Prevalence of erectile dysfunction and associated factors among men without concomitant diseases: a population study.

We interviewed a population sample of 2412 men aged 40-70 y in Brazil, Italy, Japan and Malaysia about medical history, lifestyle habits and sexual behavior. Men were classified as having moderate or complete erectile dysfunction (ED) if they reported to be sometimes or never able to achieve and maintain an erection satisfactory for sexual intercourse, respectively. There were 1335 men with no diagnosis of cardiovascular or prostate diseases, diabetes, ulcer or depression, nor taking hormones. The prevalence of ED was 16.1%. ED was associated with age (the risk increased 8% per y), moderate (odds ratio (OR)=2.2) or severe (OR=4.9) lower urinary tract symptoms and smoking (OR=2.3 for >30 cigarettes/day). It was inversely associated with physical activity (OR=0.5) and higher educational levels. Between the ages of 40 and 70 y, almost one in six 'healthy' men is affected by ED. Further research should look at preclinical disease stages and genetic factors.

Comparison of two direct-reading instruments (FM-7400 and Fibrecheck FC-2) with phase contrast optical microscopy to measure the airborne fibre number concentration.

The use of direct-reading instruments to measure the airborne fibre number concentration is on the increase. The response of two of these instruments (FM-7400 and Fibrecheck FC-2) was compared with the conventional method of sampling on filters and counting by phase contrast microscopy. Four types of fibres were studied at different concentrations and relative humidity levels. The FM-7400 can be calibrated by the manufacturer for two different levels of sensitivity (standard and high). For the tests where it was set to the sensitivity level with which it had been calibrated, the ratio of the concentration measured by the instrument to the concentration obtained by the conventional method varied in the range 0.5-1 for the different types of fibres studied (chrysotile, glass wool and ceramic fibres). The Fibrecheck FC-2 is a much less versatile instrument. On the basis of a calibration allowing correct detection of asbestos fibres, it greatly overestimated the concentration of man-made mineral fibres. In its normal calibration state a fine chrysotile aerosol was poorly detected. For man-made mineral fibres, the response was highly dependent on the nature of the fibres. These instruments require calibration with the type of fibres to be studied. Unfortunately, this operation is not always accessible to the user and may require the services of a specialized laboratory, as the manufacturer is not always in a position to carry this out.

Barkas effect for antiproton stopping in H2.

We report the stopping power of molecular hydrogen for antiprotons of kinetic energy above the maximum (approximately 100 keV) with the purpose of comparing with the proton one. Our result is consistent with a positive difference in antiproton-proton stopping powers above approximately 250 keV and with a maximum difference between the stopping powers of 21%+/-3% at around 600 keV.

Cobe Spectra is superior to Fenwal CS 3000 Plus for collection of hematopoietic stem cells.

One hundred and seventy-seven stem cell apheresis procedures performed on 91 patients using the Fenwal CS 3000 Plus cell separator and 61 procedures performed on 37 patients using the Cobe Spectra cell separator were studied to compare the CD34(+) cell collection efficiencies (CE; the proportion of the total CD34(+) cell content in the blood volumes processed that is harvested) of the two machines. The absolute peripheral blood CD34(+) cell count was comparable for the two groups (P = 0.27). A strong correlation was seen between the blood CD34(+) cell count and the total number of CD34(+) cells collected for the Spectra (r(2) = 0.59; P < 10(-6)) and for the CS 3000 Plus (r(2) = 0.60; P < 10(-6)). No significant correlation emerged between the peripheral blood CD34(+) cell count and the CE of either machine. The total number of CD34(+) cells collected per procedure was comparable (P = 0.51): median 113 x 10(6) for CS 3000 Plus and median 218 x 10(6)for Spectra. CE was significantly higher with the Spectra (median 45.7%, range 9.8-98.6%) than the CS 3000 Plus (median 30.3%, range 1.7-89.3%; P < 0.00001). We conclude that the CD34(+) cell CE of the Spectra is superior to that of the CS 3000 Plus. Therefore, under the usual clinical conditions, Cobe Spectra should be used preferentially for peripheral blood progentor cell collection to maximize the number of hematopoietic stem cells collected.

