Bioequivalence study of two long-acting oxytetracycline formulations in sheep.

Two commercially available long-acting oxytetracycline hydrochloride formulations (Primamycin LA (Pfizer) and Terralent 20% LA (I.E. Ulagay)) were administered by the intramuscular route to 20 clinically healthy sheep at a dose of 20 mg/kg. The study was performed in a two-period crossover design. Plasma samples were analysed by high-pressure liquid chromatography. The mean maximum concentrations (C (max)) was 8.00 +/- 2.05 microg/ml and 8.61 +/- 1.42 micro/ml, respectively. The mean area under the concentration time curve (AUC) values were 154.95 +/- 50.37(microg h)/ml and 161.70 +/- 47.02(microg h)/ml, respectively. The 90%confidence intervals for the ratio of C (max) and AUC values for the test and reference product are with in the interval 70-143% for C (max) and interval 80--125% for AUC proposed by EMEA. It was concluded that Primamycin LA and Terralent 20% LA formulations are bioequivalent in their rate and extent of drug absorbtion.

Hydatid cysts with unusual localizations: diagnostic and treatment dilemmas for surgeons.

Although the liver and lung are by far the most common localizations for the larval cysts of Echinococcus granulosus in humans, the cysts may develop in other sites and there cause signs and symptoms that may be easily confused with those of other illnesses. In a retrospective study in Turkey, six male and 11 female cases of cystic echinococcosis, each with at least one cyst in an unusual site, were investigated. The patients, who had a mean age of 41.6 years, had cysts in the pancreas, intra-abdominal cavity, kidney, spleen, ovary, breast, mediastinum, chest wall, muscle and/or subcutaneous tissue. In terms of Gharbi's classification, 15 (75%) of the 20 cysts in these patients were type I and five (25%) were type II. Fourteen of the cases each had single cysts, two had multiple cysts, and one had an unknown number of cysts. All but one of the cases (who had a pancreatic cyst) were treated by total cystectomy. In areas where cystic echinococcosis is endemic, any patient presenting with a cystic mass, in any tissue or organ, should be considered a potential case of the disease and be carefully investigated by radio-imaging and/or ultrasonography.

An anatomical study of the origins of the medial circumflex femoral artery in the Turkish population.

The medial circumflex femoral artery (MCFA) usually branches from the deep femoral artery (DFA). It may also branch from the femoral artery (FA). In this study 100 inguinal regions of 50 cadavers were investigated. In 79 extremities (79%) MCFA branched from DFA, while in 15 (15%) it branched from FA. In four extremities (4%) MCFA was found to be double; in each case one of MCFAs branched from FA and the other from DFA. In one of these four cases the lateral circumflex femoral artery (LCFA) was also double. In one case we found a common trunk of DFA and MCFA and in another case a common trunk of MCFA, DFA and LCFA. Clinicians must be familiar with the variations of this clinically important artery to improve their success in the diagnosis and treatment of pathologies in the region.

Anti-androgen induced inhibition of testicular descent is associated with a decrease in sympathetic tonus.

BACKGROUND/AIMS: Responses of cremaster muscles and sacs from boys with undescended testis suggested less exposure against sympathetic, but more exposure against parasympathetic tonuses. Since the sympathetic tonus is androgen dependent, it has been suggested that androgens control the descent by influencing the sympathetic tonus. Therefore an experimental study was planned to evaluate the contractile responses of cremaster muscles according to locations of associated testes in rats subjected to intrauterine steroidal or non-steroidal anti-androgen exposure. METHODS: Time-mated pregnancies were started in 18 rats. They were divided into three groups and each group was given physiologic saline, flutamide or cyproterone acetate (Androcur Depot). Flutamide was administered from day 15 to day 19, cyproterone acetate on day 15 of intra-uterine life. At twelve weeks of age the localization of testes was evaluated, cremaster muscles were removed, and contractile properties were studied. Twitch and tetanic contractions were recorded isometrically at 37 degrees C. Effects of verapamil and isoprenaline were investigated. Results were compared by ANOVA and p values less than 0.05 were considered significant. RESULTS: Both testes of all available male offspring in the saline (n = 22) and cyproterone acetate-treated (n = 19) groups were in the scrotum. Sixty percent of males in the flutamide-treated group (n = 20) had undescended testes. Cremaster muscles of rats exposed to flutamide had a lower sensitivity to voltage sensitive Ca+2 channel blockade by verapamil (3 x 10(-4) mol/L, p < 0.05) and displayed greater contractile response to isoprenaline (10(-5) mol/L, p < 0.05). Alterations in contractile properties of the muscles did not differ according to localization of testes in rats subjected to flutamide exposure. CONCLUSIONS: CM in rats subjected to non-steroidal anti-androgen exposure revealed alterations that indicate a decrease in sympathetic tonus. Since non-steroidal anti-androgen also inhibits the descent, the present study provides experimental support for the involvement of sympathetic tonus in the androgenic control of testicular descent.

