Congenital muscular dystrophy with laminin alpha 2 chain deficiency: identification of a new intermediate phenotype and correlation of clinical findings to muscle immunohistochemistry.

UNLABELLED: The laminins comprise of a family of heterotrimeric proteins of the extracellular matrix. The crossshaped proteins consist of a heavy alpha-chain and two light chains, called beta and gamma. Each group of chains, classified on their sequence identity and domain organization, include different isoforms. A deficiency of the alpha 2 chain of laminin-2, previously termed merosin or M component, was shown to be responsible for one form of congenital muscular dystrophy (CMD). We investigated muscle biopsies of 20 patients with the clinical diagnosis of CMD and histological evidence of muscular dystrophy for the expression of different laminin chains. Patients with evidence of pachygyria/lissencephaly of the CNS were excluded from this series. The immunohistochemical analysis was correlated to clinical findings and MRI data of the brain. Of 20 patients, 11 (55%) revealed complete or near-complete deficiency of the alpha 2 chain in their skeletal muscle specimens. So far none of these patients became ambulant. Of 20 patients 2 showed partial but clear-cut alpha 2 chain-deficiency. These two patients became ambulant at 18 months and 3 years. All 13 patients with complete or partial alpha 2 chain-deficiency demonstrated cerebral white matter changes on MRI. In contrast, 6/7 CMD patients with normal alpha 2 chain expression became ambulant and none of the 6/7 tested showed evidence of cerebral abnormal T2 sequence signal of the myelin on MRI. CONCLUSION: Our findings emphasize the use of immunohistochemistry for laminin alpha 2 as a diagnostic tool in defining CMD and characterize a milder phenotype with partial alpha 2 chain-deficiency.

[Early manifestation of polycystic kidney changes in tuberous sclerosis].

Tuberous sclerosis (McKusick number: 19110) is well documented as an inheritable autosomal dominant disorder. On the occasion of results of an autopsy of a male fetus (1950 g, 29th gestational week) we report on the very early onset of a giant cell astrocytoma (microcalzified focally) and a cystic kidney disorder which in association with this phacomatosis is described first for this age. Thus, based on sonography there is a new differential diagnosis for the weighty prenatal diagnosis of a "Potter syndrome" or of "cystic kidneys". Moreover, on a chromosomal level we discuss possible connections between tuberous sclerosis and distinct forms of autosomal dominant polycystic kidney disease, ADPKD.

Laparoscopic stapler appendectomy. A prospective study of 267 consecutive cases.

BACKGROUND: The value of laparoscopic appendectomy remains controversial. Therefore, we investigated the accuracy of diagnostic laparoscopy in detecting acute appendicitis and tested the applicability and safety of stapling appendectomy as a routine procedure. METHODS: Data from 267 consecutive patients with suspicion of acute appendicitis were recorded prospectively. RESULTS: Histopathological examination revealed nonperforated and perforated appendicitis in 63.3% and 13.1%, respectively, and no inflammation in 10.8%. Other pathological findings were observed in 12.7%. Diagnostic laparoscopy detected appendicitis with a sensitivity and specificity of 95.6% and 96.6%, respectively; the positive and negative predictive value were 99.5% and 74.3%, respectively. Morbidity was 10.2% in total and 40% for perforated appendicitis. Planned laparoscopic reexploration reduced morbidity by 23.4% in patients with perforated appendicitis and substantial peritonitis. Mortality was 0.4%. CONCLUSIONS: Laparoscopy improves diagnostic accuracy for acute appendicitis and laparoscopic stapling appendectomy is a safe and efficient procedure for all forms of appendicitis.

CNV, PINV and probe-evoked potentials in schizophrenics.

