[Apoptosis-inducing and -preventing signal transduction pathways in cultured cerebellar granule neurons].

1. Cultured cerebellar granule neurons maintained in medium containing 26 mM potassium (high K+ or HK+) undergo cell death when switched to medium with 5 mM potassium (low K+ or LK+). This low K(+)-induced cell death has typical features of apoptosis. The intracellular signaling pathway of low K(+)-induced apoptosis has been investigated. 2. Cerebellar granule neurons become committed to undergo apoptosis between 2 and 5 h after K+ deprivation, judging from the inability of high K+ to rescue them after this time. Although the levels of most mRNAs decrease markedly concomitant with commitment, expression of c-jun mRNA increases 2-3 h after K+ deprivation. Among the family of caspases, a caspase-3-like protease is activated within 4 h of lowering the K+ concentration. A caspase-1-like protease is also activated within 2 h of K+ deprivation. 3. Inhibition of phosphatidylinositol 3-kinase (PI3-K) activity by LY294002 or wortmannin also induces apoptosis in cerebellar granule neurons. The intracellular signaling pathway of LY294002-induced apoptosis has been investigated. The activity of c-Jun N-terminal kinase (JNK) increases 8 h after addition of LY294002 to high K+ medium or low K+ medium containing BDNF. Expression of c-Jun protein also increases almost simultaneously. 4. The low K(+)-induced apoptosis of cerebellar granule neurons is prevented by high K+ (membrane depolarization by high K+), BDNF, IGF-1, bFGF or cAMP. The intracellular signaling pathways by which these agents prevent low K(+)-induced apoptosis have been investigated. Agents other than cAMP prevent apoptosis through PI3-K and a Ser/Thr kinase, Akt/PKB. The survival-promoting effect of cAMP does not depend on the PI3-K-Akt pathway.

[Cytokine in cancer chemotherapy--clinical trials of IL-3, IL-11 and thrombopoietin against thrombocytopenia].

Clinical trials of IL-3, IL-11 and thrombopoietin (TPO) against chemotherapy-induced thrombocytopenia were reviewed. Many clinical trials of IL-3 were conducted, which increased the platelet nadir counts and improved the recovery from thrombocytopenia at a dose of 5 micrograms/kg/day for 7 to 14 days. Major side effects, fever and headache, were tolerable. The combination with G-CSF was also examined. IL-11 improved the platelet nadir counts and the recovery from thrombocytopenia at a dose of 50 micrograms/kg/day for 14 days. Adverse events of IL-11 were general fatigue and edema, which were mild. Phase I/II studies of TPO were conducted, which showed a marked increase of platelet counts and improvement in recovery from thrombocytopenia. A few patients experienced mild thrombo-embolism. Improved QOL and survival benefits in the evaluation of these cytokines must be demonstrated.

[Change in intraocular pressure after intraocular lens implant during a follow-up period of over 10 years].

Changes in intraocular pressure were studied in 141 eyes following intraocular lens implant. The average observational time was twelve years. We tried to fix both loops of the implant in the ciliary sulcus. The intraocular pressure increased over the ten year interval at 0.19 mmHg per year. Twenty-seven of the 141 eyes developed glaucoma. We studied the intraocular pressure of the cases with or without complications. Thirty-eight eyes had complications, fifteen eyes (39%) developed secondary glaucoma, and three eyes (8%) developed glaucomatous visual field defect. In twenty-three cases without secondary glaucoma, the intraocular pressure increased over a three-year interval after surgery and remained constant thereafter. 103 eyes had no complications to influence the intraocular pressure, twelve eyes (12%) developed secondary glaucoma, and four eyes (4%) developed glaucomatous visual field defect. In 91 cases without secondary glaucoma, the intraocular pressure increased over a five-year interval after surgery at 0.28 mmHg per year and remained constant thereafter. We recommend regular examination, especially during the initial five-year period when the intraocular pressure is apt to increase even if complications are not evident.

Missense mutation at the C terminus of the PAX6 gene in ocular anterior segment anomalies.

PURPOSE: To report a rare case of ocular anterior segment anomalies including uveal ectropion of the iris, invasion of the conjunctival epithelia into the cornea, and posterior embryotoxon with a missense mutation of the PAX6 gene. METHODS: The authors performed polymerase chain reaction-single-strand conformation polymorphism analysis and sequencing of the PAX6 gene using genomic DNA of family members and more than 100 control subjects. RESULTS: The A to G transition at nucleotide 1682 in exon 13 in the patient was identified in an allele that resulted in a Gln to Arg substitution (Q422R) at the C terminus of the protein. The mutation was not found in the parents, a sibling, or control subjects. CONCLUSIONS: The mutation indicates that the proline-serine-threonine-rich domain at the C terminus of the PAX6 protein plays a role in ocular anterior segment morphogenesis.