Prevalence and correlates of erectile dysfunction in Salvador, northeastern Brazil: a population-based study.

Our objectives were to determine the prevalence of erectile dysfunction (ED) in Brazil and to explore potential sociodemographic, medical, and lifestyle correlates. A cross-sectional, population-based, household survey was conducted in Salvador, Bahia, Brazil. Cluster samples of representative households were randomly selected for interviews. Of 654 eligible subjects, 602 (92%) participated. A structured questionnaire was administered by trained interviewers. ED was categorized as 'none', 'mild', 'moderate', or 'severe' according to the ability to 'attain and/or maintain an erection satisfactory for sexual intercourse'. All data were obtained by self-report. The age-adjusted prevalence of ED was 39.5% (minimal 25.1%, moderate 13.1%, severe 1.3%). Prevalence and severity increased with age. Having never been married, diabetes, depression, or prostate disease and current depressive or lower urinary tract symptoms were significantly (P<0.05) associated with increased prevalence. Medical, sociodemographic, and lifestyle variables associated with ED may alert physicians to patients at risk for ED and offer insight to its etiology.

Skin graft for 2-stage treatment of severe hypospadias: back to the future?

PURPOSE: Despite the introduction of more refined surgical techniques, the optimal treatment of the most severe forms of hypospadias remains to be determined. Single stage procedures, whether with the use of flaps or grafts, have long been regarded as the best approach, although the complication rate is nonnegligible with all procedures. MATERIALS AND METHODS: We report the use of a 2-stage repair with preputial graft interposition and subsequent tubularization of the urethral plate applied it to all severe cases of hypospadias with significant chordee or small glans. RESULTS: Both stages of the procedure were completed in 34 patients. Complications in 8 cases (23.5%) included 4 glans disruption in 4, coronal grove fistula in 2, urethral diverticulum in 1 and urethral stenosis due to balanitis xerotica obliterans in 1. Two pinhole fistulas also occurred which closed spontaneously. No complete disruptions or postoperative hematomas/bleeding was noted. Cosmetic and functional outcome after a minimum followup of 1 month was optimal in all cases with a normally located "slit" meatus and straight penile shaft. CONCLUSIONS: Although the controversy between use of grafts and flaps will probably continue forever, we believe that our 2-stage approach should be considered as a valid alternative for the most severe forms of hypospadias. Long-term results appear to outnumber the necessity of a learning curve for appropriate graft manipulation.

Recent trends in the HIV epidemic among injecting drug users in Northern Italy, 1993-1999.

OBJECTIVE: To monitor and describe the time trends of the HIV epidemic among intravenous drug users (IDU) attending drug dependence treatment centres (DDTC) in Northern Italy. DESIGN: A cohort of all seronegative IDU attending DDTC in Lombardy between 1993 and 1999; all had been tested for HIV at least twice. Periodic sample interview surveys were done to assess risk behaviours. METHODS: The incidence rates of HIV infection were calculated using the person-year (PY) method and expressed as the number of cases per 1000 PY at risk. Background HIV prevalence was calculated by dividing the number of positive cases by the total number of IDU tested at all DDTC in Lombardy. RESULTS: Between 1993 and 1999, 135 seroconversions occurred in 7945 subjects followed for 19 671 PY, yielding an incidence rate of 6.9/1000 PY. Ninety seroconversions occurred among the 6563 males and 45 seroconversions among 1382 females (incidence rates 5.5 and 14.0, respectively). Among the males, the incidence of HIV was 4.5 in those aged less than 25 years and 5.8 in those aged 25 years or more. Among the females, the corresponding figures were 21.1 and 10.3. HIV prevalence decreased over time, and it was higher among females. Sexual behaviours at risk were more common among females. CONCLUSIONS: The incidence of HIV infection among IDU in Northern Italy was stable between 1993 and 1999. The higher incidence and prevalence among females and the different prevalence of risk behaviours between genders suggest an increasing role of heterosexual transmission.