Distractibility and temporal modulation of attention in schizophrenics were studied using a visual reaction time task with additional auditory probe stimuli during the forewarning period or between trials. The probes were thought to exert a distracting influence, especially on schizophrenics, and at the same time they generated auditory EPs which allowed to track the modulation of cortical excitability during response preparation. The midline distribution of the terminal contingent negative variation (tCNV) and the amplitude of the postimperative negative variation (PINV) were clearly different in 20 DSM III-R schizophrenics, as compared with 20 alcoholics and 20 normal controls. In schizophrenics, the more frontal distribution of the tCNV was associated with a higher degree of psychopathology (measured with the Brief Psychiatric Rating Scale) and with delayed reactions. Probes between trials reduced tCNV and PINV in all subjects alike. However, this effect could not be attributed to distraction, because reaction times were faster in these trials, possibly due to an alerting effect of the auditory probes. The N100 and P300 amplitudes to probes in the forewarning period, i.e., during the negative potential shift of the CNV, were significantly enhanced in all groups. Apparently there is a state of increased cortical excitability during the CNV which is not selectively "tuned" toward relevant stimuli. In schizophrenia, the temporal and topographical regulation of this excitability is disturbed.

The effect of papermill wastewater and organic amendments of sodium accumulation by potted cottonwoods.

In this experiment, the impacts of pulp mill effluent irrigation, Fraser cottonwood (Populus deltoides 'Fraser') seedlings, and pulp sludge and manure soil amendments on sodium accumulation and distribution in the soil profile were evaluated during a 6 month greenhouse study. Sludge soil amendments and wastewater irrigation did not reduce stem biomass production of the cottonwood. Increased stem biomass production associated with manure soil amendments resulted in greater total uptake of sodium into stem material. This uptake was 0.002% of wastewater sodium inputs. In containers with seedlings, sodium concentrations were less in the surface horizon and more in the lower horizons than in containers with no seedlings. Infiltration rates and total sodium accumulation in the soil profile were not affected by the presence of Fraser cottonwood or the application of sludge or manure amendments to soil.

Phototoxicity of argon laser irradiation on biofilms of Porphyromonas and Prevotella species.

Species of Prevotella (Pr.) and Porphyromonas (Po.) and other microorganisms were cultivated as biofilms on agar medium and examined for their susceptibility to argon laser irradiation (continuous mode; wavelengths, 488-514 nm; fluences, 20-200 J cm(-2)). Fluences of 35 to 80 J cm(-2) inhibited biofilm growth in Po. endodontalis, Po. gingivalis, Pr. denticola, Pr. intermedia, Pr. melaninogenica and Pr. nigrescens. A fluence of 70 J cm(-2) did not affect biofilm growth in species of Bacillus, Candida, Enterobacter, Proteus, Pseudomonas, Staphylococcus and Streptococcus. The phototoxic effects of argon laser irradiation against Prevotella and Porphyromonas species were: (1) caused by the radiation alone; (2) modified by biofilm age; (3) dependent on the presence of atmospheric oxygen; (4) influenced by medium supplements of hemin, hemoglobin and blood; (5) greater when compared with other microbial species; (6) demonstrated without augmentation with an exogenous photosensitizer; and (7) apparently unrelated to the protoporphyrin content of the cells. Overall, these in vitro findings suggest that low doses of argon laser radiation may be effective in the treatment and/or prevention of clinical infections caused by biofilm-associated species of Prevotella or Porphyromonas.

Permissiveness of human embryonal fibroblasts for coxsackieviruses B3. Investigations on virus genetic markers in vitro and localization of virus receptor distribution by immunogold replica technique.

In our previous paper (29) we could show, that the replication of four Coxsackie B3 virus strains in human fibroblast lines from different origin was dependent on both the virus strain and the cell line used. Although generally no cytopathic effect could be observed out of one virus strain more pathogenic virus variants could be selected able to destroy virus-sensitive as well as insensitive fibroblasts. The present study was designed to characterize these virus strains by using several in vitro genetic markers. Differences were found concerning plaque size and the temperature marker, whereas the other markers (d, DD, DEAE-D) remained constant. Furthermore, these models of persistent as well as lytic CVB3 infection were analysed by immunoelectron microscopy to study the interaction of viral ligands with cellular receptors. The qualitative and quantitative differences in adsorption of the CVB3 strains to two human fibroblast lines as well as to HeLa cells corresponded well with the virological results. They underline that even in vitro in human cell lines of different origin changes in distribution, quantity and quality of receptors were demonstrable forming the base for the various virus sensitivity.

