Control mechanisms in platelet activation.

Those activation stages that are energy dependent rely upon a complex control mechanism involving the interaction of platelet thrombasthenin, free ionic calcium, cyclic AMP and the prostaglandins. This article describes the feedback mechanisms inherent in these controls, and the sources, activities and degradation of all control materials. Particular attention is paid to the prostaglandins and their activating enzymes, both within the dense tubular system of the platelet and the cytoplasm of endothelial cells. The implications of the prostaglandin pathway for both therapy and laboratory diagnosis are explored.

A review of platelet function and testing.

Platelet function testing is an area of recent growth and development in clinical laboratories. Additions to classical testing procedures include aggregation and retention tests and measurement of platelet release products. The necessity exists for understanding platelet structure and function. In this paper, the various platelet structures are demonstrated, their composition is described, and their individual contribution to platelet activation discussed. Platelet activation is described in four phases: adhesion, reversible contraction, aggregation, and release. Each is described in relationship to the others and laboratory test procedures marking each are described. The question of the relationship of measurement of platelet release markers to clinical condition is reviewed.

In vitro activity of cefamandole, cefoxitin, cefuroxime, and carbenicillin, alone and in combination with aminoglycosides against Serratia marcescens.

Synergistic antibiotic studies were undertaken to compare the effectiveness of two new beta-lactamase resistant cephalosporins, cefamandole, and carbenicillin, with four aminoglycosides against clinical strains of Serratia marcescens. The strains demonstrated various combinations of resistance and/or susceptibility to the antibiotics tested. Tobramycin was the most effective aminoglycoside when used in combination with beta-lactam antibiotics. Carbenicillin and cefamandole demonstrated similar activity with aminoglycosides in synergy experiments. Tobramycin-carbenicillin was found to be the superior pairs as indicated by the total number of strains inhibited. This combination was the only one effective against certain high drug resistant strains and the strain resistant to all four aminoglycosides. Carbenicillin or cefamandole with tobramycin exhibited comparable activity against multiple drug resistant organisms. However, mutants significantly more resistant to cefamandole developed during susceptibility testing. The findings of this study have clinical relevance for treating infections by this formidable pathogen.

Development of hepatocellular carcinoma in a man with auto-immune chronic active hepatitis.

A 57 year old man with auto-immune chronic active hepatitis, regularly treated with immunosuppressive therapy, had hepatocellular carcinoma (HCC) 10 years after diagnosis of the hepatitis. Assays of the hepatitis C virus antibodies against capsid and non-structural proteins revealed seronegativity in serial serum samples of this patient stored in the previous 10 years during follow up. The seronegative hepatitis C antibodies excluded hepatitis C virus as the cause of the HCC. The occurrence of HCC in this case suggests the necessity of surveillance for early detection of liver cancer in patients with auto-immune chronic active hepatitis undergoing long-term immunosuppressive therapy.
