Collateral circulation before and after coronary artery reconstruction.

The changes in collateral circulation after coronary artery reconstruction were analyzed in 75 patients with 177 bypass grafts and 18 complementary gas endarterectomies. The quantity of collateral circulation was directly proportional to the degree of coronary obstruction. The changes in collateral circulation depended on the success of coronary surgery or on the progression of the disease in coronary vessels. So, when the grafts remained patent, the collaterals disappeared and when the grafts became occluded, the collateral circulation returned to the preoperative level. Progressive obstruction of the coronaries caused increased collateral circulation.

Non-traumatic chylous effusion in the thorax and abdomen.

A female of 31 with chyloascites and bilateral chylothorax is presented. The thoracic duct was obstructed below the diaphragm. The lymph vessels in the left iliac and para-aortic areas were enlarged and there were lymphocysts. When the lymph loss was greatest the patient was in a state of grave malnutrition with marked hypoalbuminaemia and an absolute and relative lymphocytopenia in the blood. "Malignant" cells were demonstrated in the chylous fluid, but no malignancy could be found at laparotomy. It is possible that the cells were confused with immature lymphocytes. The lymphatic cysts were excised and the lymph vessels ligated. Decortication of the right lung was performed. The patient recovered. The follow up time has been over four years.

Regional renal blood flow in hypertensive patients.

Renal blood flow measurements in three regions of each kidney were carried out in eighteen hypertensive patients using the radioactive Xenon-133 washout technique in conjunction with renal angiography. It was found that the regional differences were generally small but differences existed in four patients significant in spite of normal angiographic findings. The reduction in blood flow in the two kidneys was generally of the same degree which implies a uniform response to the elevated blood pressure.

Reduction of motion unsharpness in coronary angiography using rare-earth oxysulfide intensifying screens and green-sensitive X-ray film.

During cardiac cycle the coronary arteries move rapidly; the maximum velocity can exceed 400 mm/s. To eliminate the motion unsharpness, exposure times under ten milliseconds are necessary. Using new rare-earth oxysulfide intensifying screens and fast green sensitive X-ray film the exposure times can be reduced to as little as 2 milliseconds. Thus the motion unsharpness can be almost completely eliminated, producing enhanced resolution and better diagnostic accuracy.
