A 67-year-old woman with a history of thrombophlebitis had been taking warfarin sodium for 1 1/2 years when she developed multiple skin lesions resembling warfarin-induced skin necrosis or purpura fulminans. Despite discontinuing the warfarin and administering prednisone, the lesions increased in size. Disseminated intravascular coagulation (DIC) was found and resolved with heparin sodium therapy, and her skin lesions healed. This patient was believed to have an atypical form of purpura fulminans rather than warfarin-induced skin necrosis because of the duration of warfarin therapy and the dramatic resolution with heparin. A monoclonal (IgG) gammopathy was found, which stabilized as the skin lesions resolved, and fulfilled the criteria for a benign (asymptomatic) monoclonal gammopathy.
Joint allotransplantation has been employed in the management of osteoarthritis affecting primarily one compartment of the knee. Biological resurfacing and realignment of the knee has been the goal. The immunological results of six patients posttransplant were studied with the lymphocyte microcytotoxicity test. Definite immunogenicity was demonstrated at an early stage. The sera from the recipients posttransplant was noted to abrogate the in vitro cytotoxicity suggesting the presence of blocking factors. The actual clinical significance of this phenomenon was difficult to judge insofar as no evidence of allograft joint rejection was seen.
Rhodes' silver-plating technique for staining flagella was tested for its reliability and convenience as a routine procedure in the clinical laboratory. Modifications were made in the stain preparation and the procedure of staining and were tested with smears of known motile gram-negative nonfermentative bacilli. The stain has proved to be accurate and reliable and can be easily utilized with a minimum of training.
A patient presented with spontaneous subarachnoid hemorrhage (SAH) from a cerebral arteriovenous malformation (AVM) which was later totally removed at surgery. The patient presented again with a new SAH from a spinal AVM that was also totally removed at surgery. Coexistence of spinal and cerebral arteriovenous malformations are exceedingly rare and hemorrhage from each is not previously reported. This case emphasizes the importance of investigating the spinal canal in otherwise unexplained spontaneous SAH.

The EEG responses to intermittent photic stimulation were examined in a group of subjects experienced in meditation, and compared with those of a control group. The meditators exhibited a significantly smaller decrement in alpha activity and alpha blocking over the course of the experiment than did the control group, and alpha induction occurred earlier and more frequently in the meditators. These findings support the hypothesis that experienced meditators spontaneously enter the meditative state on closing the eyes, and also the view that physiologically the meditative state is one of prolonged drowsiness. An alternative interpretation, that meditation is a state of sustained attention, is discussed.
The effect of fenfluramine, administered orally in a daily dose of 1 mg/kg for 40 days, on the disposition and rate of elimination of antipyrine was studied in 15 obese patients. Although the plasma half-life of antipyrine was unchanged, the apparent volume of distribution (1/kg) fell by 11.6% (p less than 0.001) and the plasma metabolic clearance rate (1/kg/h) of antipyrine was reduced by 14.1% (p less than 0.01). No correlation occurred between change in clearance, on the one hand, and change in patient weight or apparent volume of distribution of antipyrine, on the other. In vitro drug-metabolizing enzyme activity in the rat was measured using rate of aminopyrine and hexobarbital metabolism as indices. Fenfluramine inhibited the metabolism of both substrates. It is concluded that fenfluramine can diminish the rate of elimination of drugs which are extensively metabolized by reduction of microsomal enzyme activity. Also, drug clearance may be diminished by reduction of apparent volume of distribution.