Primary hydatid disease of the chest wall: a case report.

Primary hydatid disease of the chest wall is extremely rare. We report a case of a primary chest wall hydatid cyst in a 72-year old woman who had not been operated on before for hydatid disease. Chest wall hydatid disease should be considered in the differential diagnosis of chest wall mass.

In vitro bacterial adherence to teicoplanin and calcium sulfate-soaked bone cement.

The aim of this study was to assess in vitro the improvement in release kinetics for teicoplanin and the inhibition of bacterial adhesion on calcium sulfate-soaked PMMA discs. Calcium sulfate has been used in vivo and shown to be biocompatible, and prevention of bacterial adhesion may be expected with calcium sulfate-soaked polymethylmethacrylate (PMMA). Discs were made by adding teicoplanin and calcium sulfate in powder form to PMMA powder. The antibiotic concentration eluted from PMMA discs was assayed by agar diffusion assay. Nonadherent bacteria were removed by washing and adherent bacteria were detached by sonication. The suspension including nonadherent bacteria was seeded on sheep blood agar plate and incubated for 24 h at 37 degrees C for the growth of microorganisms. The teicoplanin released from discs containing calcium sulfate was higher than that released from discs which had not been soaked with calcium sulfate. The count of bacteria adhering to the calcium sulfate-soaked discs was lower than that from the discs without calcium sulfate. In conclusion, the addition of calcium sulfate to teicoplanin-loaded PMMA bone cement may provide local antibiotic concentrations higher than MIC values due to increased antibiotic release. Furthermore, calcium sulfate was found to be effective in reducing bacterial adherence to treated discs.

The role of human herpes virus 6 and 7 in the pathogenesis of pityriasis rosea.

Pityriasis rosea (PR) is an acute exanthematous inflammatory skin disease in which many epidemiological studies suggest a viral infection in its pathogenesis. Several viruses such as picornaviruses, parvoviruses and herpes viruses were accused as causative agents in PR. Human herpes virus-6 (HHV-6) and HHV-7 are newly discovered members of herpesviridae family. Several diseases including roseola infantum and PR have been linked with HHV-6 and HHV-7 in dermatology. We aimed to investigate the role of HHV-6 and HHV-7 in the pathogenesis of PR. Blood samples from 35 patients with acute PR and 30 healthy volunteers were examined to obtain titres for HHV-6 and HHV-7 antibodies. Anti-HHV-6 and -HHV-7 immunoglobulin (Ig) G antibodies were detected by IFA. The high titres of anti-HHV-6 IgG were determined in four patients and anti-HHV-7 IgG in only two of four patients. In our study, we haven't found any significant relationship between HHV-6, HHV-7 and PR. Our results were supported with the data of several previous studies.

Crusted scabies in a healthy infant.

Crusted scabies is generally seen in mentally retarded and immunosuppressed individuals. We report the case of a patient with crusted scabies with neither systemic disease nor immunosuppression; all the lesions were cured with applications of 5% permethrin lotion.

The role of oxidants and antioxidants in generalized vitiligo at tissue level.

