[A case of dural arteriovenous malformation in the anterior fossa].

UNLABELLED: Dural arteriovenous malformations (AVMs) are not uncommon. But most of dural AVMs are located in the cavernous portion or the transverse-sigmoid portion. Dural AVMs in the anterior fossa and very rare. We experienced a case of dural AVM in the anterior fossa and review 17 cases of dural AVM in the anterior fossa in the literature. CASE: a 62-year-old male was admitted to our center because of unconsciousness and vomiting on Sept. 28; 1985. For nineteen months he had been on anticoagulant therapy after aorto-coronary bypass. Neurological examination on admission revealed somnolence and right-hemiparesis. CT scan revealed intracerebral hematoma in the left frontal lobe with ventricular perforation. Enhanced CT scan revealed patchy enhancement in the tip of the left frontal lobe. Selective angiographies were performed. Dural AVM, fed by both anterior ethmoidal arteries and drained by the cortical veins into the superior sagittal sinus and left superficial middle cerebral vein, was found. Selective external carotid angiography failed to visualize any AVM. MRI revealed spotty low signal intensity in the frontal tip by inversion recovery and spin echo. After admission his consciousness and right-hemiparesis improved completely. Left frontal craniotomy was done. On opening the dura, aneurysmal dilatation was found in the left frontal lobe, arising from the inner surface of the dura at the antero-medial corner of the left anterior fossa. From this aneurysmal dilatation a few veins entered the superior sagittal sinus. Red veins including aneurysmal dilation was removed from its origin together with the intracerebral hematoma. [The prevention of CDDP-induced emesis with a combination regimen including metoclopramide, dexamethasone, droperidol and diphenhydramine].

The dose limiting factors of cisplatinum are nephrotoxicity and emesis. Nephrotoxicity has been reduced by hydration but nausea and vomiting caused by cisplatinum have led to refusal of potentially curative therapy by a number of patients. The prevention of nausea and vomiting by a combination of antiemetic drugs administered to ovarian patients receiving chemotherapy inducing (cisplatinum 50mg/m2, adriamycin 300 mg/m2, cyclophosphamide 300 mg/m2 and 5FU 350 mg/m2) was studied. the combination antiemetic drugs were metoclopramide (1mg/kg), dexamethasone (10mg/m2), droperidol (1mg/m2) and diphenhydramine (20mg/body). These drugs without diphenhydramine were administered intravenously 30 minutes before and 2.5 hours, 5 hours and 7.5 hours after chemotherapy. Diphenhydramine was administered intramuscularly 30 minutes before and 5 hours after chemotherapy. No vomiting was noted in 82.6% (19/23) of cases, and no patient vomited more than four times. This combination regimen provided very good protection against cisplatinum induced emesis.

[A study of gastric cancer invasion of the duodenum].

Gastric cancers that have invaded in the duodenum were clinicopathologically studied in 83 patients in an attempt to clarify surgical therapeutic problems. The classification and results are itemized below. 1) The extensive forms of these cancers were found to include the types of continual superficial extension (12 patients), expansion (11), deep-layer (39), whole-layer (9), non-continual extensive intralymphatic duct type (8) and specific type (4). 2) More patients manifesting a deep-layer or intralymphatic duct cancers were Borr. 3 and 4 types with a longer distance of infiltration. All of the 14 patients with an aw (+) showed these patterns of extension. 3) The n3 (+) was more common with Nos. 12, 13 and 14 lymph nodes, and similar results were seen by RI lymphography. Thus, special care may have to be exercised in treating the duodenal stump in the gastric cancers that have invaded the pyloric ring. Pancreato-duodenectomy may be required for the removal of lymph nodes in patients with gastric cancers that show a severe infiltration into the duodenum.

Partial purification and characterization of feline gamma-like interferon.

Feline interferon (IFN) was produced from spleen cells stimulated by Staphylococcus enterotoxin A (SEA). The IFN was purified by a three-step procedure using controlled pore glass adsorption chromatography, concanavalin A (ConA)-agarose column chromatography and gel filtration. The final product of these procedures which had activity of an IFN appeared as a single peak of activity with molecular weight of approximately 50,000. It was sensitive to acid and heat, suggesting that the isolated material was a gamma IFN. The total recovery of feline gamma IFN was 8.2%. Specific activity was 2.95 X 10(4) unit/micrograms protein and was concentrated 2.8 X 10(4) times. This preparation of purified feline gamma IFN was destroyed completely by 0.1% sodium dodecyl sulfate within 20 min. As an inducer of feline gamma IFN, SEA appeared to produce a more uniform IFN product than did ConA. Further, the presence of 10% ethylene glycol in the sample applied to ConA-agarose column as well as its absence in the elution buffer was effective in reducing contaminating acid- and heat-resistant IFNs in the preparation.