[An analysis of sclerodermatous graft-versus-host-disease after allogeneic bone marrow transplantation: CD8+CD57+T-cell proliferation and increased production of TGF-beta].

A 19-year-old woman with acute lymphoblastic leukemia received an allogeneic bone marrow transplantation (BMT) from an HLA-identical sibling during the second remission, on September 28, 1993. The conditioning regimen consisted of total body irradiation and cyclophosphamide. Short term methotrexate and cyclosporin A were given for prophylaxis of graft-versus-host disease (GVHD). On day 771 after BMT, she complained of bilateral forearm pain, and developed sclerotic lesions on the skin of the abdominal wall, forearms and legs. The diagnosis of sclerodermatous GVHD was established by skin biopsy on day 834. The values of CRP and IgG were elevated, and both antinuclear antibody and anti-DNA antibody became positive. Flow cytometric analysis showed a significant increase in the number of CD57+ cells after appearance of sclerotic change. In addition, 65% of CD8+ cells were positive for CD57. Circulating level of transforming growth factor (TGF)-beta 1 was high. These results suggest that overproduction of CD8+ CD57+ T cells and high level of circulating TGF-beta are related to the development of sclerodermatous GVHD.

Stability of metallothionein isoforms by capillary zone electrophoresis.

Stability of isoforms on storage of metallothionein (MT) was investigated by capillary zone electrophoresis (CZE) at neutral pH. The mouse liver was removed 1 day after injection of zinc, and homogenized in 0.25 M sucrose with 100 mM TRIS-HCl buffer, pH 7.4. The specimens for MT analysis were obtained by two partial purification methods, i.e., heat treatment (HT) and ethanol/chloroform precipitation (ECP) methods. The specimens were stored at either 4 degrees C or -80 degrees C for 1 and 3 days, and peak areas of MT-1 and MT-2 isoforms obtained by CZE were compared. When specimens were obtained by HT method, both MT isoforms were stable at -80 degrees C within 3 days. However, the peak area of MT-1 stored at 4 degrees C was gradually decreased to one third on the 3rd day. The MT-2 peak did not decrease the next day, but decreased to 57% on the 3rd day. In contrast, peak areas of both MT isoforms decreased remarkably the 1st day, when specimens obtained from ECP method were stored at 4 degrees C and -80 degrees C. From these results, we conclude that stability of MT isoforms is different depending on the purification methods, and MT isoforms are not stable after partial purification, especially by ECP method, even if stored at -80 degrees C.

[Internalized norms and social norms relating to negative health behaviors. The relationship of sociodemographic characteristics and reference groups].

So-called "negative health behaviors" (e.g. smoking, excessive eating/drinking), which are related to morbidity and mortality, are generally well known to the public. However there are many who undertake negative health behaviors despite knowing of their negative health effects. Using a mail survey of subjects aged 25 to 54, we examined reasons and justifications for engaging in negative health behaviors and their related factors from the lay person's perspective. Concerning the reasons for negative health behaviors, we hypothesized seven concepts (work priority, group harmony priority, the contribution to relaxation, tools for communication, prior orientation focused on the present, individualistic priority, and appearance), and devised multiple questions for each concept. The results are as follows. 1. Compared to females, males tended to support of the five concepts about the internalized norms concerned with negative health behaviors (e.g. the tendency of each individual to approve or disapprove of the reasons for negative health behaviors). In females, the younger generation and those that were married expressed more support than the older and the unmarried. There were differences in males according to occupation. 2. Concerning which reference groups had the largest influence on the social norms concerned with negative health behaviors (e.g. the perceptions as to what degree a group regards the reasons about negative health behaviors as desirable), most perceived that the "office" and "friends" groups would be more supportive of reasons for negative health behaviors than the "family" groups. 3. The health behaviors, which correlated with the internalized norms or the social norms concerned with negative health behaviors, were eating breakfast, smoking, and drinking (frequency and amount). In particular, drinking behavior was influenced most by the internalized norms and the social norms.

[Vertigo and dizziness related to platelet aggregability].

