Pancreatic carcinoma cell killing via adenoviral mediated delivery of the herpes simplex virus thymidine kinase gene.

OBJECTIVE: Use of adenoviral mediated delivery of the herpes simplex virus thymidine kinase (HSV-TK) gene as a gene therapy strategy for carcinoma of the pancreas. SUMMARY BACKGROUND DATA: Expression of HSV-TK selectively sensitizes cells to the nucleoside analog ganciclovir (GCV). This strategy has been used to treat other compartmentalized tumor models. Therefore, the containment of pancreatic carcinoma makes it amenable to this gene therapy approach. METHODS: A recombinant adenoviral vector encoding the HSV-TK gene was used to induce GCV sensitivity and test the potential bystander effect in established pancreatic carcinoma cell lines and patient-derived tumor material. Additionally, Balb/C nude mice were injected intraperitoneally with human pancreatic carcinoma cells and treated with GCV (50 mg/kg per day) for 14 days. RESULTS: Expression of the HSV-TK gene elicited a significant bystander effect in the presence of GCV. Pancreatic tumor cells injected intraperitoneally into nude mice resulted in significant tumor formation. Treatment of animals with AdCMVHSV/HSV-TK and GCV induced a dramatic decrease in overall tumor burden for up to 8 weeks post-GCV treatment. CONCLUSIONS: Pancreatic carcinoma cells are highly susceptible to transduction with recombinant adenoviral vector and elicit a potent bystander effect on neighboring tumor cells. Additionally, in vivo treatment of tumor-bearing animals results in dramatic reduction of overall tumor burden, thus providing the rationale for molecular chemotherapy of pancreatic carcinoma.

Rhinocerebral mucormycosis: evolution of the disease and treatment options.

Rhinocerebral mucormycosis is recognized as a potentially aggressive and commonly fatal fungal infection. The classic presentation is involvement of nasal mucosa with invasion of the paranasal sinuses and orbit. Mucormycosis is most commonly seen in association with diabetic ketoacidosis, but disease demographics have changed with the onset of AIDS and the advent of powerful immunosuppressive drugs. Treatment includes aggressive debridement, systemic antifungal therapy, and control of underlying comorbid factors. Although surgical intervention remains essential, advances in medical therapy have permitted a more limited surgical approach to minimize functional loss without compromising survival. We present the UCLA experience with rhinocerebral mucormycosis from 1955 to 1995, with emphasis on the evolution of disease presentation and alternative treatment options.

Self-assembly of the JC virus major capsid protein, VP1, expressed in insect cells.

The major capsid protein of human polyomavirus JC virus, VP1, has been cloned into a baculovirus genome and expressed in insect cells. The VP1 protein was expressed in the cytoplasm and transported into the nucleus. It was then purified by a sucrose cushion and CsCI density gradient centrifugation to near homogeneity. Electron microscopy showed that isolated recombinant VP1 protein self-assembled into a capsid-like structure similar to the natural empty capsid. Both chelator (EDTA) and reducing agent (DTT) are required to disrupt the capsid structure into the pentameric capsomeres, as demonstrated by haemagglutination assay and electron microscopy. These results suggest that JC virus VP1 can be transported into the nucleus and self-assembled to form capsid-like particles without the involvement of the viral minor capsid proteins, VP2 and VP3. In addition, metal ions and disulphide bonds appear to be important in maintaining the integrity of the viral capsid structure.

Activation of the human histamine H2 receptor is linked to cell proliferation and c-fos gene transcription.

