Two siblings with classical xanthinuria type 1: significance of allopurinol loading test.

Two brothers with classical xanthinuria who lacked xanthine dehydrogenase activity were encountered. Their hypouricemia was caused by underproduction of uric acid. In their duodenal mucosa, no xanthine dehydrogenase (oxidase) activity was detected. The patients had no symptoms except for duodenal ulcer in one case. The conversion of allopurinol to oxipurinol during an allopurinol loading test for determining the type of classical xanthinuria revealed that the patients had classical type 1 xanthinuria, because aldehyde oxidase activity was present. Furthermore, the allopurinol loading test was conducted to determine the optimal examination times and specimens required for this test.

Relationship of food intake and dietary patterns with blood pressure levels among middle-aged Japanese men.

To investigate the relationship of food intake habits and dietary patterns to blood pressure, a cross-sectional study was conducted for 473 middle-aged Japanese males. After adjustment for age, residence, occupation, body mass index and alcohol consumption, mean systolic and diastolic blood pressure (SBP and DBP) were inversely associated with each intake frequency of dairy products, coffee, fruits, egg, beef, pork and chicken. The adjusted mean SBP and DBP of the individuals with 'all (= three)' of the following three dietary habits; 'once and over/wk of dairy products', 'once and over/wk of fruits', and 'three times and over/wk of beef, pork or chicken', were 7.4 mmHg and 6.9 mmHg lower (p < 0.001, for each) than those of 'zero or one' group. According to the analyses on 3-days weighed food records of 157 volunteers, the adjusted mean daily intake of total protein, animal protein and potassium were markedly higher in the 'two' or the 'three' group than in the 'zero or one' group (p < 0.05, for each). These results suggest that habitual intake of dairy products, fruits, and meat or chicken may be associated with the reduction of blood pressure possibly through the intake of protein and potassium.

Relationship of occupation to blood pressure among middle-aged Japanese men--the significance of the differences in body mass index and alcohol consumption.

To clarify how and why blood pressure differs between occupations, the proportions of hypertensives, and the measures of blood pressure, body mass index (BMI) and alcohol consumption among the individuals not taking antihypertensive drugs were compared between the eight occupational categories using the data from a health check-up for 589 middle-aged Japanese males, mostly randomly selected from five areas in Japan. After adjusting for age, the relationships of occupation to the proportion of hypertensives and the mean systolic and diastolic blood pressure substantially differed among the five areas. However, after further adjustment for residence, these blood pressure levels (the proportion of hypertensives, and the mean systolic and diastolic blood pressure) were found to be higher for the "Personnel in transport and communications", the "Clerical personnel", the "Managerial and civil personnel" and the "Professional and technical personnel", whereas these values were consistently lowest in the "Service personnel". Age and residence-adjusted mean BMI was also higher for the four occupational categories with the increased blood pressure levels. According to a weighted multiple regression analysis across the eight occupations, the age and residence-adjusted mean BMI was a significant predictor of the age and residence-adjusted mean systolic and diastolic blood pressure (p = 0.068 and 0.018, respectively). These results suggest that the occupation-related changes in BMI may largely contribute to the occupation-related changes in blood pressure.

In vivo characteristics of injectable poly(DL-lactic acid) microspheres for long-acting drug delivery.

Poly(DL-lactic acid) (PLA) microspheres containing testosterone (T) were prepared by the solvent evaporation process to evaluate their physical properties such as size distribution, shape, drug content, in vivo controlled drug release, pharmacological influences on the prostate gland in castrated rats, and histopathological findings of tissues surrounding the implants. The in vivo release of T from PLA microspheres containing 30 mg of drug obtained with chloroform was continued over a 6-week period. This effect is attributed to high dispersibility of T in the device when obtained with chloroform. Both serum drug levels and prostate gland weight recovery suggested the effects of a long-acting drug delivery system. The histopathological findings showed that the devices used were completely degraded 10 weeks after injection.

Preparation and characterization of oil-in-water type poly (D,L-lactic acid) microspheres containing testosterone enanthate.

Poly (D,L-lactic acid) (PLA) microspheres containing testosterone enanthate (ET) were prepared by using an oil-in-water (O/W) emulsion technique. The size distribution of the microspheres obtained could be explained by a log-normal distribution, and as a result, it was found that ET fully incorporates into microspheres even when the drug is loaded at up to 50%. On the other hand, the dissolution behavior of ET from microspheres was strongly dependent on particle size, suggesting that dissolution of the drug from microspheres can be easily controlled by controlling the preparative conditions.

Oculomotor nerve invasion of acute myelogenous leukemia demonstrated by magnetic resonance imaging.

