Prevention of postoperative deep-vein thrombosis by low-dose heparin in open prostatectomy.

A double-blind trial was designed to investigate the efficacy of low-dose heparin in preventing deep-vein thrombosis (DVT) after open prostatectomy... The diagnosis of DVT was established by the 125I-fibrinogen test. The incidence of DVT was 39.4% in the control group (33 patients) and 9.7% in the heparin group (31 patients) (p less than 0.01). These results suggest that low-dose heparin effectively prevents DVT after open prostatectomy

Abdominal aortic aneurysms and ureteric obstruction.

Ureteric obstruction is rarely encountered in abdominal aortic aneurysms and is due to perianeurysmal fibrosis. 3 cases are described in which aortic aneurysm and retroperitoneal fibrosis are found. Excision of the aneurysm and ureterolysis with intraperitonization of the ureter is the most appropriate surgical procedure but treatment must be individualized according to the patient's condition and the operative discoveries.

Angiographic features of xanthogranulomatous pyelonephritis.

Preoperative diagnosis of xanthogranulomatous pyelonephritis (XGP) is possible in numerous cases by renal angiography, thus permitting the differentiation of this disease from renal adenocarcinoma. Angiographic features are reviewed in patients with proved XGP. The most useful findings are: (1) presence of hypervascularised areas; (2) dilated renal capsular and perforating arteries, and (3) heterogenous aspect of the kidney during the nephrographic period.

Prevention of postoperative deep-vein thrombosis by low-dose heparin in urological surgery. A double-blind, randomised study.

The efficacy of low-dose heparin in preventing deep-vein thrombosis (D. V. T.) after urological surgery was investigated in a double-blind trial. Thromboses were detected by the 125I-labelled fibrinogen technique. The incidence of D. V. T. was 36% in the control group (25 patients) and 9% in the treated group (22 patients) (p less than 0.05). Contrary to the current opinion this form of prophylaxis was effective following open prostatectomy.

[Late urological complications of reconstructive surgery of the aortic bifurcation (author's transl)].

Ureteral obstruction following aorto-iliac or aorto-femoral dacron prosthesis has rarely been described. It seems, however, as shown in our three cases, that this complication is more common than was previously believed. In all three cases, ureteral obstruction was attributed to retroperitoneal fibrosis enclosing the graft. The threat of such a complication should lead the surgeon to carry out intravenous urography in all patients with a vascular prosthesis.

Total replacement of both ureters by prostheses.

Complete bilateral ureteric replacement by silicone prosthesis is described in a patient suffering from retroperitoneal involvement from prostatic carcinoma. Immediate and long-term results were very satisfactory from a functional and social point of view and encourage consideration of this new surgical procedure in certain forms of ureteric obstruction.

Uretic obstruction secondary to aortic femoral bypass graft.

Ureteric obstruction after aortic femoral bypass grafting is reported in 2 cases. This complication may occur more frequently than is suspected. In these 2 cases the ureter lay in its normal position in front of the graft whilst in most reported cases the ureter was passing behind the graft. Treatment is discussed.