Adolescent varicocele: Tauber antegrade sclerotherapy versus Palomo repair.

PURPOSE: There is general agreement on treatment for varicocele in pediatric patients. Randomized prospective studies have shown that anatomical and functional lesions may be corrected. Due to the impossibility of seminal examination patients with moderate to large varicocele or ipsilateral testicular hypertrophy, characterized by a change in testicular consistency or symptoms, should undergo surgical correction. The best therapeutic approach is still under discussion. MATERIALS AND METHODS: At 2 centers 2 therapeutic approaches to varicocele treatment in pediatric patients were compared, namely the Palomo repair and antegrade sclerotherapy according to Tauber. The 89 patients from the same geographical area elected 1 procedure after an explanation. From the medical records we retrospectively evaluated operative time, postoperative analgesics, postoperative fever onset, complications, convalescence, recurrence and postoperative hydrocele. RESULTS: After Palomo repair in 45 patients there were 2 recurrences (4.4%) and 2 postoperative hydroceles (4.4%). Of 44 antegrade sclerotherapy cases 1 was converted to Palomo repair, there was no hydrocele formation and recurrence developed in 2 (4.5%). Testicular atrophy was not observed in any patient regardless of the method used. The cost of the procedure was lower in the sclerotherapy group. CONCLUSIONS: These data suggest that the failure rate was similar in both groups. The principal advantages of sclerotherapy are simplicity, decreased cost and lack of hydrocele formation.

CD34(+) cell collection efficiency does not correlate with the pre-leukapheresis hematocrit.

One hundred and seventy-seven large-volume leukapheresis procedures performed on 91 patients over a 15 month period were reviewed to see if the pre-apheresis hematocrit (Hct) affected the CD34(+) cell collection efficiency (CE) of the Fenwal CS 3000 Plus cell separator. The Hct was 0.174-0.461 (median 0.317), and the peripheral blood CD34(+) cell count 2-2487 per microl (median 21). The total CD34(+) cell quantity collected was 3.0-2677.2 x 10(6) (median 113.0). Based on the number of CD34(+)cells contained in the blood volume processed (23.3-37303.2 x 10(6); median 318.0), the CE was 1.7-87.5% (median 30.3). No correlation was found between the Hct and CE (r(2) = 0.0034; P = 0.44) or the total CD34(+) cell quantity collected (r2 = 0.0040; P = 0.40). CEs for Hct <0.25 (median CE 36%), Hct 0.25-0.299 (median CE 30%) and Hct 0.30 (median CE 30%) were comparable. As expected, highly significant correlations were seen between the CD34(+) cell quantities collected and quantities processed (r2 = 0.59; P < 10(-6)) as well as the peripheral blood CD34(+) cell counts (r2= 0.60; P < 10(-6)). We conclude that the minimum acceptable Hct or hemoglobin level for leukapheresis should be dictated by clinical circumstances because it does not affect stem cell collection.

Esophagogastric dissociation versus fundoplication: Which is best for severely neurologically impaired children?