A 53-year-old male was diagnosed as having acute myelogenous leukemia (M2, FAB). He complained of double vision and right blepharoptosis after receiving remission induction chemotherapy. Magnetic resonance imaging (MRI) showed enlargement of the bilateral oculomotor nerves. Intrathecal injections of methotrexate and cytosine arabinoside were partially effective and repeated MRI showed shrinkage of the enlarged oculomotor nerves, after therapy. This case shows the importance of MRI in the early diagnosis of CNS leukemia.

A selective transthyretin-adsorption column for the treatment of patients with familial amyloid polyneuropathy.

A transthyretin (TTR)-adsorption column has been developed for the removal of variant TTR from the plasma of patients with familial amyloid polyneuropathy (FAP). The adsorbent is an ion-exchange resin made of porous beads of polyvinyl alcohol gel covalently bound with dimethylaminoethanol. This column was used for three patients with type I FAP. It reduced the concentrations of both normal and variant TTR in the plasma to about half of their pre-adsorption levels. Except for thyroxine, retinol-binding protein and IgM, other proteins in serum were not significantly decreased and there were no adverse effects in long term clinical usage of this TTR-adsorption column. In this trial, we did not obtain concrete evidence that TTR-adsorption therapy can stop or delay the progression of the disease in a FAP patient. However, if we are able to apply this technique more frequently and effectively, TTR-adsorption therapy using our column might be useful for the treatment of FAP patients.

[Importance of AUC of carboplatin in head and neck cancer].

Combination chemotherapy with Carboplatin (CBDCA) and 5-fluorouracil (5-FU) was given to 115 patients with head and neck cancer. The area under the blood concentration-time curve (AUC) of CBDCA was calculated by Calvert's formula for 111 of the 115 patients. A retrospective study was conducted to define the relationship between the clinical outcome of the combination chemotherapy and the AUC of CBDCA. The overall response rate was 58.4% in the 115 patients. The objective response rate for 103 patients with squamous cell carcinoma was 61.9% with eight (7.6%) with complete responses (CR) and 57 (54.3%) with partial responses. The mean AUC for the administration of 300 mg of CBDCA per m2 was 3.91 +/- 0.68 mg/ml.min in the patients with squamous cell carcinoma. Although, there was no statistically significant relationship between the AUC of CBDCA and therapeutic efficacy, all CRs were obtained at an AUC above 3. Thrombocytopenia was the major dose-limiting toxicity in this study. The magnitude of thrombocytopenia increased significantly with increase in AUC values. Severe thrombocytopenia did not seem to occur in the patients with an AUC under 4.5. From these results, the AUC was considered helpful for determining the CBDCA dose to prevent severe side effects. In respect of both therapeutic efficacy and toxicity, AUC values from 3 to 4.5 are recommended for neo-adjuvant chemotherapy with CBDCA and 5-FU. Prospective validation is necessary to settle the question of whether a higher AUC value within this range can improve the outcome.

[Obstructive sleep apnea in children with cerebral palsy].

To determine the etiology of obstructive sleep apnea (OSA) in children with cerebral palsy (CP), a survey using a questionnaire was conducted on parents of 233 CP children (1-5 years old, mean age: 2.7) and on those of 343 control children during a routine medical check-up at the age of three. The prevalence of habitual snoring and nasal obstruction was 63 and 20% in CP children, which were significantly higher than in control children. Sleep apnea episodes and stridor were noted in 19.7 and 15.4% of CP children. A screening sleep study was performed using Apnomonitor II and a pulseoximeter (Pulsox 5) in 48 CP children whose questionnaires revealed habitual snoring and sleep apnea, and it was found that 27 and 58% of the children had on apnea-hypopnea index (AHI) of greater than 5 and a low level of SaO2 (LSaO2) of less than 85%, respectively. When another 10 CP children visited our hospital for treatment of severe OSA, precise evaluations including pharyngeal pressure and fiberscopic examination during sleep as well as a sleep study using an inductive prezysmograph (Respigraph) were performed. Adenoid and/or tonsillar hypertrophy were noted in only 4 children, and the main cause of sleep apnea in the other 6 children was pharyngeal collapse at the lingual base. Their OSA was successfully treated by adenotonsillectomy in 2 children, adenotomy in 2 children, UPPP and lingualplasty in 1 child, tracheostomy in 2 children, and nasal CPAP in 1 child. Before treatment of OSA in CP children, precise evaluation is recommended in order to perform appropriate treatment.

[Speech audiometry using the American Word Lists for Japanese subjects with normal hearing].

The characteristics of listening to English words for Japanese people were studied in ten normal hearing subjects who had taken English classes to the level of college graduates and had opportunities to learn English continuously. Following pure tone audiometry, speech audiometry was performed using the Central Institute for the Deaf (CID) W-1 and W-22 word lists for English and the 67-S word lists for Japanese. The speech reception thresholds (SRTs) for the CID W-1 lists were significantly higher than average pure tone threshold (PTT), although the SRTs for the 67-S lists were equal to the average PTT. The difference in average SRT between the CID W-1 lists and the 67-S lists was about 15dB, which is statistically significant. The speech discrimination rate for the CID W-22 lists ranged from 78 to 100 percent with an average of 89.5 percent, while all subjects achieved the discrimination rate of 100 percent for the 67-S lists. Analysis with transient matrices of the perceived words demonstrated that the articulation rates were below 90 percent for the consonants /m/, /n/, /p/ and /delta/ . The observed variation in the speech discrimination score and the pattern of confusion among the subjects was assumed to be much more pronounced in noisy conditions.

