[Echography and percutaneous transhepatic cholangiography in the evaluation of obstructive lesions of the bile ducts].

Among diagnostic techniques proposed for the study of obstructive jaundice, the initial use of echotomography clearly distinguishes between medical and surgical forms. If surgery is not indicated, further diagnostic investigations, (transhepatic percutaneous cholangiography followed if necessary by drainage of the bile duct) are conducted. Used in conjunction, echography and TPC will solve clinical doubts in most cases.

[Rehabilitation of patients after colostomy. Personal experience].

Personal experience of aiding 58 colostomised patients is reported. Certain basic aspects of aid in the immediate and later postoperative period are emphasised: diet, stools, irrigation, psychological support. Follow up showed that most patients returned successfully to normal life, returning to work and in some cases to the practice of sports.

[Clinical value of hemorrheology in peripheral arterial occlusive diseases].

The authors report their study on the red blood cell deformability of 30 patients with isolated and combined obstructive lesions in both aorto-iliac and femoral trunk, performed before and after the exercise test. Such a test does not significantly change the deformability in normal patients while, on the contrary, the exercise test significantly changes the deformability in patients affected by atherosclerotic disease. Furthermore, in the experience of the authors the decrease of the deformability is directly related to the degree of the circulatory impairment.

[Erythrocyte filterability and treatment with pentoxifylline].

The purpose of our study was to verify the possibility of obtaining a reduction in the erythrocyte filtration rate and an improvement in the O2 tissue uptake in twenty patients suffering from insulin-dependent diabetes mellitus and diabetic angiopathy, randomly divided in two groups receiving pentoxifylline (1,200 mg/day for 30 days) or a placebo solution, respectively. The erythrocyte filtration rate decreased in patients treated with pentoxifylline and the difference of erythrocyte filtration rate between the two groups was statistically significant.

[Erythrocyte filterability in Raynaud's disease].

Hemorheological changes were more recently believed to be responsible for the pathogenesis of the Raynaud's phenomenon. In order to verify such changes, the authors report the data on red blood cell deformability obtained from a study performed in 8 patients with cold-induced Raynaud's phenomenon. From the evaluation of these data it seems likely that hemorheological changes observed during the phenomenon are the consequence of the ischemia and not vice versa.

[Usefulness of sequential isotopic cholangiography in biliary surgery follow-up].

Personal experience in the follow-up of 39 patients subjected to bile duct surgery is reported. The results indicated that sequential isotopic cholangiography is less effective as a supplementary examination in the postoperative period by comparison with its use in preoperative diagnosis. It does, in fact, constitute the only possibility of visualising the main biliary pathway in subjects who have been operated for biliodigestive anastomosis, with the exception of invasive methods, such as PTC, itself out of the question in the execution of periodic postoperative controls.

[Use of single-layer anastomoses in intestinal surgery; experimental and clinical studies].

Reference is made to an experimental and clinical investigation of single-layer intestinal anastomoses. The techniques of Gambee, D'Onofrio, Lembert, Halsted Connel were applied in six groups of 10 rabbits. The survival and weight data, gross and microscopic inspection, and the stenosis index showed that the best results were obtained with Halsted's, D'Onofrio, and Gambee's techniques. An account is also given of a clinical series of 124 patients in which Halsted's technique was employed.

A comparative evaluation of endoscopic retrograde cholangiopancreatography and the secretin-cholecystokinin test in the diagnosis of chronic pancreatitis: a multicentre study in 124 patients.

A comparative evaluation of ERCP and the bicarbonate output 30 min after stimulation of the pancreas with secretin and cholecystokinin-pancreozymin, was carried out in 124 patients, of whom 65 were affected by proven chronic pancreatitis (PCP), and 59 by only suspected chronic pancreatitis (SCP). In PCP patients the false negative results were 14.7% and 21.5%, respectively. In 5 of the PCP patients with false negatives on ERCP and functional test a normal result of both the procedures was found. ERCP and bicarbonate output were found to be abnormal in 12% and 40%, respectively of 59 patients with SCP. In 30 SCP subjects both procedures gave a normal result, and only in two cases were results abnormal. A correct classification of the SCP patients with pathological bicarbonate secretion and normal ERCP seems quite impossible. The diagnostic usefulness of ERCP results is preeminent but the of carrying out both types of investigation together may give complementary information in the evaluation of pancreatic disorders.

Study of parotid and mixed saliva in the diagnosis of chronic pancreatitis.