We examined whether histamine could regulate cell proliferation and expression of the early response gene c-fos in HEK-293 cells stably transfected with the human H2 receptor (HEK-H2). Histamine stimulated [3H]thymidine incorporation [50% effective concentration (EC50) = 3.6 x 10(-6) M] in HEK-H2 cells in a cimetidine-sensitive manner and increased c-fos mRNA in a time-dependent fashion, reaching maximal induction after 30 min. Histamine induced luciferase activity in HEK-H2 cells transiently transfected with a construct containing the luciferase reporter gene (Luc) coupled to the serum response element (SRE) of the c-fos gene promoter (EC50 = 1.5 x 10(-6) M) or a plasmid containing the SRE core fragment (bases -320 to -298). The protein kinase C (PKC) inhibitor staurosporine and long-term pretreatment of HEK cells with phorbol ester inhibited the effect of histamine on PKC activation, SRE-Luc activity, and [3H]thymidine incorporation. We have demonstrated that activation of the human H2 receptor can lead to induction of c-fos gene transcription and cell proliferation through a PKC-dependent mechanism.

Self-reported depression and risk-taking behaviors among Hispanic adolescents.

In assessing the relationships between self-reported depression and certain key risk-taking behaviors among a national sample of Hispanic adolescents (N = 935, ages 12 to 19 years), girls who reported depression were significantly more likely to engage in these behaviors than girls who did not report depression. No significant differences were found among boys (n = 470), although they were more likely than girls (n = 465) to take risks regardless of depressive state. These results suggest that depressive symptoms may be a market for engagement in risk-taking behaviors among Hispanic girls.

Changes of mu opioid receptor binding sites in rat brain following electroacupuncture.

With [3H]-ohmefentanyl as a ligand, autoradiographic technique was used to observe the effect of electroacupuncture (EA) on mu opioid receptor binding sites in the brain areas related to pain modulation. The results were as follows: (1) The distribution of the mu receptor in the rat central nervous system was consistent in general with the results reported previously. (2) After EA of Tsu-San-Li, the mu receptor binding sites were increased significantly in the following examined structures: the caudate nucleus, septal nucleus, medial preoptic area, amygdalaoid nucleus, periaqueducal gray, interpeduncular nucleus, nucleus raphe magnus, and cervical and lumbar enlargements. The results indicate that EA is able to increase mu binding sites in the brain areas related to analgesia, suggesting the enhancement of mu receptor function by EA.

[Assessment of sputum culture and serology for detection of chlamydia pneumoniae in patients with symptomatic acute respiratory tract infection].

OBJECTIVE: To evaluate sputum culture and serology for detection of chlamydia pneumoniae (C. pneumoniae) in adult patients with acute respiratory tract infection and normal subjects. METHODS: Microimmunoflorescence (micro-IF) test for IgG and IgM antibodies to C. pneumoniae specific antigen was performed in every subject. Thirty-seven patients also had sputum HEP-2 cell culture tested for C. pneumoniae. RESULTS: Twenty-six patients (29.7%) with micro-IF IgG titres equal to or greater than 1:512 or/and IgM titres equal to or greater than 1:32 indicated recent infection. Eight patients (21.3%) with positive sputum culture of C. pnuemoniae were identified. No similar titer was shown in control normal subjects. CONCLUSIONS: Acute infection of C. pneumoniae was common in symptomatic patients with acute respiratory tract infection and attention should be drawn to this special illness.

[The correlation of TGF-alpha, EGFR in precancerous lesions and carcinoma of stomach with PCNA expression].

OBJECTIVE: To assess the changes and possible role of expression of TGF-alpha and EGFR in gastric carcinogenesis and its relationship with PCNA labelling index (PCNA LI). METHODS: Immunohistochemical study using LSAB kit. RESULTS: (1) The expression of TGF-alpha was increased in normal mucosa and metaplastic tissue near the carcinoma when compared with non-cancer controls (P < 0.01). (2) Increased expression of EGFR was found in the intestinal metaplastic and dysplastic areas compared with nomral mucosa and carcinoma tissue (P < 0.01). (3) Coexpression rate of TGF-alpha and EGFR was higher in dysplasia than in other tissues (P < 0.01). (4) There was a close correlation between the intensities of TGF-alpha, EGFR and PCNA. (5) The expression of TGF-alpha, EGFR and PCNA was not related to infiltration and lymph node metastasis of gastric carcinoma. CONCLUSION: The increased expression of EGFR, TGF-alpha may serve as an important molecular marker of gastric premalignant lesions when combined with measurement of PCNA LI and may be of assistance in screening of early gastric carcinoma in high risk populations.

