[Urine free light chains kappa in multiple sclerosis].

The evaluation of free light chains kappa in urine was performed in 77 cases of multiple sclerosis, including 52 patients before and after treatment with 2 CDA and in 25 patients before and after therapy with high doses of prednisone. The high variations in the level of free kappa chains indicate a limited diagnostic value, and only for cases with very high level. We have found effect of 2 CDA therapy in chronic progressive MS group on free kappa light chain value. A significant effect of prednisone treatment was observed in early onset cases of multiple sclerosis and in cases with clinical improvement after therapy. In conclusion, the study suggests that urinary free light chains level may be considered as one of markers for monitoring of the effect of therapy on the activity of the immunological processes in multiple sclerosis.

[Immunological markers of disease activity in the course of 2-CDA treatment of multiple sclerosis].

52 clinically definite multiple sclerosis (MS) patients were treated with subcutaneous injection of 5 mg 2-CDA in 5 consecutive days. The injection courses were repeated 6 times in one month interval. The MRI pattern and immunological markers were studied in serum and CSF before and after 6 months of treatment. The obtained results suggest that treatment with 2-CDA has not any significant effect on humoral immunological events in multiple sclerosis, what is in contrast to some normalization of cellular immunopathological processes.

[Myelin basic protein stimulation index of CD 2 cells in the course of steroid treatment].

Myelin basic protein CD 2 lymphocyte stimulation index calculated as the proportion of percentage of myelin basic protein stimulated CD 2 lymphocytes to unstimulated CD 2 lymphocytes was evaluated in a group of 23 multiple sclerosis patients in the course of prednisone or methylprednisolone treatment. After both treatments a decrease of myelin basic protein CD 2 lymphocyte stimulation index was observed, statistically significant after methyloprednisolone medication.

[The effect of large dose prednisone therapy on immunological markers in multiple sclerosis].

The effect of large-dose prednisone therapy (3960 mg over 56 days) on humoral immunological markers and MRI pattern was studied in 25 patients with clinically definite multiple sclerosis. There was a decrease in levels of some studied markers (TNF in CSF, MBP level, IgG index, kappa chains in urine and kappa/creatinine ratio) after the treatment, but the differences reached statistical significance in some groups only. The decrease of total lesion area in MRI was not statistically significant. The obtained results proved clearly, that the above mentioned immunological markers might be helpful for monitoring of therapy in MS patients.

[Morphology of demyelination plaques vs cognitive and emotional disorders in multiple sclerosis patients].

The interrelationship between psychological examination and MRI findings was studied in 70 patients with MS. The cognitive and emotional functions were examined by a battery of tests: Wechsler Adult Intelligence Scale, Visual Retention Test, Hamilton Depression Scale. In MRI examination the localization, area, and the morphology of the plaques were examined. According to plaque's morphology the patients were divided into two groups: with confluent plaques and those with patchy-shaped ones. The signs of dementia were found significantly more frequently in the group with confluent plaques (p. < 0.04). In this group of patients also single-function disorders like disturbances of verbal memory, attention, visual memory, cause- and effect thinking, abstract thinking, and visual-motor coordination were significantly more frequent (p. < 0.01). In the same group the signs of fatigue syndrome were more frequently encountered (p. < 0.02). The authors conclude that the disturbances found in cognitive function may reflect the symptoms of subcortical dementia in MS patients.

[Changes of regional cerebral blood flow in stroke patients assessed by SPECT].

UNLABELLED: The aim of our work was to evaluate changes in regional cerebral blood flow (rCBF) in patients with TIA and/or completed stroke and localisation of fotopenic foci in basal conditions and, in some cases, after stimulation pharmacological trial with acetazolamide. One hundred and fifty three patients participated in the study--105 with completed stroke and 48 with TIA. Regional CBF (rCBF) was evaluated using a single-headed rotating gamma camera (Diacam, Siemens) with high resolution collimator, after injection of 740 MBq 99mTc-HMPAO (Ceretec, Amersham). Sixteen patients with TIA underwent SPECT at baseline and after injection of acetazolamide (ACZ). Relative regional perfusion was expressed as the ratio of cerebral/cerebellar activity. In the completed stroke population, the perfusion ratios were significantly lower than those of controls and TIA at baseline (p < 0.001) and TIA after ACZ (p < 0.01). CONCLUSIONS: This study shows a significant difference in rCBF in patients with completed stroke and/or TIA and controls. In patients with TIA and rCBF similar to that observed in controls, perfusion deficits may be visible after ACZ injection.