[Concomitant injuries after upper ankle joint dislocations].

Functional treatment with the Air Stirrup Ankle Brace recommended by C. N. Stover in 1979 can reduce pathological inversion of the ankle joint. In our retrospective study of 109 patients treated by this kind of ankle brace we found 96 patients (88%) with excellent results. Only 13 patients (12%) reported moderate to good results. To detect and characterize their painful conditions of ankles we did a clinical, radiological and MRI-Investigation. In only 2 cases we found a moderate instability after clinical investigation, anterior stress roentgenogram and talar tilt. By using the MRI-investigation 1.0 Tesla with a 512 x 360 Matrix we could find 10 cases with osteochondral lesions of the ankle. In 7 cases there was separated ossicle in the fibulotalar joint, in 1 case we detected a fracture of the processus anterior tali, in another case we could see a posttraumatic lesion of the talus and calcaneus with bone bruise and at least one osteochondral fracture of the distal tibia. The capability of the MRI to detect particularly osteo-chondral lesions of the talus and the tibiofibular joint was shown in 10 of 13 cases. Therefore we recommend to do an MRI-investigation on all patients after ankle sprain if there are painful conditions within the ankle after conservative treatment.

Preliminary results of uncemented metal on metal stemmed and resurfacing hip replacement arthroplasty.

Seventy uncemented stemmed total hip replacements and 35 uncemented surface replacements with all metal Metasul articulating surfaces were followed up in a prospective study. There was no evidence that this metal on metal articulation causes new problems or complications that were not known already from other polyethylene-aluminum oxide ceramic articulating combinations. The results of 64 of 70 patients could be assessed as excellent and good. When tissue samples obtained during 2 reoperations for ectopic ossification were examined histologically, there was no light microscopic evidence of metal particles. In these cases, aseptic loosening seemed to be due to the lack of initial fixation with the original femoral component design, and was not related to the use of the Metasul bearing. The metal on metal articulation reduces the production of particles considerably according to experience to date. It is therefore hoped that foreign body reactions due to wear particles will be significantly reduced. The results support the continued investigation of metal on metal joint replacements for younger, active patients.

Phylogenetic probes for analyzing abundance and spatial organization of nitrifying bacteria.

A hierarchical set of five 16S rRNA-targeted oligonucleotide DNA probes for phylogenetically defined groups of autotrophic ammonia- and nitrite-oxidizing bacteria was developed for environmental and determinative studies. Hybridization conditions were established for each probe by using temperature dissociation profiles of target and closely related nontarget organisms to document specificity. Environmental application was demonstrated by quantitative slot blot hybridization and whole-cell hybridization of nitrifying activated sludge and biofilm samples. Results obtained with both techniques suggested the occurrence of novel populations of ammonia oxidizers. In situ hybridization experiments revealed that Nitrobacter and Nitrosomonas species occurred in clusters and frequently were in contact with each other within sludge flocs.

In situ visualization of high genetic diversity in a natural microbial community.

Simultaneous in situ visualization of seven distinct bacterial genotypes, all affiliated with the phylogenetically narrow group of beta-1 Proteobacteria, was achieved in activated sludge. This finding indicates that the high diversity found in the same sample by direct rRNA sequence retrieval was indeed present in this complex community. By the combination of comparative rRNA sequence analysis, in situ hybridization with fluorescently labeled, rRNA-targeted oligonucleotides and confocal laser scanning microscopy several microbial populations can be analyzed for abundance, relative spatial distribution and phylogeny directly at their site of action without prior cultivation.

Effect of prostaglandin E2 on eicosanoid release by human bronchial biopsy specimens from normal and inflamed mucosa.