Active site of trichosanthin acting as a ribosome-inactivating protein.

AIM: To localize the active site of ribosome inactivation of trichosanthin (Tri), a Chinese herb protein. METHODS: Hydroxylamine was used to specifically cleave the unique Asn-Gly peptide bond of Tri. Preparative SDS-polyacrylamide gel electrophoresis was applied to get 2 cleaved fragments, HATf1 and HATf2. Western blotting was used to determine the different epitopes of Tri and screen the antibodies. A cell-free system, rabbit reticulocyte lysate, was introduced to quantitate the inhibitory activity of Tri and its fragments on protein biosynthesis. RESULTS: HATf1 and HATf2 were separated with the purity of 96.9% and 80.5% respectively. HATf1, like intact Tri, retained the inhibitory activity on protein biosynthesis. The mAb No 14 and No 16 against Tri showed different immunoreactivities with 2 fragments and were selected as representatives in further blocking tests. The mAb No 14 hindered the activities of Tri and HATf1, whereas the mAb No 16 did not. CONCLUSION: The active site of Tri responsible for inhibitory activity on protein biosynthesis was on the HATf1 side near the junction of two portions.

[Establishment of a new rat model of portal hypertension by intraportal injection of microspheres].

Microspheres were injected intraportally to block intrahepatic portal radicals and to produce a new rat model of portal hypertention. Different sized microspheres (15 microns, 40 microns, 80 microns) were injected into the portal vein. The resultant changes in arterial, portal, hepatic venous and splenic pulp pressures were monitored. The results showed that a small-dose injection of 80 microns microspheres (1.8 x 10(5)) produced a steady state portal venous pressure of 2.53 +/- 0.17 kPa, and all rats showed a normal arterial pressure. This volume of 80 microns microspheres was considered as the suitable dose for the induction of portal hypertention in the model. Besides, numerous microspheres were found in the pulmonary vascular bed, suggesting the existence of intrahepatic portal-systemic shunts in the normal rat liver.

[Effects of nocturnal oxygen therapy and continuous positive pressure ventilation on nocturnal oxyhemoglobin desaturation of chronic obstructive pulmonary disease patients with acute exacerbation].

To evaluate the effect of nocturnal oxygen therapy (NTOT) and NTOT plus continuous positive pressure ventilation on nocturnal oxyhemoglobin desaturation (NOD) of chronic obstructive pulmonary disease (COPD) patients with acute exacerbation, we monitored 58 COPD patients accompanied with NOD with pulse oximetry day and night. Arterial blood gas analysis was done before and after night sleep. Their PaO2, PaCO2 and basic SpO2 measured in day time were positively related to their nocturnal mean SpO2 (corresponding r = 0.702, 0.613, 0.605, all P < 0.01). Under NTOT, 37 patients showed favorable response showing elevation of the average values of nocturnal mean SpO2 (MSpO2) (P < 0.01), nocturnal minimal SpO2 (mSpO2) (P < 0.01) and decrease of cumulative time of NOD% (CTNOD%) (P < 0.01) in contrast to the values obtained when they breathed in air through whole night. However, another 21 patients failed to respond favorably on NTOT. When they were put on NTOT plus BiPAP ventilation, remarkable improvement of all indices of nocturnal SpO2 was found, and there was also an elevation of daytime PaO2 (P < 0.01) and lowering of PaCO2 (P < 0.01). We conclude that nocturnal oxygen therapy can release NOD of most part of COPD patients, while those who did not respond well to NTOT will improve favorably when continuous positive pressure ventilation was added.

[Corneal topographic analysis of small incision in cataract surgery].