PURPOSE: Neurologically impaired children (NIC) often have swallowing difficulties, severe gastroesophageal reflux, recurrent respiratory infections, and malnutrition. Bianchi proposed esophagogastric dissociation (EGD) as an alternative to fundoplication and gastrostomy. The authors compared these 2 approaches. METHODS: Twenty-nine consecutive symptomatic NIC refractory to medical therapy were enrolled in a prospective study and divided into 2 groups: A (n = 12), NIC who underwent fundoplication and gastrostomy; B (n = 14), NIC who underwent EGD. Three were excluded because of previous fundoplication. Anthropometric (percentage of the 50th percentile/age of healthy children) and biochemical parameters, respiratory infections per year, hospitalization (days per year), feeding time (minutes), and "quality of life" (parental psychological questionnaire, range 0 to 60), were analyzed (t test and Mann-Whitney test) preoperatively and 1 year postoperatively. Complications were recorded. RESULTS: Compared with group A, group B presented a statistically significant increase of all anthropometric and nearly all biochemical parameters with a statistical difference in terms of respiratory infections, hospital stay, feeding time, and psychological questionnaire. In group A, 2 bowel obstructions, 1 tight fundoplication, 1 dumping syndrome, and 3 failures of fundoplication occurred. Group B presented 1 anastomotic stricture, 1 paraesophageal hernia, and 1 bowel obstruction. CONCLUSIONS: Compared with fundoplication and gastrostomy, EGD offered better nutritional rehabilitation, reduction in respiratory infections, and improved quality of life. EGD can be rightfully chosen as a primary procedure.

Physico-chemical characterization of a novel tricyclic beta-lactam antibiotic.

GV118819X, a novel tricyclic beta-lactam antibiotic of GlaxoWellcome, is a racemic mixture of two diastereoisomers, A and B. Of the two diastereoisomers, only A is available as a pure compound. By analyzing mixtures of GV118819X and A, a partial phase diagram is constructed, which indicates the presence of a eutectic when the A fraction is approximately 39%. Moreover, the melting enthalpies of the eutectic mixture and of diastereoisomer B can be estimated. With the exception of the pure A form, all mixtures undergo important modifications in morphology and microstructure as a consequence of thermal treatments, which induce melting/amorphization of the eutectic, and crystallization of the A form. Analyses of the sieved fractions of GV118819X demonstrate that it consists of acicular crystals of different composition, with the larger crystals having a larger A fraction than the smaller ones. Grinding causes melting/amorphization of the eutectic and, following hours-long treatments, the formation of a substantial fraction of submicron particles with unusually low melting temperatures.

A Theoretical Study of the Methyl and Aldehyde Torsion FIR Spectra in Symmetric Propanal Isotopomers.

This paper is an extension of the techniques developed by us [A. Vivier-Bunge, V. H. Uc, and Y. G. Smeyers, J. Chem. Phys. 109, 2279 (1998)] for standard propanal. In that paper the potential energy surface for the simultaneous methyl and asymmetric aldehydic torsions was calculated at RHF/MP2 level using the 6-311(3df,p) basis set for propanal. The fit of the energy values to symmetry-adapted functional forms was carried out by using the 28 energy values which retain the C(3) dynamical symmetry of the methyl group in the optimization procedure. With this potential, as well as with the kinetic parameters and the electric dipole moment variations, the FIR frequencies and intensities for the methyl and aldehyde torsions of seven symmetric isotopomers of propanal were determined theoretically using two-dimensional calculations. The calculated spectra of propanal and three of its isotopomers were compared with the available experimental data. It is found that the calculations for the cis conformer satisfactorily reproduce the aldehyde and methyl torsion spectra and furnish also methyl torsionally excited progressions for the aldehyde torsion modes. The methyl torsion frequencies agree especially well whenever the methyl group is nondeuterated. The small deviations encountered for the deuterated compound are probably due to some mass effect, such as the zero-point vibrational energy correction, which is not taken into account in the present calculations. Finally, the influence of the deuteration on the intensities is discussed. Copyright 2000 Academic Press.

Is the 13C-acetate breath test a valid procedure to analyse gastric emptying in children?