BACKGROUND: The role of oxidative stress has not been fully understood in the aetiopathogenesis of vitiligo in different studies. AIM: We aimed to investigate the role of the oxidative stress in the pathogenesis of vitiligo. METHODS: In this study, we examined levels of superoxide dismutase, glutathione peroxidase, malondialdehyde and nitric oxide in tissue of 25 patients with generalized vitiligo and 25 healthy controls. RESULTS: Our results revealed that levels of superoxide dismutase, glutathione peroxidase and malondialdehyde in tissue were significantly increased in patients with generalized vitiligo (P < 0.05). However, there was no statistically significantly difference between two groups at tissue level of nitric oxide (P > 0.05). CONCLUSION: Our results demonstrate the presence of an imbalance in the oxidant-antioxidant system in vitiligo at tissue level and provide further support for a free radical-mediated damage as an initial pathogenic event in melanocyte degeneration in vitiligo.

A case of condyloma acuminatum treated successfully with low-dose isotretinoin and interferon.

The treatment of condyloma acuminatum may be problematic in clinical practice. We present a 27-year-old man with condyloma acuminatum who was treated successfully with a combination therapy of isotretinoin and interferon-alfa-2a. In the previous reports of combination therapy, isotretinoin has been given in a dosage of 1 mg/kg/day. However, we achieved the same success at a dosage of 0.5 mg/kg/day of isotretinoin.

99mTc-MDP bone SPECT in evaluation of the knee in asymptomatic soccer players.

OBJECTIVE: To evaluate stress fractures in leg (particularly around the knee, tibia, and femur) and knee pathology in active asymptomatic (no symptoms in the preceding month) soccer players. METHOD: The study included 42 asymptomatic soccer players (21 women, 21 men; age range 19-31 years). Players from seven teams in the major female professional and amateur male soccer leagues were examined by technetium-99m-methylene diphosphonate ((99m)Tc-MDP) bone scintigraphy during the soccer season. Four hours after intravenous injection of 20 mCi (99m)Tc-MDP, standard imaging included anterior planar spot images of the legs, lateral images of the knee, and single photon emission computed tomography (SPECT). RESULTS: Although the players were asymptomatic, increased tracer uptake, indicating stress fracture, was found in 28 (66%). Most of the stress fractures were in the tibia (62%) and femur (5%). In the 42 subjects (84 legs), 35 sites (42%) showed rupture of the posterior horn of the lateral meniscus and bone bruising of the tibial plateau, 16 sites (19%) showed rupture of the anterior horn of the medial meniscus, 11 sites (13%) showed bone bruising of the lateral femoral condyle, eight sites (10%) showed bone bruising of the medial femoral condyle, and there was avulsion injury to the infrapatellar tendon insertion in the anterior tibia in 34 sites (40%). There were 11 anterior cruciate ligament injuries. CONCLUSION: Bone SPECT is very accurate, easy to perform, cost effective, may give valuable information before magnetic resonance imaging studies in the detection of meniscal tears, and may be used successfully when magnetic resonance imaging is unavailable.

Inflammatory pseudo-tumour as an unusual cause of colonic obstruction: a case report.

Inflammatory pseudo-tumour (IPT), also known as inflammatory myofibroblastic tumour or plasma cell granuloma, is a benign inflammatory solid tumour that may mimic malignancy. The etiology is obscure. Lung, spleen and mesentery are common sites of involvement. Colonic occurence is rare. Herein we report a case of IPT arising from the hepatic flexura with regard to its radiological, operative and histologic findings, that lead to colonic obstruction.

A rare accessory coracobrachialis muscle: a review of the literature.

During previous dissections to investigate the incidence of the third head of the biceps brachii in neonate cadavers an accessory coracobrachialis muscle was unilaterally found in a neonate male cadaver. This muscle originated from the coracoid process and the capsule of the shoulder joint. It was inserted into the antebrachial fascia and the medial epicondyle of the humerus. The coracobrachialis muscle is used as a transposition flap in deformities of infraclavicular and axillary areas and in postmastectomy reconstruction. It is also a guide to the axillary artery during surgery and anesthesia. This additional muscle may cause musculocutaneous or high median nerve paralysis. We were unable to find any similar case in the published literature. The anatomic variations of the coracobrachialis muscle may cause confusion during surgery or evaluation of CT and MRI scans.

Feed deprivation of channel catfish, Ictalurus punctatus (Rafinesque), influences organosomatic indices, chemical composition and susceptibility to Flavobacterium columnare.

