TNF-related apoptosis-inducing ligand (TRAIL) and erythropoiesis: a role for PKC epsilon.

The regulation of the hematopoietic stem cell pool size and the processes of cell differentiation along the hematopoietic lineages involve apoptosis. Among the different factors with a recognized activity on blood progenitor cells, TRAIL - a member of the TNF family of cytokines - has an emerging role in the modulation of normal hematopoiesis.PKC(epsilon) levels are regulated by EPO in differentiating erythroid progenitors and control the protection against the apoptogenic effect of TRAIL. EPO-induced erythroid CD34 cells are insensitive to the apoptogenic effect of TRAIL between day 0 and day 3, due to the lack of specific surface receptors expression. Death receptors appear after day 3 of differentiation and consequently erythroid cells become sensitive to TRAIL up to day 9/10, when the EPO-driven up-regulation of PKC epsilon intracellular levels inhibits the TRAIL-mediated apoptosis, via Bcl-2. In the time interval between day 3 and 9, therefore, the number of erythroid progenitors can be limited by the presence of soluble or membrane-bound TRAIL present in the bone marrow microenvironment.

Experience in "radical" surgery of supratentorial gliomas in adults.

This study reports the preliminary results obtained in a series of 107 patients with histologically proven malignant gliomas (86 glioblastomas and 21 anaplastic astrocytomas) operated upon between 1986 and 1989, with an aggressive attitude aimed to achieve extensive and possible "radical" excision of the tumor. Gross total removal was achieved in 62% of cases, while in the remaining the postoperative contrast enhanced CT scan showed more than 10-15% of residual tumor mass. There was no operative and postoperative mortality. The one-year survival rate was 60% in patients with total removal and 24% in those with partial resection. Furthermore the Karnofsky rating at discharge was improved in the former group while was unchanged in the latter. Although preliminary, these data seem to confirm the primary positive role of radical surgery in the combined management of malignant gliomas.

[Aztreonam: monocenter microbiological study with prevention experience in surgery].

Aztreonam is a monobactam antibiotic active against aerobic gram-negative bacteria. The susceptibility of 127 urinary tract isolates to aztreonam, cefotaxime, cefonicid and ceftazidime was determined. Aztreonam showed good antibacterial activity even against Pseudomonas spp. Only 7 bacterial strains were resistant to aztreonam. The clinical efficacy and pharmacokinetics of aztreonam were assessed in two patients treated for urinary tract infections. The concentrations of aztreonam in serum and urine are reported. Aztreonam safety was evaluated on 20 patients given aztreonam immediately prior to an elective abdominal, urinary or gynecological operation. The results provide support for the use of aztreonam for prophylaxis.