BACKGROUND: Eicosanoids such as prostaglandin E2 (PGE2), thromboxane A2 (TXA2), and peptidoleukotrienes (pLT) are known to be biologically highly active lipid mediators, especially in human lung epithelium. PGE2 is thought to have mostly bronchoprotective effects, whereas pLT and TXA2 are bronchoconstrictive. This study was undertaken to assess the release and interaction of eicosanoids in human bronchial biopsy specimens of normal and inflamed mucosa. METHODS: Bronchial biopsy specimens were obtained from 16 patients, seven controls without signs of inflammation and nine patients with severe inflammatory processes in the epithelium. The release of pLT, TXA2 (measured as TXB2), and PGE2 was investigated using a "functional in vitro test" and the addition of several stimuli. RESULTS: Specimens incubated with arachidonic acid released higher amounts of pLT, TXB2, and PGE2 than unstimulated specimens. Preincubation with PGE2 revealed significant inhibition of arachidonic acid-induced release of pLT and TXB2 (> 50%). The inhibitory effect was higher in normal than in inflamed epithelium. CONCLUSIONS: Exogenous PGE2 has inhibitory effects on the release of pLT and TXB2 in human bronchial biopsy specimens. This finding could explain the bronchoprotective effect of inhaled PGE2 in normal subjects and asthmatic subjects as direct eicosanoid interactions. It also supports the concept of PGE2 as a bronchoprotective endogenous substance. The complex effects of PGE2 as a modulating mediator in inflammation may be worth investigating.

[Stromal tumors of the digestive tract. Prognostic evaluation of a series of 36 cases].

Clinical outcome of gut's stromal tumors is difficult to predict by morphological data. The prognostic significance of a set of morphological criterias and immunostaining for proliferating cell nuclear antigen (PCNA) was studied. Thirty six tumours were classified benign (group A) or malignant (group B) according to ten year follow-up. Patient's sex and age, site of origin, tumour size, mitotic count and cytonuclear atypia were considered. Growth kinetic was studied by immunohistochemical analysis of PCNA (PC 10, DakoR). In univariate analysis, mitotic count, PCNA index, tumor size and cytonuclear atypia were associated with patient outcome. However, in multivariate analysis, only mitotic count and tumour size were independent prognosis factors. Thus, mitotic count remains the best prognostic indicator for these tumours since a cut off value of 2 mitosis/high power field allows an accurate prognosis in 33/36 tumors.

[Pancreatic carcinoma: surgical therapy].

The incidence of pancreatic diseases has increased over the last years. Low morbidity and mortality rates after pancreatic resection procedures and an increasing 5-years survival rate allow more liberal indications for operation. These better results are due to careful preoperative investigations, perfect surgical techniques and the use of Octreotide perioperatively to reduce morbidity and leakage rate. Therefore resections for cancer of the pancreas should be carried out whenever possible, as well in curative as in palliative attempt. We discuss the indications and contraindications of pancreatic resection. At last, the decision to perform a resection depends of the tumor extension found at the operation and of the surgeon's skill. Infiltration of the wall of portal or superior mesenteric vein and retroperitoneum are not always definitive criteria of non-resectability, as there is still the possibility of vessel resection or partial excision of the retroperitoneum followed by locoregional chemotherapy. Today encouraging results of pancreatic resections allow the surgeon to carry out pancreatic resections whenever possible in patients fit for the operation. These resections should be carried out by experienced surgeons who carry out more than 10 pancreas resections per year and achieve a mortality rate below 5%.

[Hip joint arthrodesis using the cobra plate. Indication, technique, outcome].

In the era of successful total hip replacement, hip arthrodesis has become an infrequent surgical procedure. Current indications are severe deterioration of the hip joint with a painful, reduced range of motion in young patients, such as disorders subsequent to sepsis of the hip joint, slipped capital epiphysis, Perthes' disease and trauma with contraindications for a total joint replacement. Another important indication is paralysis or musculature loss. From 1980 to 1990, 20 consecutive patients underwent a hip arthrodesis with a cobra plate and were evaluated. None of the patients was lost for follow-up; complications occurred in 5 out of 25 cases (20%). All patients had radiographic evidence of union, 2 patients required re-osteosynthesis and bone grafting. In two cases infection occurred. In one case the position of the fused join went into abduction. Thirteen patients were able to walk free of pain; however, 5 patients were disappointed with the arthrodesis result because of the handicap involved with a fused hip. Hip arthrodesis using the cobra plate is a technically demanding operation, but immediate mobilization and partial weight bearing are facilitated. There is little risk of non-union compared to other surgical procedures. If the technique is correct functional problems will be rare.
