Predictive value of the index of desirable body weight for total body fat mass as measured by dilution of tritiated water--problems and limitations.

The index of desirable body weight is currently used for classification of patients' weights. We examined how it is related to the degree of adiposity, and whether it can serve in the prediction of body fat to the same degree as Quetelet's index. Sixty-five women and 142 men comprising a wide range of weight indices were studied. Body fat (F) was measured by the tritium dilution technique with a precision of +/- 1.26 kg. Regressions of F/W0 on the index of desirable weight W/W0 gave a correlation coefficient of 0.969 for women and 0.939 for men. Regressions of F/H2 on Quetelet's index W/H2 had correlation coefficients of the same order, i.e. 0.971 for women and 0.936 for men. Hence both indices are equally powerful for calculation of body fat within the study population. Further progress was achieved by multiple regression of F on W and W0 which yielded correlation coefficients of 0.976 for women and 0.953 for men. The residual standard deviation reflecting the mean difference between calculated and estimated fat was 3.4 kg for women and 3.8 kg for men. Despite this self-consistency, prediction of body fat for persons outside the study population is only possible within wide limits. These are given by about twice the residual standard deviation. Therefore, prediction of body fat from weight indices can only be used for detection of larger abnormalities in body composition.

[Results of protein-sparing modified fasting in hyperlipoproteinemias].

We investigated the influence of a PSMF on body weight, body composition, blood lipids, physical working capacity, and nitrogen balance in obese HLP patients. During 4 weeks, 12 patients were treated by PSMF (approximately 340 kcal/d) under health resort conditions. At the same time, they daily exercised on a bicycle with an intensity of 80% of their physical working capacity estimated by an exercise load. The therapeutic regimen induced a significant weight loss (10.1 kg in the mean). Evidently, the patients diminished their body fat only as could be shown by estimations of total body water. Triglycerides, total cholesterol, and LDL-cholesterol also decreased significantly, while HDL-cholesterol remained unchanged. In spite of the weight reduction, the physical working capacity significantly increased from 57.8 to 93.2 kgm. Measurements of total nitrogen excretion with urine gave evidence of the protein saving effect of the therapy. Thus, PSMF--representing an effective therapeutic procedure--can be recommended to be used in obese HLP patients, proceeded that contraindications have been taken into consideration carefully.

[Field bean protein diet in hypercholesteremia].

The lipid-lowering effects of Vicia faba protein are well-known only from animal experiments. According to these studies Vicia faba protein is the most powerful cholesterol-lowering agents among vegetable proteins. Therefore we examined these effects in a group of patients with hypercholesterolemia (HLP type IIa). We observed a cholesterol-reducing efficacy of the Vicia faba protein comparable with those under soy protein diets.

The C4 beta-chain: evidence for a genetically determined polymorphism.

Using sodium dodecyl sulphate polyacrylamide gel electrophoresis (SDS-PAGE) five different beta-chain patterns of denaturated human C4 were observed: C4 beta H, beta H greater than L, beta H = L, beta H less than L, and beta L. In 32 families with 174 offspring the segregation of C4 beta-chain types followed that of the other major histocompatibility complex (MHC) markers. It is concluded that the C4 beta-chain patterns represent a genetically determined polymorphism, coded within the known C4A and B gene region.

[Hyperinsulinemia in obesity: significance of adipose tissue cell size].

Adipose tissue biopsy and an oral glucose tolerance test (OGTT) (50 g) were performed in 17 non-hyperlipoproteinemic subjects without overt diabetes mellitus. All the persons were weight stable at the time of investigation. A significant correlation between fasting insulin concentration and the mean adipocyte size was observed, whereas no correlation was noted between the ideal body weight index and fasting insulin level. Persons with larger adipocytes had elevated insulin levels as well as higher and longer lasting increments following the glucose challenge. They also exhibited significantly higher mean glucose levels during the OGTT. When these patients were matched for glucose tolerance with the subgroup having smaller mean adipocyte sizes, the difference in insulin levels was still demonstrable. The importance of adipose cell enlargement regulating basal and stimulated insulin output is underlined.

[Apolipoproteins: analytical methods, chemical composition, metabolism and diagnostic relevance].

The estimation of apoprotein profiles makes an essential contribution to the study of the lipid metabolism, particularly of the normal and deranged lipid transport. For the clinician patterns of apoproteins might open new possibilities of the differential diagnosis of dyslipoproteinaemias. Four fundamental apoprotein profiles are presented by Alaupovic et al. (2): 1. Significantly reduced serum levels of the apolipoproteins AI, AII and B (essentially corresponding to HLP type I) 2. Increased values of apo-B (types IIa, IIb) 3. Increased concentration of apo-C-III reduction of the CI/CII-quotient (types IIb, IV) 4. Increased levels of the apolipoproteins C and E (types III, V). The authors suppose that these four basic patterns of apolipoproteins reflect the fundamental biochemical defects of the lipoprotein transport. The biochemical characterization of certain, genetically determined disturbances of the lipid transport with the help of apoprotein profiles is the best example for the essential role of these proteins in the lipid metabolism with regard to the clinical diagnostics it is at present not yet possible to get additional informations with the help of the estimation of apoproteins as they would not be got by far simpler methods. As was already mentioned, in these cases the rare genetically determined disturbances of the lipid metabolism and the diagnostics of the HLP type III form an exception. Only few authors try to prove that the proportion of proteins in lipoproteins seems to be somewhat better suited for the differentiation of persons endangered by arteriosclerosis than the proportion of lipids. Especially the increase of concentration of the apo-B is to be regarded as significant factor of risk of a chronic ischaemic heart disease. This is unequivocally confirmed, but the essentially simpler estimation of the LDL-cholesterol yields well comparable results with regard to the characterization of persons, endangered by chronic ischaemic heart disease.
