Parathyroidectomy and postoperative hypocalcemia: a retrospective study.

The data of 24 patients, operated for different types of hyperparathyroidism were reviewed in order to evaluate the evolution of calcium levels during the first week following surgery. A correlation with the preoperative values of alkaline phosphatase could be demonstrated. It may be concluded that in the primary affected patients a stay in the ICU, only for hypocalcemia-related problems, is not required. In secondary hyperparathyroidism however, the more spectacular drops of calcium levels and the risk of other vital complications, justify a follow-up in the ICU for at least 48 hours.

Alfentanil kinetics in renal insufficiency.

Alfentanil 100 micrograms/kg was administered as an i.v. bolus to 9 patients with severe chronic renal dysfunction (creatinine clearance 1.0 +/- 1.2 ml/min) requiring regular haemodialysis. Plasma alfentanil concentrations were measured by a specific radioimmunoassay. Individual plasma concentration-time curves were fitted to a two-compartment open model. Mean distribution and elimination half-lives were 3.7 min and 58 min, respectively. The apparent volumes of distribution of the central compartment and the total volume of distribution at steady-state were 91 ml/kg and 304 ml/kg, respectively. Alfentanil plasma clearance was 5.3 +/- 2.5 ml/min/kg. All the patients tolerated alfentanil well and no side-effects nor delayed recovery were observed.

The secular trend of height and menarche in Belgium: are there any signs of a future stop?

Changes such as an increase in stature, in weight and an earlier physical and sexual maturation have been observed since the 19th century in Belgium as in most industrialized countries. In this paper, we consider the secular trend in height, weight and puberty (especially menarche) over the last 20 years in Brussels. During the period 1980-1982, 4177 subjects, from the Belgian population of Brussels, aged 3-26 years, have been measured and compared with Belgian subjects living in Brussels in 1960. In our sample, we observe a statistically significant increase in height (and weight), but this trend is less rapid than the increases noticed before in Belgium. Status quo data on menarche from 1048 girls from this sample were analysed by centiles and the probit method. For the first time in Belgium, it seems that the median age is stabilized at 13.0 years. However, the number of late menarches is still evolving.

Craniostenosis: the importance of the anesthesiologist.

Based on the experience with 15 patients operated for craniostenosis in our hospital, guidelines were determined for the anesthetic and postoperative management with optimal maintenance of vital parameters. Subgaleal accumulation of blood may cause a spectacular drop of hemoglobin level during the first postoperative days. Since it is better to prevent than to cure, systematic anti-convulsive and antibiotic prophylaxis must be considered.

[Current age of the menarche in Belgium].

In Belgium, as in most industrialized countries, studies about menarcheal age have shown an earlier physical and sexual maturation. In this paper we will consider the secular trend in menarcheal age over the last twenty years in Brussels. The results concerning menstruation have been obtained in 1980-1982 from 1048 Belgian girls aged between 9 and 17. These girls have very varied socio-economic backgrounds, representative of the diversity of social origin in the Belgian population of Brussels (city and suburbs). Our data were collected using the status quo-method and compared with Jeurissen's (menarcheal age in Belgium between 1915 and 1959). Percentiles and probits were used. The lower limit, where 10% of the girls have had their menarches remains more or less constant. Between 1915 and 1959, Jeurissen noted the tendency of the median menarcheal age toward earlier sexual maturation from 14 years 4 months to 13.0. But, for the first time in Belgium, it seems that the median age is stabilized at 13.0 years. However, the higher limit (age, where 90% have had their menarches) is attained at a lower age: 15.0 for girls born between 1950-1954 and 14.0 in our sample. In conclusion, it seems that better conditions of life cause an evolution in the cases of late menarches but have no more repercussions on the median menarcheal age in this Belgian group.

Fetal heart rate abnormalities after regional analgesia for labor pain: the effect of intrathecal opioids.

BACKGROUND AND OBJECTIVES: Intrathecal opioids used to relieve labor pain have been associated with uterine hyperactivity and nonreassuring fetal heart rate abnormalities. We retrospectively evaluated all charts of singleton, term, vertex-presenting parturients in active labor requesting neuraxial pain relief during a 1-year period for the occurrence of nonreassuring fetal heart rate tracings. METHODS: Three different strategies to relieve labor pain were routinely used in our department: conventional epidural using 10 mL bupivacaine 0.125% and sufentanil 0.75 microg/mL; combined spinal and epidural (CSE) using intrathecal sufentanil (7.5 microg); and CSE using intrathecal bupivacaine (2.5 mg) and sufentanil (1.5 microg). Charts (n = 1,293) were evaluated for nonreassuring fetal heart tracings and uterine hyperactivity as well as for neonatal and labor outcome. RESULTS: Intrathecal sufentanil (7.5 microg) produced significantly more nonreassuring tracings and uterine hyperactivity. However, this did not result in more cesarean deliveries or detrimental neonatal outcome. CONCLUSION: Based on this retrospective analysis, we conclude that intrathecal sufentanil in a dose of 7.5 microg has the potential to result in more nonreassuring fetal heart rate tracings compared with both intrathecal analgesia using a bupivacaine (2.5 mg)/sufentanil (1.5 microg) mixture and epidural analgesia using bupivacaine, sufentanil, and epinephrine. Genetic and environmental sources on familial transmission in Basque families. II. Stature, weight and body mass index.

PRIMARY OBJECTIVE: The present research analyses a cross-sectional sample of 359 individuals (107 fathers, 107 mothers, 71 sons and 74 daughters) in 107 nuclear families from the Basque Country (Spain). The aim was the estimation of both transmissible and non-transmissible components acting on familial resemblance for stature, weight and body mass index (BMI). METHODS: Standardized data of each generation and sex were adjusted to a BETA model of path analysis independent of sex effects, allowing the possibility of differentiating between transmissible (genetic and cultural) and non-transmissible (environmental) components acting on the observed phenotypic variance in Biscayan offspring. RESULTS: The results supported the full model of familial transmission for the three variables, in addition to other reduced models that gave rise to transmission effects regardless of any influence from the siblings' shared environment on the environmental transmitted component from their parents (b = 0), and without genetic effects on the phenotype (h = z = 0). The most parsimonious model was accepted for weight and rejected for stature and BMI. The correlations existing between children and their mothers were higher than those with their fathers. CONCLUSION: The results confirmed the hypothesis of maternal effects influencing the inheritance of stature.

Trunk asymmetries in a Belgian school population.

Two hundred seventy schoolchildren attending Belgian town schools were screened for trunk asymmetries. The present investigation confirms a wide prevalence of clinically evident asymmetries of the trunk. In the large majority of cases, the asymmetries were accompanied by a measurable rib hump and/or lumbar prominence, which have to be considered in scoliosis screenings. Age and sex do not have a clinically demonstrable influence on the degree and laterality of asymmetry in the school population examined. A height discrepancy of shoulders, scapulae, and iliac crests not exceeding 1 cm and a discrepancy in the depth of the waist triangles not exceeding 1.5 cm are so frequency that they must be considered as physiologic. The same applies to the rib hump (not exceeding 8 mm) and the lumbar prominence (not exceeding 5 mm). As in idiopathic scoliosis, the rib hump is predominantly situated on the right, especially in girls in the prepubertal period.