Platelet aggregates were measured in 82 cases with vertigo and dizziness and 60 cases with additional symptom of headache (so-called cervical vertigo). Results showed that patients with vertigo and dizziness had increased aggregability and patients with cervical vertigo were within normal range (p < 0.0001). 53 cases of these with vertigo and dizziness were administrated platelet aggregation inhibitors (mainly ticlopidine). After 1 approximately 2 weeks, most of patients became symptom free, and at this time platelet aggregates of these patients were suppressed to within normal range. Then patients were followed with chronological measurements of platelet aggregates. During this time, aggregabilities were attempted to be kept within the normal range, thus dose of platelet aggregation inhibitor was controlled depending on the result of aggregability. Also patients were encouraged to show good drug compliance by being informed of sudden increasing aggregability. One patient experienced recurrence of vertigo after ceasing the medicine. This study demonstrates that platelet aggregation may increase in most of patients with not only vertigo but also dizziness in whom administration of a platelet aggregation inhibitor is effective. It also stresses that chronological measurements of platelet aggregations are important to control the dose of medication.

[Analysis of inspiratory flow-volume curve].

Inhaled antiasthmatic agents can be effective, but their efficacy depends on the delivery system and on inhalation technique, especially in metered-dose inhalers (MDI). To find out whether inhalation therapy for asthmatics had been performed correctly, we studied inspiratory flow in the slow matter in healthy subjects and bronchial asthma patients. About half of 30 patients studied inspired more than 1 L/sec inspiratory flow rate, especially in male. Inspiratory flow in the fast matter (peak inspiratory flow; PIF) is important in the use of dry powder inhaler, so we studied the correlationship between PIF and expiratory flow and volume parameters. PIF correlated with PEFR, FEV1 and FEV1% in female, FVC and %FVC in male, respectively. In summary, we must recognize slow inspiratory flow can not be gotten easily, and asthmatic patients always have to use MDI carefully in inspiratory flow. Some expiratory parameters of spirometry can accurately predict PIF. The analysis of inspiratory flow-volume curve gives us beneficial data in inhalation therapy.

Thrombopoietin supports in vitro erythroid differentiation via its specific receptor c-Mpl in a human leukemia cell line.

Thrombopoietin (TPO) acts on megakaryopoiesis and erythropoiesis in vitro and in vivo. We isolated a novel subline, UT-7/GMT, from the human leukemia cell line UT-7/GM (N. Komatsu, et al., Blood, 89: 4021-4033, 1997). A small population of UT-7/GM cells positively stained for hemoglobin (Hb) after a 7-day exposure to TPO. More than 50% of TPO-treated UT-7/GMT cells positively stained for Hb. Using UT-7/GMT cells, we examined how TPO promotes hemoglobinization. TPO induced tyrosine phosphorylation of the TPO receptor but not the erythropoietin (EPO) receptor. There was no competition between TPO and EPO for binding to EPO receptor. These findings suggest that TPO has a direct effect on hemoglobinization via a specific receptor on UT-7/GMT cells. Isoelectric focusing demonstrated that TPO induced fetal and adult Hb synthesis, whereas EPO induced embryonic, fetal, and adult Hb synthesis. Thus, our data suggest that TPO has a distinct action on erythropoiesis.

[Three Japanese children with X-linked alpha-thalassemia/mental retardation syndrome (ATR-X)].

X-linked alpha-thalassemia/mental retardation syndrome (ATR-X), which was first reported by Wilkie, et al. in 1991, is a disorder with severe mental retardation, characteristic facial appearance, genital abnormalities, and mild form of alpha-thalassemia. At present, about 50 cases have been reported in the world, but few in Japan. We report 3 cases of this disorder in 2 families. All cases prefer a peculiar posture and show unique movements, such as self-induced vomiting or self-hanging, which can be diagnostic. ATR-X should be considered as a differential diagnosis in all male patients with severe mental retardation.

[TRH receptor].

Thyrotropin-releasing hormone (TRH) receptor (TRHR) cDNAs have been cloned from several species since 1990. Sequence analysis of these cDNAs showed that TRHR belongs to the family of G-protein coupled receptors containing seven transmembrane domains. The human gene spans more than 30 kb and contains three exons and two introns, which located on chromosome 8q23. Several new evidences have been reported including primary structure of TRHRs, distribution of TRHR mRNA, three dimensional structure of the TRH binding pocket, structure of the TRHR gene promoter, G-proteins and TRHR etc. Furthermore, a patient with central hypothyroidism caused by compound mutations of the TRHR gene and a novel transcript of TRHR gene in the pituitary have been recently reported.

[Immunocytochemistry of proteoglycan in dentin and odontoblasts].

