The Swiss Lithoclast: a new device for endoscopic stone disintegration.

Even in the presence of extracorporeal shock wave lithotripsy there is still the need for endourological treatment of stones in the urinary tract. Stone fragmentation usually is achieved with either ultrasonic, electrohydraulic or laser lithotripsy. We report our early experience with a new, simply constructed machine at a reasonable cost for endoscopic stone disintegration--the Swiss Lithoclast. The principle of this lithotriptor is based on pneumatic shock waves induced by the central compressed air system of a hospital or by a compressor. This device was used to treat 151 patients with stones in the kidney, ureter, bladder or a Kock pouch continent urinary diversion. Endoscopic fragmentation was successful in all patients. Independent of the composition, all stones were disintegrated within a short period, indicating that the device may well represent an attractive alternative to standard endoscopic lithotriptors.

[Quantification of the intra-penile smooth muscle with a fuzzy logic algorithm].

We have developed a new computer program for tissue segmentation designed for the quantification of smooth muscle in the corpus cavernosum. The program uses digitalized images of stained histological sections taken with a CCD camera. The recognition of the different tissues is based on the examination of absorption of monochromatic light. The section is successively illuminated with light of three different wavelengths and the three absorption values and texture parameters are used for the identification of the tissue using a fuzzy algorithm.

[Urethral trauma: retrospective study of 15 years].

In urethral traumas are included pendulous, bulbar and prostatomembranous injuries generally associated with a pelvic fracture or a symphysis diastasis. Because of the possible sequelae concerning potency and urethral stricture of prostatomembranous disruptions in peculiar, treatment remains controversial. Authors present a retrospective series of 27 patients (26 males, 1 female) treated during these last 15 years to try to modify or redefine their strategy and to choose the harmless method of treatment by a multidisciplinary approach between the anaesthesiologist, the traumatologist and the urologist.

[Neurophysiologic evaluation of erectile dysfunction].

The bulbocavernosus reflex and pudendal evoked responses were obtained in a group of 12 healthy male subjects and in 38 impotent men. The dorsal nerve of the penis was stimulated electrically. Responses were recorded over the bulbocavernosus muscle by a percutaneous method and over the cerebral cortex from the Cz-2 point.

[Erection disorders: the value, significance and limits of various examinations].

Erection, a complex physiological phenomenon, is the result of numerous simultaneously interacting mechanisms. A defect of one of these mechanisms can induce erectile dysfunction. In this paper, the authors define the various tests used to investigate the principal pathways governing erection, taking into account their respective value in view of the increasing pressure of budget restrictions on health systems.

[Priapism: review of current knowledge and presentation of a diagnostic and therapeutic algorithm].

In medicine, priapism is defined as pathological state: a prolonged, generally painful erection not accompanied by sexual desire not ending with an ejaculation. Rare before 1980, priapism became more frequent after introduction of erection-inducing drugs by intracavernosal injections for treatment of impotence. Therefore, the medical practitioner can be faced with this problem, and the authors present with an algorithm the current diagnostic and therapeutic steps based on knowledge of pathology, physiopathology and etiology of this disease.

[First clinical evaluation of the CHUV ballistic lithoclast].

The authors present the first clinical evaluation of the ballistic lithoclast CHUV based upon results of 55 treated patients. Their conclusion is that the lithoclast CHUV is a simple, reliable, easy to use, efficient and patient-friendly machine.

[Preoperative or palliative embolization of renal tumors].

This study evaluates our experience of renal preoperative or palliative embolization in 72 patients with a localized renal carcinoma or a metastatic disease. Angio-infarction of a renal malignant neoplasm is a helpful procedure to reduce the tumoral vascularization before an operation and to palliate symptoms and paraneoplastic syndromes of a metastatic disease with a very low morbidity.

[Long-term results of the Cobb and Badge method of bladder neck suspension in the treatment of urinary stress incontinence in women].

According to their authors, surgical techniques proposed to correct female stress incontinence give excellent short-term results. But there is no long-term evaluation and it is generally agreed that half of these patients will be incontinent again several years later. This study evaluates long-term results of the COBB and RADGE bladder neck suspension performed on 47 female patients. At five years 49% of patients are still continent or have improved. If this surgical technique gives no better results compared to others, it has the advantage of being simple and quickly executed.

[Coralliform calculi and recurrent urinary infection: a case of newberyite calculi (MgHPO4.3H2O)].

In some 20% of cases urolithiasis is directly linked to chronic urinary tract infection by urea-splitting bacteria and essentially Proteus mirabilis. The stones are generally composed of struvite and carbonate apatite, which usually occur as the staghorn calculi feared because of their obstructive and infectious consequences which cause important renal damage. A case is described of newberyite calculus in a woman with chronic urinary infection. Newberyite is thought to be a breakdown product of struvite in particular physico-chemical conditions, and is a very rare constituent of urolithiasis. X-ray diffraction and infrared spectroscopy were used to identify the constituent. Clinical and technical investigations are briefly described, and the literature is reviewed.
