Magnetic resonance (MR) scanning shows great promise as an imaging tool to evaluate the pelvis as a result of contrast resolution and lack of ionizing radiation. For these reasons, it is assuming an important role in staging gynecologic neoplasms, and evaluating diseases in the gravid pelvis. A case of uterine leiomyosarcoma is presented in which MR imaging was able to delineate the extent and tissue characteristics of the lesion more precisely than CT scan.
Previous studies have suggested that insulin may play a role in the hormonal regulation of neurotransmitter metabolisms within the central nervous system. In order to provide additional information to support this hypothesis, we examined the distribution of insulin receptors within the forebrain of adult male rats. Insulin receptors were localized by immunocytochemistry, using an antibody directed against the carboxy-terminus of the beta-subunit of the insulin receptor. The antibody specificity was tested by immunoprecipitation of brain insulin receptors with antiserum and the purity of the receptor-antibody preparation was determined using hormone binding-assays with radiolabeled insulin and insulin-like growth factor-l. Insulin receptor-like immunoreactivity was found in a widespread, but selective, distribution on neurons throughout the rat forebrain. Double-labeling with glial fibrillary acidic protein did not demonstrate any detectable insulin receptor-like immunoreactivity on glial cells. Areas with the highest density of insulin receptor-like immunoreactivity were found in the olfactory bulbs, hypothalamus and median eminence, medial habenula, subthalamic nucleus, subfornical organ, CA 1/2 pyramidal cell layer of the hippocampus and piriform cortex. Double-staining of hypothalamic sections with somatostatin and vasopressin antisera revealed insulin receptor-like immunoreactivity on a subpopulation of somatostatin neurons in the periventricular region and on vasopressin neurons in the supraoptic nucleus. A moderately dense insulin receptor-like immunoreactivity was observed in layers II-IV of cerebral cortex, medial amygdala, reticular thalamic nucleus, zona incerta, and preoptic and septal regions, whereas a low density of insulin receptor-like immunoreactive neurons was found in basolateral amygdala and most thalamic regions. The basal ganglia and most parts of the thalamus were almost devoid of insulin receptor-like immunoreactivity. Our findings provide morphological support for a direct action of insulin on selected regions of the rat forebrain and suggest that the insulin receptor may modulate synaptic transmission or the release of neurotransmitters and peptide hormones in the CNS.
The development of methods to isolate eukaryotic genes, alter these genes in vitro and reintroduce them into the cell has had a major impact on the study of recombination in the yeast Saccharomyces cerevisiae. In this paper we discuss how recombinant DNA techniques have been employed in the study of recombination in yeast and the results that have been obtained in these studies.

Angioid streaks were observed in a 31-year-old man with abetalipoproteinemia. Angioid streaks have now been described in hereditary spherocytosis, beta thalassemia, alpha thalassemia, sickle cell anemia, and acanthocytosis secondary to abetalipoproteinemia and hypobetalipoproteinemia. The nature of the association of angioid streaks and structural defects of hemoglobin and the red blood cell is not understood.
The carbon dioxide laser was used for the treatment of 20 lesions of the oral mucosa in 14 patients. These lesions ranged histologically from benign hyperkeratosis to verrucous carcinoma. The patients selected for this treatment were identified at high risk to develop malignant lesions in the oral cavity. Precise vaporization of the affected mucosa were carried out using the Cavitron 40-300-A CO2 Surgical Laser. Destruction of the surface epithelium and submucosa was achieved by using defocused beam at a setting of 10W. Biopsies were taken at 1 cm intervals at the time of laser treatment. Fourteen of the 20 procedures were carried out under local anesthesia on an outpatient basis. Seven procedures were performed under general anesthesia and 2 patients required 24 h postoperative observation. Local control was achieved in 17 of the 20 treated sites. Two of the three treatment failures occurred in patients in whom the final histology revealed either in situ or invasive squamous carcinoma. Only one patient with dysplasia was not controlled after vaporization of the lesion by the carbon dioxide laser. The laser continues to show encouraging results as an alternative to surgical resection of precancerous mucosal lesions of the oral cavity. Vaporization of the dysplastic lesion(s) with carbon dioxide laser is recommended for patients with an identifiable risk for the development of intraoral malignancy. This is an effective, nonmorbid, inexpensive, quick, and relatively painless method of managing this condition.

