[East German percentile values for newborn infants--1985 relation of age, parity, body weight and height of the mother and the birth weight of newborn infants].

Based on an analysis of very comprehensive data material multidimensional investigations on connections between age, parity, weight and height of mothers and birth weight of infants have been made. Maternal height as well as maternal weight have a considerable influence on birth weight of infants. At the same maternal height infant birth weight will rise in connection with the rise of maternal weight. Maternal age, parity and weight are correlated variables. The older a woman, the higher on average is not only her parity but at the same her weight, too. The rise of maternal weight per year in the statistic mean amounts to 401 g. Moreover, at the same age like primiparae second and subsequent parae are of higher weights so that partly maternal weight contains indirectly not only maternal age but also maternal parity. The following range of the influencing factors maternal height, weight, parity and age on birth weight can be given: 1. Height and weight as relatively equivalent factors, 2. parity and 3. age.

[Use of the new East German percentiles in the classification of newborn infants].

Re-classifying the newborns treated in the NICU of the University of Greifswald between 1983 and 1987, according to the percentiles (birthweight and length) of Lubchenco and the new standards recommended for the GDR after a data sampling in 1985, identic results could be found for the number of LGA using the 90th percentile. Certainly until the 34th week of gestation (but probably also thereafter), using the 10th percentile of the GDR standard, a to high rate (greater than 20%) of SGA infants could be found, whereas the range of birthweights of our own patients fitted exactly the distribution found by Lubchenco. The reason might be an underrepresentation of lifeborn VLBW infants in the GDR sample due to differences of the definitions used for the assessment of lifeborns and stillborns/fetal loss. Therefore, for practical use of the new standards as criterion for classifying AGA/SGA newborns until the 34th week of gestation the 5th percentile is recommended. A reevaluation of the distribution of birthweight in the GDR population is proposed including even-dead newborns in the data sampling.

[New percentile values for birth weight, body length, head and breast circumference of newborn infants in East Germany].

Somatometric data of 51,570 newborns (singles) delivered between the 28th and 42th week of gestation from different, representative areas of the G.D.R., concerning birth weight, body length, head and breast circumference are presented as tables and percentile curves separately for either sex. The values are recommended as reference values for the assessment of fetal development related to gestational age in the G.D.R.

[Neonatal percentile values for East Germany--1985. 3. On the correlation of body height and weight of the mother in a two-dimensional classification of newborn infants according to birth weight and gestational age].

There were published percentiles of birthweight in relation to bodyheight of the mother as well as in relation to bodyweight and parity, bodyheight and bodyweight of the mother for term range 37th to 41st week of pregnancy. They were calculated through median quantities. The dates again derive from the investigation of 51,570 single births in elected regions of GDR in 1984 and 1985. The length of pregnancy was stated in full weeks. It was determined on the base of three parameters (by Naegele method, by sonography, and by neonatal maturity judgment). Selected percentiles of birthweight regarding the above influencing factors, distributed into sex, were summarized for use in practice.

[Prenatal detection of open neural tube defects in a mass screening program by determination of alpha-fetoprotein concentration in maternal serum].

A partially automatized ultramicroencymimmunoassay developed by the GDR and Cuba should be tested in a mass screening for the prenatal detection of open neural tube defects by measurement of maternal alphafetoprotein concentration (MS-AFP). In the screening program 24,271 pregnant women were included (Berlin/District of Schwerin). In 18,144 patients AFP-concentration in maternal serum was measured as a routine procedure in the care for all pregnant women of the district of Schwerin. The time of MS-AFP determination in this connection was limited from 15th to 18th completed gestational week. 1986 already in 84.9 per cent of all pregnant women of the district of Schwerin MS-AFP concentration could be measured in early pregnancy. The detective rate of open neural tube defects among 24,271 tested pregnant women was 86.1 per cent. AFP concentration was expressed in ng/ml and the "cut off" was set at 2.0 MOM. Infants born with open neural tube defects were reduced by 75 per cent under the conditions of MS-AFP screening. Better results are possible in connection with a progress in the qualification of the following diagnostic steps, if MS-AFP-levels are elevated. Not unexpected and unspecific MS-AFP-elevation was found in connection with twin pregnancies and a higher risk of abortion and the delivery of infants with very low birth-weight less than 1,500 g. A general MS-AFP-screening is recommended for prenatal detection of open neural tube defects all over the country.

[Percentiles of newborns in the DDR--1985. 1. Percentiles of birth weight and birth height].

Body measures of newborn infants and their parents were comprehended and statistically revised in 30 hospitals of German Democratic Republic in 1984-1985. There were records of 52,570 single livebirths without birth defects of women living in the GDR (51.3 per cent boys and 48.7 per cent girls). Percentiles were calculated and demonstrated in tables and figures. Base of calculation of percentiles was a pregnancy week final diagnosis. It was determined by the physician resulting from three investigations of pregnancy week (methods Naegele; ultrasound; maturity index). Percentiles of birthweight and birthlength of newborn infants in GDR in 1984-1985 are lower than those in 1973-1974. Causes are discussed. Our results are compared with two Middle European curves and with Lubchenco curve (1963).