A study on the diagnostic value of the parotid and mixed saliva assay after stimulation with 1% pilocarpine hydrochloride was carried out in 36 controls and 26 patients affected with chronic pancreatitis. No statistical difference between the two groups was found as far as saliva volume, bicarbonate and amylase (concentration and output) are concerned. No correlation was found between the results of the saliva test and those of the secretin-pancreozymin test or endoscopic retrograde pancreatography. These data excluded any diagnostic role of the saliva test in chronic pancreatitis.

[Duodenal ulcer therapy with cimetidine. A clinical endoscopic study in 76 patients (author's transl)].

34 patients were treated with cimetidine and placebo in a double blind trial and 42 were treated with cimetidine alone. Criteria investigated were the pain response, consumption of antacids when required and especially the endoscopically demonstrated scarring of the duodenal ulcer. Compared with placebo, cimetidine is capable of soothing pain and also reducing the consumption of antacids more quickly and to a greater extent. The differences are statistically significant. Complete anatomical healing of the ulcer was established in the double blind study in 80% of the patients in the cimetidine group and in 40% of the placebo group. Similar results were also shown in the group which received cimetidine alone.

Treatment of active duodenal ulcer with oral cimetidine: a multicenter controlled endoscopic trial.

A double-blind 4-week trial of cimetidine (1 g daily) versus placebo was conducted in 68 adult outpatients with active duodenal ulceration endoscopically confirmed. 3 of the patient admitted to the study (1 on cimetidine and 2 on placebo) did not complete the trial. After 4 weeks of treatment, 25 of 33 patients (76%) receiving cimetidine had healed their ulcers compared with 15 (47%) of 32 patients receiving placebo (p less than 0.02). Antacid consumption was also significantly decreased by cimetidine when compared with placebo. Tolerability of the drug was good.

A case of common bile duct ascariasis diagnosed by duodenoscopy.

A case of common bile duct ascariasis diagnosed by duodenoscopy is presented. At the admission, the patient, cholecystectomized for gallstones 13 years before, had been complaining of epigastric pain associated with post-prandial and nocturnal vomiting. Physical examination showed only slight tenderness in the epigastrium. Laboratory findings were within normal limits, with the exception of a moderate leukocytosis. Intravenous cholangiography showed the lack of visualization of the terminal common bile duct, but the flow of contrast medium was normal. Duodenoscopy, carried out without a specific clinical suspicion, revealed an ascaris lumbricoides inserted in the common bile duct and partially protruding from the papilla Vateri. The patient was treated by piperazine, intravenous fluids, antibiotics and a choleretic compound. After 24 hours an ascaris 33 cm long was excreted in the faeces and the patient became symptom-free. Some pathophysiological, clinical and epidemiological aspects of biliary ascariasis are discussed.

Rapid increase of resistance to erythromycin and clindamycin in Streptococcus pyogenes in Italy, 1993-1995. The Italian Surveillance Group for Antimicrobial Resistance.

A survey of antibiotic resistance in Streptococcus pyogenes in Italy showed a sharp increase in erythromycin resistance. In 1993, the incidence of erythromycin-resistant strains was on average 5.1%, with marked variations by geographic area. Two years later, the incidence of these strains had registered a 1.5- to roughly 20-fold increase, with a mean value of 25.9%, exceeding 40% in three centers out of 13 and 30% in another four. For all the strains studied, normal levels of susceptibility to penicillin were reported.

Microsatellite instability and MLH1 and MSH2 germline defects are related to clinicopathological features in sporadic colorectal cancer.

Clinical and pathological features were evaluated to predict tumor microsatellite instability (MSI) and germline mutations in MLH1 and MSH2 DNA mismatch repair genes in two patient groups with sporadic colorectal cancer (CRC): 38 young patients (age </=45 years) and 31 old patients (age >/=60 years). Nine (25.7%) young patients out of 35 and five (16%) old patients out of 31 exhibited MSI in their cancers. MSI+ cancers were related to proximal cancer and mucinous carcinoma independently of the age at cancer onset. Three (7.9%) out of 38 young patients had mutations in MLH1 and MSH2 genes that led to truncated protein products; they were all at age <35 years and showed MSI in their tumors, with mucinous histotype in two cases. In conclusion, histopathological and clinical features of CRC allow identification of cancers showing DNA microsatellite instability. MSI in CRC at very early onset (age <35 years) appears useful to predict germline MMR gene defects.
