[Photodynamic therapy of benign viral vulvar lesions].

OBJECTIVE: The aim of the study was to evaluate the use of the photodynamic effect in the treatment of genital warts in women. DESIGN: Prospective study. SETTING: Department of Obstetrics and Gynecology, Hospital Na Bulovce, 1st Faculty of Medicine, Charles University, Prague. METHODS: Twenty-five women with genital warts (HPV infection), which were verified by biopsy, were involved into the study. The affected area was treated by ten milligrams of the 5-aminolevulinic acid in three milliliters of gel for 8 hours. Red light with the 630 nanometers wavelength was applied for 15 minutes, with calculated energy intake 30 J/cm2. RESULTS: All women were free of symptoms and had negative colposcopy in the followed period of 3 to 12 months. On average 3.8 sessions were necessary to reach the remission. It was possible to repeat the session after two weeks time. During the application of light, 17 (68%) of the patients felt either no or slight burning sensation, 6 (24%) had unpleasant but bearable sensation and 2 patients had to be treated in short intravenous anesthesia. CONCLUSION: Photodynamic therapy (PDT) is a promising modality in the treatment of HPV vulvar lesions and can be considered to be a method of choice.

[Cervical carcinoma during pregnancy].

OBJECTIVE: To evaluate management and outcome of pregnant women with cervical carcinoma. DESIGN: Retrospective clinical study. SETTING: Institute for Mother and Child Care, Prague, Czech Republic. METHODS: Clinical data of pregnant women suffering from cervical carcinoma were summarized and assessed. The primary treatment was radical hysterectomy and pelvic lymphadenectomy including the foetus in utero in the first and early second trimester and caesarean section and radical surgery after viability of the foetus has been reached. Patients were assessed with regard to the stage of the disease and the stage of pregnancy. RESULTS: In this retrospective study we discuss 8 cases of cervical cancer diagnosed during pregnancy. According to the TNM classification 3 patients were in stage Ib1, 3 patients in stage Ib2, one patient in stage IIa and one patient in stage IIb. With the exception of one patient in stage IIb all cases were operable with no complication after radical surgery. After a follow-up of at least 12 months we are convinced that pregnancy has no negative effect on the prognosis of cervical cancer.

[Micronized progesterone in the treatment of imminent necrosis of a myoma during pregnancy. Ultrasound changes during treatment].

In two pregnant primigravidae with signs of imminent necrosis of a myoma treatment with micronized progesterone, preparation Utrogestan, was successful. After oral and vaginal treatment with a dose of 300-600 mg/day the symptoms receded within several days. Complete regression of symptoms was recorded within one to two weeks. Both women terminated their pregnancy during the 37th-38th week and were delivered of healthy infants. One women gave birth to the infant by Caesarean section, the other one per forcipem Shutte. During surgery on the myoma no signs of degeneration were found and it was left in situ. Progesterone treatment prevented in both women surgery on account of an acute abdomen. Treatment was monitored by ultrasonographic assessment of the flow in afferent vessels of the myoma. This measurement revealed marked changes, i.e. an increased flow. At the onset of treatment there was a small difference between the indices of pulsatility (PI) and resistance (RI). This difference increased gradually, and also the difference between systole and diastole in the investigated vessels increased. This led to an increased flow and blood supply of the myoma. Thus evidence can be provided that large doses of natural progesterone have a favourable effect on the blood vessels and increase the blood flow in the vascular wall at the site of the myoma.

[Parvovirus B 19 in pregnancy].

The human parvovirus B 19 was discovered in 1975 by Cossart but its etiological association with erythema infectiosum and hydrops of the foetus was proved only in 1984-1985. The objective of the submitted prospective study was to assemble basic findings on the herd immunity of the female population and the risk of infection with this agent during pregnancy. Seropositivity of women of fertile age before the epidemic of parvovirus B 19 in 1993 and 1994 was cca 50%, after the epidemic 70%. Erythema infectiosum is a childhood disease (96% of the cases are children under 14 years of age), but seroprevalence of parvovirus IgG in these age groups is only 11%. Thus the infection in adults is frequent but not diagnosed. A parvovirus etiology of possible loss of the foetus on the basis of serological examination is encountered only exceptionally. The Czech commercial preparation gammaglobulin can be used for prevention and treatment of a non-immune foetal hydrops.
