The quantitative determination of saturated and unsaturated fatty acids (ranging from acetic acid to lignoceric acid) in biological samples is presented. The secondary amine group of 5-(dimethylamino)-1-naphthalenesulponyl-semipiperazide (dansyl-semipiperazide) reacts with the carboxyl group of the fatty acids to form an amide linkage in order to obtain fluorescent derivatives of the acids. The fluorescent derivatives are analysed by high-performance liquid chromatography (HPLC) using an internal standard.
From November 1981 to the end of August 1984, 456 patients with breast lesions underwent aspiration biopsy cytology (A.B.C.). This study includes 109 for whom the diagnosis was histologically confirmed at surgical biopsy. Seventy-five lesions were histologically proven to be malignant and 34 were benign. The accuracy of diagnoses with A.B.C. was; true positive 86.7 per cent (65/75) of the time, true negative 82.6 per cent (28/34) of the time, false negative 5.3 per cent (4/75) of the time and false positive 5.7 per cent (2/34) of the time. Unsatisfactory or inadequate aspirated tissue made A.B.C. diagnosis difficult in 5.5 per cent (6/109) of the cases. Three out of 7 with malignant tumors, who were wrongly diagnosed as benign by A.B.C., had tumors with a diameter of 1.0 cm or less. Two benign cases which were falsely diagnosed as malignant also had small tumors about 1.0 cm in diameter. Although A.B.C. is more reliable than other conventional supplementary diagnostic techniques (mammography, ultrasonography, etc.), it is important to carefully follow clinically questionable cases which appear negative, using A.B.C..

Microscopic cancer is extremely rare even among "occult breast cancers". We treated a 70 year old woman with complaints of serosanguineous nipple discharge which yielded cytologically Class V cancer cells. Microdochectomy was performed and intraductal carcinoma was verified histologically in serially cut sections. Following a simple mastectomy, she has been well with no evidence of recurrence or metastasis, at this time.
Surgical specimens from 60 Japanese women with breast cancer were studied histochemically to detect estrogen receptors (ER). Forty-six were analyzed in a double blind study designed to compare the histochemical method using fluorescent estradiol conjugate for detection of ER (FITC method) with findings in case of the currently employed dextran coated charcoal (DCC method) techniques. The ER test was positive in 39 (65 percent) of the 60 cases studied by FITC method. Of the 46 cases analysed in a double blind study, 41 (89.8 percent) showed the same results. Of the 5 with inconsistent test results, 2 showed positive DCC and negative FITC, and 3, negative DCC and positive FITC. According to the histologic type, the FITC method demonstrated a low positive ER response rate in scirrhous cases (57 per cent), whereas the positive rate was 67 percent among papillotubular-type cases and 65 percent among medullary tubular-type cases. In relation to menstrual status, the positive rates were 68 percent and 64 percent among premenopausal and postmenopausal women, respectively. However, in relation to tumor size, no difference was noted in the positive response rate by either method. Therefore, both the DCC and FITC methods are clinically useful to determine the indications for hormone therapy.
L-1- Methylheptyl -gamma- bromoacetoacetate was found to be a competitive inhibitor of the acetylcholinesterases (electric eel, Ki = 17.2 microM; rat brain, Ki = 32.6 microM) and of butyrylcholinesterase (horse serum, Ki = 1.2 microM). The L-isomer was a more effective inhibitor than the D-isomer. The bromine atom at the gamma-position of the acidic moiety, the specific length of the carbon chain constituting the secondary alcohol moiety, and the presence of the ketone radical at the acidic moiety of the ester were necessary for the anticholinesterase action. 1- Methylheptyl -gamma- bromoacetoacetate formed a complex with acetylcholinesterase or butyrylcholinesterase without hydrolysis of its own molecule.
An impedance audiometric analysis of the stapedial reflex was carried out in 17 patients with motor neuron disease (MND), 11 with myasthenia gravis (MG), and 3 myotonic dystrophy (MD). The features examined were the threshold and latency parameters and the response pattern of the reflex. All groups had mean reflex thresholds within normal limits (80-90 dB hearing level). The MND group showed a lengthened mean reflex latency (L1). The MG group showed an unstable response pattern to 10-second tone stimuli and a lengthened L1. Half the group demonstrated abnormal decay of the reflex. The MD group showed a reduction in the growth of averaged reflex in response to stimuli with short interstimulus intervals (2.6 s) and the L1 and C50 were lengthened.
Delayed endolymphatic hydrops, an entity which can be differentiated from Meniere's disease, was first reported simultaneously by Nadol et al. (1975) and Wolfson & Leiberman (1975) and specified by Schuknecht (1978). They diagnosed it from the anamnesis of patients and confirmed the hydropic condition of the labyrinth solely by evidence obtained from surgical procedures such as labyrinthectomy. The aim of the present study was to emphasize the usefulness of the furosemide test for distinguishing this entity before surgery by detecting the hydropic condition in the labyrinth. In these 3 years, we encountered 11 cases of endolymphatic hydrops, 9 ipsilateral and 2 contralateral, in our clinic (Univ. of Tokyo Hospital). Among the ipsilateral cases, the estimated genesis of pre-existing hearing loss was most frequent in mumps (44%). The result of the furosemide test in this group was 89% positive. On the basis of this result, we have been able to choose the method of treatment, i.e. 4 cases treated by epidural shunt operation on the endolymphatic sac, 2 by labyrinthectomy, one by cochleosacculotomy and 2 by medication with steroids and diuretics. In the 2 contralateral cases, one was treated by epidural shunt and the other by medication. The authors are therefore able to confirm the usefulness of the furosemide test for detecting delayed endolymphatic hydrops.

During endolymphatic sac surgery on patients with Meniere's disease, very small amounts (1 microliter) of the intra-sac endolymph were obtained with micro-glass pipets. An immunological assay of microanalysis of the protein by the single radial immunodiffusion (SRID) method was applied to 2 patients, one with bilateral involvement and the other unilateral. Among the protein fractions Alb, IgG, IgM, IgA, C3, C4, transferrin and alpha 2M, the concentrations of IgG were relatively higher, especially in the Meniere patient in the active stage. From 2 other patients and a control deaf patient with temporal bone fracture, the same amounts of endolymph were obtained, accompanied by a tiny specimen of the outer wall of the sac. When a radio-immunological procedure was applied to the endolymph and the homogenized sac, the specific concentration of IgG was higher in the Meniere patients than in the control.