A case-control study of hypertension was conducted, using as a sampling frame a cross-sectional survey of Canadian Federal civil servants. Three case groups were selected, based on prior knowledge of hypertensive status and measured blood pressure. A 3-to-1 matching scheme was employed. Data was analysed using a logistic regression model. Statistically significantly elevated odds ratios, in excess of 3.0, were noted for overweight or obese status. Other odds ratios associated with a significantly increased risk of hypertension were maternal and paternal history of hypertension and current cigarette smoking.
The ability of surgeons to accelerate wound healing through pharmacologic intervention is limited. The effects of locally applied, biosynthetic human epidermal growth factor (EGF) and transforming growth factor-beta (TGF-beta) on tensile strength of experimental incisions were investigated. A single dose of EGF in saline failed to increase tensile strength over controls. Thus, EGF was incorporated into multilamellar liposomes, which prolonged the exposure of incisions to EGF (p less than 0.001). A single dose of EGF in multilamellar liposomes produced a 200% increase in wound tensile strength over controls between 7 and 14 days (p less than 0.05). Light and electron microscopy of the wounds revealed increased collagen formation and fibroblast proliferation. A single dose of TGB-beta in a collagen vehicle stimulated a 51% increase in wound tensile strength at 9 days (p less than 0.01). We conclude that addition of EGF and TGF-beta in appropriate vehicles stimulates early transient increases in wound tensile strength in normal rats.
In a study of the dexamethasone suppression test (DST) in patients with bulimia nervosa, a non-suppression rate of about 50% was found. The only clinical correlates of DST non-suppression were a previous history of weight loss and/or of anorexia nervosa. These results suggest that DST non-suppression in these subjects may be a trait rather than a state marker of anorexia nervosa.

Data on duration of current residence are used to estimate models of duration-dependent residential mobility rates. "We use the theory of backward recurrence times in renewal processes to infer the hazard rate of residential mobility from the density of the duration of current residence. We demonstrate that the inferences made using the asymptotic solution of the density of the backward recurrence times will be severely biased if the renewal process has not reached stability. We present an unbiased finite time solution and estimate univariate and multivariate models of residential mobility rates using 1980 U.S. census data on duration of current residence. These models include mover-stayer models represented by mixture densities. The multivariate analysis shows that the most important covariates of residential mobility are age, homeownership, race, and presence of school-aged children."
We have compared famotidine 40 mg, ranitidine 300 mg and placebo given in a single oral dose at 2200 hours as the sole means of prophylaxis in 286 patients who underwent elective surgery the following day. Standardised premedication was administered and anaesthesia induced. Gastric contents were aspirated by nasogastric suction and the pH and volume measured. Median values of pH after famotidine, ranitidine and placebo were 6.17, 6.74 and 2.45 respectively; median aspirate volumes were 8, 8 and 10 ml respectively. The proportions of patients considered 'not at risk' (pH greater than 2.5) were 90% after famotidine, 91% after ranitidine and 52% after placebo. We conclude that the administration of a potent H2-antagonist in a single oral dose at night offers a convenient routine means of providing extensive prophylactic cover in patients scheduled to undergo elective surgery the following day.
This study examined levels of anxiety in 50 parents who roomed in and 51 parents who did not room in with their hospitalized children. The Spielberger State-Trait Anxiety Inventory was used to measure parental anxiety at two specific times. Statistically significant correlations were found between parental anxiety and number of children at home, parental educational level, and family social status. Of clinical importance were findings that non-rooming-in parents reported higher anxiety than rooming-in parents at both testings.
