[A study on the treatment of pulmonary metastatic osteosarcoma].

The primary site of metastasis of osteosarcoma is the lung. Even if a primary lesion was completely removed by radical surgery, more than 90% of the patients died of pulmonary metastasis in one to two years in the past. A control of osteosarcoma depends upon the prevention and treatment of its pulmonary metastasis. An introduction of chemotherapy consisting mainly of adriamycin and high-dose methotrexate dramatically improved the prognosis of osteosarcoma. We analyzed the clinical data, and gross as well as histopathologic post mortem findings, in 161 cases without chemotherapy and 42 cases with systemic adjuvant chemotherapy. Based on these analysis, we tried to clarify the nature of pulmonary metastasis of osteosarcoma and to evaluate its response to treatment from a standpoint of clinical pathology, hoping that this would yield a clue to future treatment of osteosarcoma.

Effect of a bleomycin derivative on oral carcinoma. A clinical and immunologic study of five cases.

A new bleomycin derivative NK631 was administered in five cases of advanced recurrent oral carcinoma. The visible improvement of the tumor was noted in three cases, and in the cases of lower lip carcinoma the tumor completely disappeared, however, there was no effective change in cases of cervical metastases of the floor of the mouth and tongue carcinoma. The peripheral lymphocyte counts and serum proteins disclosed a characteristic decrease, serum proteins decreased in the albumin fraction and slightly increased in alpha 2-globlin fraction. Main side effects of NK 631 were skin exanthema, alopecia, anorexia, pyrexia, fatigue and bleeding from the tumor lesion. Regarding the lung function, the vital capacity did not change, but PaO and PaCO in blood gas analysis were together observed to slightly decrease, and it may be supposed that the influence of NK631 on the lung function cannot be neglected. T-cell ratio, lymphocyte blastoformation following PHA stimulation, PPD and DNCB skin tests, and phagocytosis test of peripheral leucocytes were studied. The immuno-suppressive effect of KK631 was the same or weak as bleomycin.

Immunological features and corticosteroid therapy of midline granuloma.

This paper presents treatment of midline granuloma with corticosteroids and immunological features of this disease. The cellular immunity state of this patient was examined by counting T cell subpopulation and lymphocyte blastformation index (BI) by phytohemagglutinin at various time intervals during the corticosteroid therapy. Consequently, the percentage of T cells and BI became greatly increased according to the subsiding of the primary lesion, whereas non-T cell production and its function were depressed. In this communication we discuss a possible autoantibody mechanism and impared lymphocyte transformation in midline granuloma.

Successful replantation of an amputated leg in a child.

We report the successful replantation of an amputated leg in a 4-year-old boy. Four years after the replantation, skeletal growth and nerve regeneration were good and he had an excellent recovery of function and appearance. This result was undoubtedly enhanced by his youth, the ideal conditions for nerve repair, and the good preservation of the amputated leg in ice water. Replantation of an amputated lower extermity should be tried if the conditions mentioned above are fulfilled.

IgE-selective and antigen-specific unresponsiveness in mice. I. Induction of the unresponsiveness by administration of ovalbumin-pullulan conjugate.

An experimental model was demonstrated in mice for the induction of IgE-selective unresponsiveness to ovalbumin, a protein antigen. An administration of ovalbumin, conjugated with pullulan, a linear polymer of glucose, (OA-pullulan) into mice resulted in the induction of a long lasting, IgE-selective unresponsiveness to the subsequent immunization with native OA in the form optimal to elicit IgE antibody response. The IgE-selective unresponsiveness is antigen specific and is infectious to normal mice by transferring the spleen cells from mice receiving OA-pullulan conjugate at least 2 weeks before. In contrast to other modified antigens, OA-pullulan was found to elicit good IgM and IgG antibody responses, but not an IgE response, without the aid of an adjuvant.

An experimental study on "replantation toxemia". The effect of hypothermia on an amputated limb.

Hind legs of dogs were amputated at the middle of the thigh and preserved in three different conditions: in ice water, in a refrigerator, and at room temperature. After 6 or 12 hours of ischemia, recirculation was established. The survival rate of the animals was observed and measurement of limb edema, potassium, pH, and lactate in the blood was performed to study the effects of hypothermia on prevention of "replantation toxemia." Cooling of the amputated limb was effective for prevention of toxemia, and the cooling effect was greater in ice water than in a refrigerator. However, when cooled in ice water, some animals died due to toxemia when the time of ischemia was prolonged to 12 hours. In the dead animals, a close relationship was observed between the developement of toxemia and metabolic acidosis due to the increase in lactate.

IgE antibody production to exoenzymes and common antigen (OEP) of Pseudomonas aeruginosa in mice.

IgE antibody production to exoenzymes and common antigen (OEP) of P. aeruginosa was studied in mice that were injected with the antigens incorporated into water-in-oil-in-water (w/o/w) emulsion or Al(OH)3 gel. OEP and protease toxoid (PT) elicited IgE antibody response but elastase toxoid (ET) did not. Capacity of the OEP to produce IgE antibody was reduced remarkably by it's protease treatment, which suggested that the capacity lies in the protein portion of OEP. The w/o/w emulsion was less effective than Al(OH)3 gel in adjuvanticity to elicit IgE antibody response, but the emulsion enhanced the IgM and/or IgG antibody response to PT and maintained a constant level for a long period. These findings may suggest that IgE antibody response to some components of P. aeruginosa could be induced in man. High serum level of IgE was observed in some cases of cystic fibrosis caused by P. aeruginosa infection, although the IgE antibody activity has not yet been determined.

[Suppression and therapy of experimental autoimmune uveo-retinitis by injections of retinal extract (author's transl)].

The uveo-retinitis induced in guinea-pigs after one injection in the foot-pad of an extract of guinea-pig retina mixed with Freund's complete adjuvant (FCA) was successfully inhibited by a series of injections of the retinal extract. These injections were given after the induction by the antigen + FCA, either before (suppression) or after (treatment) the onset of the ocular symptoms. Injections of the antigenic preparation alone, or mixed with Freund's incomplete adjuvant were effective. Depression of cutaneous delayed hypersensitivty was parallel to the inhibition of the disease. The repeated injections provoqued an increase of antibodies, titrated either with the complement fixation test or by passive cutaneous anaphylaxis. But no correlation appeared between these antibodies and the inhibition of the disease.