Withholding feed has been suggested as a strategy to manage infectious disease of channel catfish, Ictalurus punctatus (Rafinesque). In a previous study, we demonstrated that deprivation of feed for as little as 7 days reduced innate resistance of catfish to Flavobacterium columnare. This study was conducted to evaluate the effect of feeding regimens [no feeding (NF), fed once every other day to satiation (FEOD) and fed once daily to satiation (FD)] on organosomatic indices, physiological changes and susceptibility of channel catfish to F. columnare. Fish that were not fed for 2 and 4 weeks had a significant increase (P < 0.05) in gutted weight:-wet weight ratio and decrease in other organosomatic indices [gut index (GI), mesenteric fat index (MFI) and hepatosomatic index (HSI)]. Haematology was not effected by feeding regimen except at week 4, when a significantly higher haemoglobin level was observed in the NF fish. Serum protein did not differ at week 2, but the level at week 4 of the NF fish (35.91 mg mL(-1)) was significantly lower than that of the FD fish (41.77 mg mL(-1)). Significantly lower (P < 0.05) blood glucose (39.5 and 40.3 mg dL(-1)) and liver glycogen (1.7 and 1.8 mg g(-1)) were seen in the NF fish at weeks 2 and 4, respectively, as compared with blood glucose and liver glycogen levels of FD fish (67.5 and 92.8 mg dL(-1) and 46.5 and 52.6 mg g(-1) at weeks 2 and 4, respectively) and FEOD (82.8 and 85.5 mg dL(-1) and 45.1 and 51.4 mg g(-1) at weeks 2 and 4, respectively). Mortality in the NF fish caused by F. columnare (78%) was significantly higher (P < 0.05) than mortality in the FD and FEOD treatments (0.0 and 1.7%, respectively). Blood glucose and liver glycogen showed the same trend of low values for NF fish following challenge (week 6). Blood glucose, liver glycogen, GI and HSI are sensitive indicators for channel catfish deprived of feed (NF) for 4 weeks. Blood glucose and liver glycogen levels around 40 mg dL(-1) and 2 mg g(-1), respectively, are indicative of starvation in juvenile channel catfish. Moreover, NF fish were susceptible to F. columnare infection. Thus, it is suggested that in the absence of natural food, juvenile channel catfish should be fed at least once every other day to apparent satiation to maintain normal physiological function and improve resistance to F. columnare.

The role of oxidants and antioxidants in psoriasis.

BACKGROUND: Psoriasis vulgaris is a chronic inflammatory skin disease characterized by well-demarcated erythema and scaly plaques. The pathogenesis of psoriasis still remains unclear. An increased reactive oxygen species (ROS) and insufficient antioxidant activity have been determined in psoriatic lesions. AIM OF THE STUDY: To evaluate and compare superoxide dismutase (SOD) and glutathione peroxidase (GP) activity in erythrocytes, catalase (CAT) activityand malondialdehyde (MDA) levels in serum of subjects with psoriasis and controls as well as MDA levels in skin biopsies from both groups. STUDY POPULATION: Twenty-two psoriatic patients (12 women and ten men) and 22 (12 women and ten men) healthy controls were involved in this study. FINDINGS: Statistically significant decreased levels of erythrocyte SOD and GP activities were noted in psoriatic subjects. Furthermore, a statistically significant increased serum CAT activity was found in the psoriasis group. No statistically significant difference was found in the serum MDA levels in the two groups, however, statistically significant increased tissue levels of MDA were noted in the psoriasis group. CONCLUSIONS: Our results support the hypothesis of an imbalance in the oxidant-antioxidant system in psoriasis.

Is the combination of calcipotriol and PUVA effective in vitiligo?

OBJECTIVE: The objective was to compare the effectiveness of psoralen plus ultraviolet A (PUVA) and the combination of PUVA and topical calcipotriol in the treatment of vitiligo. BACKGROUND: There are several reports on the response rate of patients with vitiligo treated with the combination of PUVA and calcipotriol or calcipotriol alone. SUBJECTS AND METHODS: Twenty-two patients with generalized vitiligo were taken into the study. PUVA treatment was applied on a twice-weekly schedule. Calcipotriol cream was applied to one of the two symmetrical lesions of each patient twice daily. RESULTS: Our results showed that the addition of topical calcipotriol to PUVA treatment did not lead to a significant increase in response rate of patients with vitiligo compared with PUVA treatment alone.
