nontransfused patients with increased iron status parameters, 1 was C282Y homozygous, 1 compound heterozygous for C282Y and H63D, 3 H63D heterozygous, and 7 had a normal HFE genotype. Serum ferritin and transferrin saturation were not related to hemoglobin, reticulocytes, and bilirubin concentration. At multivariate analysis serum ferritin was independently associated with age and gender, but not with splenectomy and HFE genotypes. The retrospective evaluation of the iron status profile of 10 patients (3 with abnormal and 7 with wild-type HFE genotype) with at least 10 years follow-up showed that overt iron accumulation requiring iron chelation had occurred only in the 3 patients (2 of whom were splenectomized) with the mutated HFE gene.