[Chemokines MIP-1 and MCP-1 and patients with multiple sclerosis treated with interferon beta-1a].

Chemokines: MCP-1 and MIP-1 alpha may play an important role in the pathogenesis of multiple sclerosis, influencing migration of lymphocytes to the CNS. One of possible mechanisms of interferon beta action may be an effect on chemokines. We measured MCP-1 and MIP-1 alpha chemokines in sera of 24 patients with MS treated with interferon beta-1a before and after 3 months of therapy and in 15 control patients. There was a significant increase of MIP-1 alpha concentration in sera of MS patients in comparison with control group. After 3 months of therapy with interferon beta-1a, MIP-1 alpha and MCP-1 levels did not differ from the values before therapy. Investigations will be continued after longer time of treatment with interferon beta.

[New data concerning the significance of immunoglobulins and B cells in the pathogenesis of multiple sclerosis].

Contemporary studies concerning the significance of immunoglobulins in the mechanism of multiple sclerosis are based on the analysis on the genome level. It was found that genes encoding immunoglobulins are restricted to active gene segments and showed a great frequency of mutations in sequences encoding the complementarity-determining regions (CDR s). The above presented results led to the conclusion, that synthesis of immunoglobulins and activation of B cells are not only the result of nonspecific reaction but are the antigen-driven response, related to the immunological mechanism of the disease.

[Phenotype characteristics of mononuclears in peripheral blood and csf in multiple sclerosis].

In 48 clinically definite cases of multiple sclerosis the studies were performed of mononuclears phenotypes in CSF and peripheral blood. The results lead to following conclusions: 1. The percentage of cells with dominant reactivity of Anti HLADR, CD14 and CD56 in CSF is markedly lower than in the peripheral blood. 2. There exists a marked difference in the percentage of cells in CSF with reactivity anti-HLADR, CD14 and CD56 between patients in the relapse-remitting and chronic progressive phases of multiple sclerosis.

[Expression of surface antigens on lymphocytes stimulated by non- specific mitogens in multiple sclerosis patients].

In studies performed on 30 MS cases, as compared to 16 healthy individuals, significantly higher expression of HLA class II antigens, interleukin 2 receptor and antigens specific for lymphocytes B was observed. In MS patients after stimulation by non specific mitogens increased expression of HLA class II antigens and increased proliferation induced by PMA was found. The obtained results suggest absence of any greater modulation of surface antigens on the lymphocytes stimulated by non-specific mitogens. Our observations indicate that the number of activated T cells, as assessed by expression of Il-2 receptor, is in MS patients relatively low.

[IgG1-IgG4 subclasses in the cerebrospinal fluid and blood serum and their synthesis in the central nervous system in multiple sclerosis].

IgG1, IgG2, IgG3 and IgG4 subclasses were detected in cerebrospinal fluid and sera from 32 patients with MS. The CSF levels of IgG1 and IgG3 subclasses in MS patients were significantly higher than those of controls. There were no statistical differences between the values of IgG subclasses in MS and control sera. The IgG1 and IgG3 indices were significantly higher in MS patients than in controls showing that among four IgG subclasses IgG1 ang IgG3 were synthesized in the central nervous system of patients with MS. The elevation of IgG1 and IgG3 indices in MS patients was found more frequently (in about 90% of patients) than the elevation of the general IgG index (in 72% of patients).

[Multiple sclerosis--studies of the correlations between selected parameters of immune response and the clinical course of the disease].

In the paper the following immune response parameters were analyzed: number of leucocytes in the cerebrospinal fluid, IgG index, presence and number of oligoclonal IgG bands. The correlation between these parameters and between the parameters and clinical course of multiple sclerosis was studied. Oligoclonal bands were present in the cerebrospinal fluid in 92% cases of MS, and that was the most frequent abnormality among immune response parameters studied. There was a correlation between IgG index, number of leucocytes in the cerebrospinal fluid and number of oligoclonal IgG bands in patients with relapsing-remitting course of the disease. We did not find any statistical differences between the number of leucocytes in the cerebrospinal fluid, the frequency of pleocytosis and the type of the disease, relapsing remitting or chronic progressive. The type of the disease, its duration and number of relapses did not significantly influence the IgG index values and number of IgG oligoclonal bands in the cerebrospinal fluid.