[Biochemical activity in peritoneal effusion of ovarian malignancy].

This study was provided biochemical findings on ascitic fluid following the treatment of patients with ovarian tumors. The results were as follows: LDH and LDH isozyme. The ascitic fluid from patients with germ cell tumors showed an LDH1 value of 38.8% with an H/M ratio of 2.82, which were significantly higher than those found in the benign tumor group (LDH1, 15.2%: H/M ratio, 1.05). In cases of adenocarcinomas, the LDH5 value was 27.2%, compared to 15.7% in cases of benign tumors. Of 28 patients with adenocarcinoma, 15 patients (53.6%) were positive for this elevation. The decline in total LDH activity and elevation of H/M ratio were observed as a general tendency in patients satisfactorily responding to treatment. ALP and ALP isozyme. The malignant tumor group displayed higher values for both total ALP activity and HSAP in ascitic fluid, as compared with the benign tumor group. In the malignant tumor group, alpha 2 and alpha 2-beta regions were prominent in the isozyme pattern and the isozymes of the same regions remained even after inactivation by heat. The total activity decreased significantly in the responders to treatment. TP, alpha 1-AT, alpha 1-AG, alpha 2-M and IgG. The ascitic fluid levels of alpha 1-AT, alpha 2-M and IgG did not significantly differ among the group of patients with germ cell tumors, the group of patients with ovarian gonadal stroma and the group of patients with benign tumors. In the adenocarcinoma group the alpha 1-AT and alpha 2-M levels were higher than those in the benign tumor group. The ascitic fluid alpha 1-AG level and alpha 1-AG/TP ratio were elevated in tumors of germ cell origin and in adenocarcinomas; they tended to correlate with the degree of malignancy of the tumor. The results of the present study suggest that simultaneous measurement of the ascitic fluid levels of LDH isozymes, ALP isozymes, alpha 1-AG, alpha 1-AT, alpha 2-M and IgG may play a potential adjunctive role in providing information for diagnostic differentiation between benign and malignancy, estimation of the histologic type and extent of neoplastic growth, evaluation of the therapeutic response and prediction of the clinical progress.

Circulating atrial natriuretic polypeptide during exercise in patients with essential hypertension.

We studied the relationship between haemodynamic measurements and plasma atrial natriuretic peptide (ANP) levels during exercise in patients with essential hypertension. The exercise protocol consisted of three fixed work loads (25, 50 and 75 W) using a supine bicycle ergometer. Plasma ANP levels and haemodynamic indices were measured before and during each stage of exercise. Plasma ANP levels and pulmonary artery wedge pressure (PAWP) significantly increased at the maximum work load. Significant correlations of PAWP and mean arterial pressure (MAP) to plasma ANP levels were observed at four points obtained before and during each stage of exercise (PAWP, r = 0.83, P less than 0.001; MAP, r = 0.48, P less than 0.01). Furthermore, a significant inverse correlation between shortening fraction (SF), estimated by M-mode echocardiography, and plasma ANP levels at 75 W work load was also observed (r = -0.66, P less than 0.01). These results suggest that the increase of circulating ANP levels during exercise in essential hypertensive subjects may be associated with elevated left atrial pressure and arterial pressure, or left ventricular systolic dysfunction.

[Left ventricular wall motion abnormalities evaluated by factor analysis as compared with Fourier analysis].

Factor analysis was applied to multigated cardiac pool scintigraphy to evaluate its ability to detect left ventricular wall motion abnormalities in 35 patients with old myocardial infarction (MI), and in 12 control cases with normal left ventriculography. All cases were also evaluated by conventional Fourier analysis. In most cases with normal left ventriculography, the ventricular and atrial factors were extracted by factor analysis. In cases with MI, the third factor was obtained in the left ventricle corresponding to wall motion abnormality. Each case was scored according to the coincidence of findings of ventriculography and those of factor analysis or Fourier analysis. Scores were recorded for three items; the existence, location, and degree of asynergy. In cases of MI, the detection rate of asynergy was 94% by factor analysis, 83% by Fourier analysis, and the agreement in respect to location was 71% and 66%, respectively. Factor analysis had higher scores than Fourier analysis, but this was not significant. The interobserver error of factor analysis was less than that of Fourier analysis. Factor analysis can display locations and dynamic motion curves of asynergy, and it is regarded as a useful method for detecting and evaluating left ventricular wall motion abnormalities.
