[Intestinal perforation in tuberculosis].

For 5 years the authors observed isolated abdominal tuberculosis in 6 patients. Five of them who were urgently operated upon had perforations of the tuberculous ulcers of the intestine. A specific character of the inflammation was confirmed by histological investigations. Four patients are described in detail. One of them had six perforated tuberculous ulcers of the jejunum. All these patients died. Recommendations are given how to improve the diagnosis of abdominal tuberculosis and treat patients with this severe disease.

[The surgical treatment of gunshot wounds].

The authors have made an analysis of results of clinical observations of 953 patients with gunshot wounds of soft tissues. It was found that primary surgical treatment (PST) of the gunshot wounds often proved to be not radical. Later it required reoperations in 30% of the wounded. To get more exact indications for a repeated surgical treatment it is necessary to make a careful revision of the gunshot wound within 3-5 days after getting the wound. The indications for reoperations are as follows: the development of secondary necrosis of the tissues necessary removal of the foreign body, opening of the pockets and leaks in order to provide valuable drainage as well as nonradical PST due to various causes. Reoperations performed according to the indications allowed to make the duration of treatment at an average 7 days shorter and the development of suppurations two times less frequent.

The role and significance of research in a specialized trauma center.

Compared with nonspecialized hospitals, specialized trauma centers on university or research institute campuses are distinguished by lower mortality, morbidity, and mean hospital stay because of the permanent influence research has on the results of clinical practice. Close collaboration between researchers and clinicians in St Petersburgh Trauma Shock Center helped to develop medical knowledge and technology in the initial prognostic evaluation of the injured patient in shock, to improve surgical protocols in relation to prognosis, and to develop treatment for endotoxemia and sepsis.

[The clinical aspects of toxic-septic shock in peritonitis].

An analysis of observations of 747 patients with peritonitis is presented, 94 of them had toxico-septic shock. A classification of severity of the shock is proposed in the article as well as principles of making diagnosis and treatment at different steps of management of such patients.

[A first trial of the use of human recombinant interleukin (rIL-2) in patients with tumorous diseases].

Clinical approbation of human recombinant yeast human interleukin-2 (rIL-2) was carried out in 10 patients with III-IV stages of tumor that have undergone 65 intravenous drop by drop infusions of the drug as a course of 5-11 injections in the dosage of 1-8 mln/un. The drug toxicity was shown in 4 mln and especially, in 8 mln/un dose administration. That's why the dose of 1-2 mln/un is recommended. This dose was not followed by any serious complications, and the number of slightly complicated cases was significantly decreased as compared to similar rIL-2 drug made by the "Cetus Corporation" firm. Immunostimulating effect of yeast rIL-2 was found which appeared to be able to reach it's maximum by 3-4 administrations, with it's following disappearance or inversion, which may cause immunosuppression.

[The role of adoptive immunotherapy in the combined treatment of neglected forms of colorectal cancer (a preliminary report)].

Results of treatment of 24 patients with neglected colorectal tumors in the period from 1988 till 1994 are presented. Six patients had stage C and 18 patients had stage D after Dukes. Methods of combined action on the tumor including dissection of the tumor and its metastases, using immunoadoptive therapy as extracorporal connection with the donor porcine spleen, intravenous infusions of interleukin-2 and chemotherapy were employed. All the patients with stage C according to Dukes are alive: the average period of observations was 1143 +/- 206 days with intervals within the limits of 4,5-1,5 years. Seven patients of 18 with stage D after Dukes are alive with the average period of observations 1331 +/- 216 days and an interval of observations within the limits from 6 to 2 years. Dead patients with stage D after Dukes lived 525 +/-98 days on the average, i.e. about 1,4 year. The employed method of combined treatment of colorectal tumors improves survival, prolongs the lifetime of patients with neglected forms of the disease.

[Acute intestinal obstruction as a problem in emergency surgery].

An experience with treatment of acute ileus is presented. Operations were performed on 499 patients. Postoperative lethality from small intestine obstruction was 12.5%, from the colon obstruction--32.4%. The methods of treatment of different forms of intestinal ileus are described.
