Slipover urethral instrument system.

An inovative system has been devised using an elongated hollow guide and slipover urethral instruments. Inherent in this concept are many safety features. A slipover follower and sound will replace current instruments with the threaded locking mechanism. The versatility of the system is demonstrated by the passage of the Councill catheter, cystoscope and direct vision urethrotome over the guide. The orderly interaction of all instruments facilitates their use.

[Psychogenic visual disturbances in children and adults. Ophthalmological, neurological and psychodynamic findings].

In large series of ophthalmic patients there may occasionally be some in whom the reduction of visual acuity cannot be explained by organic findings. In such patients simulation is suspected. However, simulation is not the only cause of decreased visual acuity and a lack of organic pathologic findings. It is obvious that all efforts must be made to exclude organic changes as the cause of decreased visual acuity. With modern diagnostic modalities this is possible to a large extent. If there are no organic pathologic findings, a psychiatric exploration is indicated. In the eight patients presented in this paper the reduction of visual acuity was the expression of a psychologic conflict situation. The cases are described in detail and the difference between simulation and a psychogenic decrease of visual acuity is pointed out.

[Histologic and morphometric findings in placentas of heroin addicts (author's transl)].

Twenty placentas of mothers addicted to heroin were examined macroscopically, microscopically and morphometrically. Twenty placentas of non-addicted mothers and of the same gestational age served as a control group. The following, statistically documented, peculiarities were found in the placentas of mothers addicted to heroin: 1) increased intervillous and perivillous microfibrin deposits; 2) an increased number of trophoblastic proliferation buds and nuclear nodules; 3) increased vascularization of the villi, and 4) increased signs of asphyxia. Our findings support the view, mentioned in the literature, that fetal hypotrophy is probably the result of a direct action of the heroin on the fetal tissue. We regard the placental changes as secondary effects of circulation complications in heroin addicts.

Purification and characterisation of the cyclic AMP-dependent protein kinase, the C- and the R-protein from bovine liver.

A cyclic AMP-dependent protein kinase, its regulatory (R) and catalytic (C) protein were isolated from bovine liver, The cyclic AMP-dependent protein kinase showed two protein bands on SDS-polyacrylamide gel electrophoresis with molecular weights of 54 000 and 40 000. They correspond to the data for the separately isolated R-and C-protein. The molecular weight of the holoenzyme ranged from 172 000-179 000, depending on the estimation method. The molecular weight of the R-protein ranged from 97 000-98 000. This, and the results of the SDS-polyacrylamide gel electrophoresis, demonstrates a dimeric structure. The frictional ratios (f/fo) of 1.67-1.7 for the holoenzyme and 1.9 for the R-protein correspond to highly asymmetric shapes. Assuming a prolate form, the axial ratios are 13-14 and 17, respectively. The C-protein is globular (f/fo 1.1-1.26, axial ratio 3-5). The secondary structure with 35% alpha-helix, 19% beta-sheet and 49% aperiodic form of the holoenzyme is similar to the R-protein with 35, 19 and 46%, respectively. The C-protein contains 29% alpha-helix, 21% beta-sheet and 50% aperiodic form. The dimeric R-protein binds 4 mol cyclic AMP and can be phosphorylated in the presence of the C-protein. An absorption coefficient, A280nm 1.0%, of 5.4 was calculated for the R-protein and of 13.6 for C-protein. The data for C-protein, e.g., molecular weight, heterogeneity in isoelectrofocusing, phosphate content, etc., are in good agreement with those found by Sudgen, P.H. and Corbin, J.D. (1976) biochem. J. 159, 423-437.

[Oculo-cerebral dysplasia: aplasia of the optic nerve with familial microphthalmos and cryptophthalmus. Clinical and computer tomography study].

The authors report on two siblings with severe malformations of the eyes. The 16-year-old girl has bilateral microphthalmus with aplasia of the right optic nerve and a Dandy-Walker cyst of the cerebellum. The right eye has no optic disk or retinal vessels. Her 13-year-old brother has unilateral aplasia of the optic nerve with cryptophthalmus and contralateral microphthalmus. In a computer tomography examination no optic nerve could be demonstrated on the left. The presence of an oculocerebral dysplasia in a brother and sister with healthy parents suggest an autosomal-recessive mode of inheritance of the aplasia of the optic nerve with microphthalmus and/or cryptophthalmus.

[Reye's syndrome (author's transl)].

After a viral infection (Coxsackie-A-9) a twelve-year-old boy developed symptoms of acute encephalopathy and fatty degeneration of the liver. A fatal outcome could not be averted. Clinical course, laboratory data, light and electron microscopic examinations were typical for Reye's syndrome, a rare diagnosis in Central Europe.

Cellular decay in the rat retina during normal post-natal development: a preliminary quantitative analysis of the basic endogenous rhythm.

The diurnal variation in the appearance of cell death in the rat retina during postnatal development was investigated. The frequency and the morphology of the necrotic cells and the mitotic figures were studied by light and electron microscopy at various stages of ontogenesis and at specified times during each day. Preliminary results show that there is a varying exponential behaviour of mean necrosis-densities at certain periods each day. This chronobiological pattern and the general problems of physiological cellular decay in the developing retina are discussed.

Effect of extracorporeal blood irradiation on lymphocytes and the lymphatic tissue in a goat.

In a long-term study discontinuous extracorporeal blood irradiation (ECIB) was applied to a goat using a 500 Ci-137 Caesium source. Lymphocytes of the peripheral blood were examined by light and electron microscopy. After application of a transit dose of 466,500 rad the lymphocytes in the peripheral blood were found to be decreased from 6,900/microliter to 500/microliter, revealing a complete dissolution of the nuclei in electron microscopic preparations. Histological examinations showed a severe atrophy of the whole lymphatic tissue.