To clarify the distribution of proteoglycan (PG) and matrix metalloproteinase-3 (MMP-3) in the molar tooth germ dentin of rats, immunohistochemical investigation was performed using a antibody 2-B-6 specific for chondroitin 4 sulfate (Ch4-S) and dermatan sulfate (DS) of PG, antibody 3-B-3 specific for chondroitin 6 sulfate (Ch6-S) and chondroitin (Ch) of PG and anti-human-MMP3 antibody. 2-B-6 reacted with the predentin and dentinal tubules. However, there were no reactions around the calcification front or in the calcified dentin matrix other than the dentinal tubules. 3-B-3 and anti-MMP3 antibodies reacted with only the predentin. These results showed that PG containing Ch4-S and DS was present in the dentinal tubules, while PG containing Ch4-S, DS, Ch6-S and Ch as well as MMP-3 was present in the predentin. It is suggested that PG of the predentin is degraded by MMP-3 at the calcification front. In undecalcified sections, the reaction of the dentinal tubules with 2-B-6 was localized in electron-dense sites corresponding to the peritubular dentin, suggesting that PG at this site binds to hydroxyapatite. Furthermore, in odontoblasts, the localization of 2-B-6 was noted in secretory granules distributed in the Golgi region and the odontoblastic process. This suggests that PG is transported from the Golgi region to the odontoblastic process in the form of secretory granules. Immunoreactive secretory granules were also noted in the cellular process of the calcified dentin. PG of the peritubular dentin may be secreted by these secretory granules in the process.

[Results of combination therapy of UFT-E with low-dose CDDP for patients with advanced recurrent breast cancer].

CDDP, as a modulator for 5-FU, has already been described as a very effective treatment for gastrointestinal tract cancer. We administered a dose of 400 mg of UFT-E orally every day, and 10 mg of CDDP by drip infusion twice weekly, for more than 10 weeks to 12 outpatients with metastatic local, pulmonary, hepatic, osteal or multiple-organ cancer which showed a poor response to the pretreatment, and assessed its efficacy and drug toxicity. In terms of the clinical efficacy of this therapy, CR was noted in one patient and PR in 2 patients with a response rate of 25%. The incidence of drug toxicity was low. Complications included temporal transient nausea and anorexia in two patients and leukopenia grade 2 as bone marrow suppression in 3 patients. From the standpoint of QOL, as well as in terms of both antitumor effect and drug toxicity, the therapy mentioned above was believed to be effective for outpatients with advanced recurrent breast cancer.

Lack of active lung anaphylaxis in congenitally mast cell-deficient Ws/Ws rats sensitized with the nematode Nippostrongylus brasiliensis.

Ws/Ws rats are deficient in both mucosal- and connective tissue-type mast cells. To study the role of mast cells in active anaphylaxis, changes in vascular permeability in the trachea upon intravenous antigen challenge with Evans blue dye were examined in Ws/Ws, heterogenic Ws/+, and normal +/ + rats sensitized with the nematode Nippostrongylus brasiliensis. Antigen challenge resulted in fatal anaphylactic shock in some +/+ and Ws/+ rats, but not in Ws/Ws rats. Marked dye leakage developed within 30 min in the trachea of +/+ and Ws/+ rats, while Ws/Ws rats showed no substantial increases in the levels of vascular permeability. Ex vivo stimulation of sensitized lung fragments from +/+ animals with specific antigen induced significant releases of histamine and leukotriene (LT) C4, while sensitized Ws/Ws rat-lung fragments did not. In Ws/Ws rats, levels of nematode-specific IgE, IgG1 and IgG2a antibodies as well as levels of lung eosinophilia were not significantly different from those in +/+ rats. These results show that mast cell-deficient Ws/Ws rats fail to develop active anaphylaxis, and this is mediated probably by the lack of mast cell-derived mediators required for initiation of the reaction.

[In vitro study on intrathecal application of 5-fluoro-2'-deoxyuridine (FdUrd) for meningeal dissemination of malignant tumor].

To evaluate the possible clinical intrathecal use of 5-fluoro-2'-deoxyuridine (FdUrd) for malignant brain tumors, its anti-tumor activity and neurotoxicity were compared with that of 5-fluorouracil (5-FU) and 5-fluorouridine (FUrd) in vitro. FdUrd showed good tumoricidal activity against cultured mouse 203 glioma cells and rat Walker 256 carcinoma cells as well as A172 human glioblastoma cells. Daoy human medulloblastoma cells and CADO-LC4 human lung cancer cells. It also showed less toxicity for primary cultures of neurons from C57/BL6 mouse and human embryo compared to 5-FU and FUrd. Thymidine phosphorylase (TPase) and thymidine kinase (TK), key enzymes for metabolism of 5-FU derivatives, were measured in cerebrospinal fluid (CSF). TPase or TK activity was detected in the CSF of hardly any patients with malignant brain tumors including meningeal carcinomatosis. These data indicated that the CSF is a favorable site for FdUrd chemotherapy, because the rate of conversion of FdUrd injected to 5-FU would be minimal. In conclusion, FdUrd may be potentially useful for intrathecal treatment of meningeal carcinomatosis.