OBJECTIVE: The study was designed to investigate the corneal topographical and corneal curvature changes of small incision in cataract surgery. METHODS: 38 eyes of 36 patients who had undergone small incision cataract surgery were followed up at one week and one, three, six and twelve months postoperatively by corneal topography and keratometer. RESULTS: At one week postoperatively, there was slight steeping or flattening at the center and peripheral cornea, but the change was recovered to the preoperative state after one month. No significant changes had been observed after a long-term observation. The result of keratometer was different from that of topography. CONCLUSIONS: It is clearly demonstrated that smaller wound incision produces less astigmatism, faster postoperative recovery and more stable refraction. The results of corneal topography reflect curvature changes more reliably and precisely than that of keratometer, and topography is of great value in cataractous surgical evaluation.

[Effects of captopril on the myocardial performances and hemodynamics after cardiac arrest].

UNLABELLED: The effects of cardioplegia and reperfusional blood containing 4.6 mumol/L captopril (CPL) on the myocardial performances and hemodynamics of canine heart under hypothermic extracorporeal circulation were studied. Twenty four mongrel dogs were divided randomly into 3 groups. CONTROL GROUP: Only modified St. Thomas cardioplegia was used. CPL group: St. Thomas cardioplegia containing CPL 4.6 mumol/L was used and the same concentration of captopril in the circulatory blood was established before releasing the cross clamp. CPL + phenylephrine group: The concentration was the same as the CPL group, with an additional dose of phenylephrine (2 micrograms/kg) into the oxygenator and abdominal coarctation. The results revealed that the coronary blood flow and left ventricular contractility and relaxation indices of CPL group were significantly improved than those of the control group, and increasing the systemic and pulmonary vascular resistence had no effect on the cardiac function protected by captopril. It is suggested that captopril could ameliorate the recovery of myocardial performences by inhibitting myocardial and systemic renin-angiotensin system and scavenging free radicals.

[The relationship between pulmonary function and work ability of aging workers].

To explore the relationship between pulmonary function and work ability, a field study was conducted in 285 aging workers (aged 40 to 60) of different occupations in Chengdu area. The pulmonary function indices included FVC, FEV1, FEV1%, MMEF, and the pulmonary function index (PFI) which was the sum of the rating values from the first three indices. The work ability index (WAI) was also measured. Between work types, the indices, including FVC and FEV1%, showed significant difference (P < 0.05); FVC also showed significant difference between male and female workers (P < 0.05). There was a significant relationship between PFI (including FEV1% and MMEF) and WAI (P < 0.05), but the relationship between FVC (or FEV1) and WAI was not statistically significant (P > 0.05). Moreover, the agreement percentage between the classifications of PFI and WAI was 43.8%; the disagreement percentage was 4.56%. The results suggest that FEV1% and MMEF may be beneficial to the objective evaluation of the work ability of aging workers.

[Evidence for a major role of genetic factors in the etiology of laryngeal and hypopharyngeal carcinomas].

Several investigations revealed that lymphocytes from patients with head and neck cancers showed a high sensitivity to mutagens. However, whether lymphocytes from healthy blood relatives of the patients were sensitive to mutagen or not had not been reported. This study was aimed to investigate the unknown problem. Mutagen sensitivity was investigated in 116 laryngeal and 38 hypopharyngeal cancer patients, 107 healthy controls, and 30 blood relatives of laryngeal or hypopharyngeal cancer patients. Chromosome damage was measured by enumerating mutagen-induced chromatid breaks, the average number of chromatid breaks per cell (b/c value) was used as marker which reflects variable host susceptibility to the action of environmental carcinogens. The b/c values were 0.61 +/- 0.27, 0.66 +/- 0.31, 0.28 +/- 0.12, and 0.45 +/- 0.26 for patients with laryngeal cancer, hypopharyngeal cancer, healthy controls, and blood relatives respectively. A significant difference of b/c value was found between patients and controls (P < 0.01), but not between patients and their blood relatives (P > 0.05). Our findings suggest that genetic predisposition plays an important role in the eitiology of laryngeal and hypopharyngeal cancers. Both patients with laryngeal or hypopharyngeal cancers and their blood relatives should be considered as in high cancer risk.