BACKGROUND/PURPOSE: Scintigraphy is regarded as the "gold standard" procedure in measuring gastric emptying (GE) rates. 13C-acetate breath test (ABT), which already has been validated in adults, is a noninvasive and nonradioactive alternative method. The aim of the current study was to validate ABT against technetium Tc 99m scintigraphy in children affected by delayed GE. METHODS: Sixty children were recruited and divided into 2 groups: group A, 30 healthy controls; group B, 30 patients with gastroesophageal reflux, and scintigraphy-documented DGE (15 neurologically impaired). After an overnight fast, all of them underwent ABT using 25 to 150 mg 13C-acetate. Breath samples were obtained at baseline and then every 10 minutes for 2 hours. The 13CO2 to 12CO2 ratio in breath samples was analysed by isotope ratio mass spectrometry. Data are expressed as follows: time of peak 13C exhalation (tP13CO2b) and half emptying time in ABT (t(1/2b)), and scintigraphy half emptying time (t(1/2s)). RESULTS: In controls tP13CO2b was 37 +/- 13 minutes and t(1/2b) 74 +/- 12 minutes. In patients tP13CO2b and t(1/2b) were, respectively, 65 +/- 26 minutes and 104 +/- 18 minutes t(1/2s) was 91 +/- 21 minutes. In group B tP13CO2b and t(1/2b) were delayed significantly compared with controls, respectively, P < .03 and P < .01. In group B significant correlation between t(1/2s) and t(1/2b) was noted (r1 = 0.97). A close correlation was also observed between t(1/2s) and tP13CO2b (r2 = 0.95). CONCLUSION: The 13C ABT is an easy, reliable, and less expensive procedure for measuring GE, and its results closely correlate with those of scintigraphy in a paediatric population.

Rise and fall of asthma-related mortality in Italy and sales of beta2-agonists, 1980-1994.

We performed this study with the aims of describing the trend of asthma-related mortality in Italy between 1980 and 1994, and to evaluate the relationship between sale estimates of beta2-agonists drugs and mortality from asthma. For asthma mortality we used data provided by National Institute of Statistics, for sale estimates of beta2-agonists we used data provided by IMS HEALTH. We calculated the gender specific age-standardized incidence rates of asthma-related deaths for all ages and for age classes. We found that estimates for asthma-related mortality steadily increased between 1980 and 1987 in both sexes, and thereafter decreased. In people, aged between 34 and 64 and over 64, death rates in males were significantly higher than in females while the rates in those aged less than 34, were mostly similar in both gender. The overall exposure to beta2-agonists (alone and in combination) increased from 1980 to 1990, remained stable between 1990 and 1993, and increased steeply in 1994. We conclude that asthma-related death rates have declined since the mid-1980's. This decline has been more pronounced in males and in the older ages, while the rates in younger patients of both genders have remained nearly unchanged. Our data do not substantiate the hypothesis of an increased risk of asthma-related mortality associated to the use of inhaled beta2-agonists in general nor fenoterol or salbutamol in particular.

Hydration of beta-cyclodextrin: a molecular dynamics simulation study.

We study by molecular dynamics simulations the hydration of beta-cyclodextrin. Our simulations show that within these barrel-shaped molecules hydrophobicity dominates, while at the top and bottom sides of the barrel interactions with water are mostly hydrophilic in nature. These results agree with crystallographic data at 120 K and, in particular, with the spontaneous hydration process of a cyclodextrin crystal in wet atmosphere. The predicted structure of the hydration shells is discussed and compared with previous molecular mechanics calculations which report an overall hydrophobic behavior. Moreover, the temperature dependence of the hydration process is discussed.

A new and successful chance in surgical treatment of gastrooesophageal reflux in severely neurologically impaired children: Bianchi's procedure.

AIM: Neurologically impaired (NI) children present with neuromuscular incoordination and severe gastroesophageal reflux (GER). In these cases, Bianchi has proposed total oesophagogastric dissociation (OGD) as a primary procedure and not only after failed fundoplication. METHODS: Anthropometric and laboratory data, incidence of respiratory symptoms, days of hospitalisation, time of feeding, stool frequency and quality of life were tested preoperatively and after OGD over a 12-month period in 13 severe NI patients. RESULTS: Results are shown in Table 1. We also recorded improvement in quality of life, based on communication by the person responsible for the care of the subject, and determined by psychological interviews. CONCLUSIONS: OGD is a successful and feasible procedure and can be included as a primary definitive surgical treatment in individualised nutritional rehabilitation planning for each severe NI child.