UCH9, a new antitumor antibiotic produced by Streptomyces. II. Structure elucidation of UCH9 by mass and NMR spectroscopy.

The structure of UCH9, which is a novel antitumor agent, was determined by spectroscopic methods. UCH9 consists of an aglycone and five 2,6-dideoxy sugars (three D-olivoses, one 4-O-methyl-D-olivose and one D-oliose). Four of the five sugars are sequentially connected through a beta 1-->3 linkage (olivose-1-->3-4-O-methyl-olivose-1-->3-oliose-1-->3-+ ++olivose). On the basis of the results of spectroscopic analysis, it was found that UCH9 belongs to the aureolic acid family of antibiotics. The structure of UCH9 is unique in that mono- and tetrasaccharide moieties, and a long hydrophobic side chain (sec-butyl group) are attached to the aglycone, while di- and trisaccharide moieties and a methyl or a hydrogen are attached in the case of the known aureolic acid analogs. It is known that aureolic acid analogs from a dimer in the presence of Mg2+. NMR, FAB-MS and atomic absorption analysis revealed that UCH9 isolated from Streptomyces also forms a dimer, containing one equivalent molar Mg2+.

Identification of radicicol as an inhibitor of in vivo Ras/Raf interaction with the yeast two-hybrid screening system.

Activation of cytoplasmic serine/threonine kinase Raf-1, an important effector of Ras, requires direct binding to Ras. The yeast two-hybrid screening system used for identification of inhibitors of Ras/Raf-1 interaction showed radicicol to be an inhibitor. Radicicol has been shown to induce morphological reversion of transformed cells. Immunoprecipitation with an anti-Ras antibody revealed that the in vivo Ras/Raf-1 binding in v-Ha-ras-transformed cells was also blocked by low concentrations of radicicol (0.1 approximately 1 microg/ml), while degradation of Raf-1 was induced at concentrations higher than 2 microg/ml. However, in vitro binding of glutathion S-transferase-fused Ras to a maltose binding protein-fused RIP3 containing the Ras-binding domain (RBD) of Raf-1 was not inhibited by radicicol. Similar two-hybrid assays with several truncated forms of Raf-1 showed that both the conserved serine/threonine-rich domain (CR2) and the C-terminal protein kinase domain (CR3) were required for the full inhibition by radicicol. These results suggest that radicicol interacts directly or indirectly with the region except with RBD of Raf-1, thereby inhibiting a conformational change of Raf-1 prerequisite for binding to Ras.

[Relapsing pneumothorax secondary to thin-walled cavitary pulmonary metastasis from angiosarcoma of the scalp].

A 79-year-old woman was given a diagnosis of scalp angiosarcoma. Treatment with interleukin-2 and radiotherapy aclieved a complete response. However, a few months later, the patient presented with multiple thin-walled cavitary metastases in the right lung and pneumothorax. The pneumothorax was successfully treated but soon relapsed. The patient died of respiratory failure at home. Lung metastasis of malignant tumors should be considered one cause of relapsing pneumothorax.

Histochemical staining of chromium with 2-(8-quinolylazo)-4,5-di-p-tolylimidazole.

A new highly sensitive staining agent for chromium (Cr) has been introduced. This staining agent, 2-(8-quinolylazo)-4,5-di-p-tolylimidazole (QTI), was found to be more than ten times as sensitive for Cr than the conventional staining agent, Chromazurol S, and QTI also stained cadmium, copper, lead and zinc. Differentiation between the staining of Cr and other metals was achieved by immersing the tissue sections in dilute alkylamine solutions before they were stained with QTI. Thus, it was possible to selectively stain for Cr by blocking other metals.

[Anticancer susceptibility test method using general-purpose dissolved oxygen meter].

Drug susceptibility of cell can be rapidly measured by continuously monitoring its metabolic changes. We focused on respiration volume as a signal of metabolic change, and developed a new dissolved oxygen measuring system which can detect respiration volume of cells. The main feature of the system is the use of a new type bare oxygen electrode which can easily detect the changing rate of dissolved oxygen concentration. In this study, single type electrode was used to evaluate this rapid anticancer drug susceptibility test. The result obtained was almost equivalent to that with MTT method, which is a conventional method for susceptibility test of HL-60 to various kinds of anticancer drugs. We have also developed multi-type electrode plate with oxygen electrodes embedded in the bottom of 96-well plate, with which clinical evaluation of this method can be easily made.