[Correlation between some environmental, anamnestic and social markers of pregnant patients and the delivery of eutrophic premature and hypotrophic newborn infants].

12,577 single newborns of 19 hospitals of the northern districts of GDR were classified immediately after birth following the recommendations of WHO from 1971 and the Society of Perinatal Medicine of GDR for classification of newborns. In a prospective study the influence of some different parameters on the relative frequency of deliveries of preterm appropriate-for-gestational-age infants and term small-for-gestational-age infants was proved. To secure the statistical statements u-test was used. A sometimes different efficiency of the proved parameters on the tested groups demonstrated that preterm appropriate-for-gestational-age infants, term small-for-gestational-age infants and other groups of newborns of low birth-weight should'nt be collected as infants of low birth-weight up to 2,499 g in present time. With the provided introduction of new percentiles in GDR an exact classification of newborn infants should be realized all over the country.

Cholecystokinin octapeptides alter the release of endogenous dopamine from the rat nucleus accumbens in vitro.

Sulfated cholecystokinin octapeptide (CCK-8S) has been reported to be extensively colocalized with dopamine in the posterior, but not the anterior, portion of the nucleus accumbens (NAc). The hypothesis tested in the present study was that CCK-8S alters the release of dopamine in this structure and the actions it produces are dependent on the extent of colocalization with dopamine. We observed in vitro that CCK-8S enhanced the resting release of dopamine from the posterior, but not anterior, NAc. It was also found that CCK-8S attenuated the release of dopamine induced by potassium-evoked depolarization in both regions of the NAc, although the concentration-release curves for the two areas differed. In the posterior NAc, a biphasic response was seen whereas, in the anterior NAc, there was a monophasic attenuation. Proglumide, a putative CCK-8S antagonist, was found to antagonize the action of a low concentration of CCK-8S on 40 mM K+-induced dopamine release from slices of the posterior, but not anterior, NAc. Unsulfated CCK-8 had mixed action on K+-evoked dopamine release, as it enhanced this form of release in the posterior NAc but attenuated it in the anterior NAc. Additionally, we found no effect of sulpiride on the actions of CCK-8S with respect to evoked release, suggesting that CCK-8S is not acting to alter dopamine autoreceptor function, as has recently been hypothesized. In summary, our results demonstrate that the observed effects of CCK-8S on dopamine release are dependent upon the region of the NAc studied, and there appear to be different subtypes of CCK-8S receptors present in the two regions.

[Analysis of very underweight newborn infants (less than 1,500 g) 1972-1982. Epidemiologic aspects, labor presentation, position and adjustment, method of delivery and mortality].

Infants with a very low birthweight (less than 1500 g) have a fundamental influence on perinatal mortality and mortality of babies up to one year also nowadays. The epidemiological role of different signs of risk in the context of the birth of these children is presented. The total mortality of the infants with a very low birthweight (less than 1500 g) was 64,4%. Usually there were combinations of factors, which caused the death of the infants. Complications of secundinae, immaturity and malformations were the substantial factors of lethality of the stillbirths. At the mortality of babies there were intracranial bleedings and immaturity (less than 1000 g, less than 28 completed weeks of pregnancy) in the foreground of the causes of death.

[Congenital heart defects as a cause of still birth and infant mortality].

Among 4,686 still-births and children died from 0 to 1 a in the German Democratic Republic in 1979 there were 307 (6.6%) isolated congenital heart diseases. We only analyzed 294 died infants. The first 5 diagnoses were: Hypoplastic left heart (all infants died within the first 28 d) 9.9%, Ventricular septal defect 9.9%, Transposition of great vessels 8.8%, Coarctation aorta 7.1%, Common truncus arteriosus 5.8%. The most important results are an interesting frequency with advancing years of mothers (3.4% in 10,000 mothers of live-births aged 16 a and less to 20.6% at the age of 40 a and over (see Fig. 1), 13% premature children (under 37 weeks of gestation), 23% children under 2,500 g birth weight, 55% boys and 45% girls. 80% of the above 294 infants died within the first 3 months.

[The effect of various parameters on the incidence of premature births].

In a retrospective study among about 17,000 newborns the preterm infants were registered. Newborns delivered in the District Hospital of Schwerin between 1969 and 1977 are included in the above figure. The influence of different parameters on the deliveries of preterm infants was proved. In order to exclude the influence of age and parity on the results of the examinations s standardization of age and parity was done. The frequency of the delivery of preterm infants was dependent on age, parity, occupation and case histories of abortion, interruption of pregnancy and delivery of under-weight.
