Natural killer cell activity and recycling capacity of natural killer cells in patients with carcinoma of the prostate.

Natural killer (NK) cell activity and recycling capacity of NK cells in patients with carcinoma of the prostate (CaP) was determined by combining the 51Cr-release cytotoxicity assay and single cell cytotoxicity assay in agarose. The NK cell activity in patients with advanced CaP (stage D) was significantly lower than the NK cell activity of normal controls and that of patients with localized CaP (stage B and C). The number of active NK cells in the peripheral blood lymphocytes was similar in all groups tested including normal controls, patients with localized CaP, and patients with advanced CaP. Maximal killing potential (Vmax) and maximal recycling capacity (MRC) were significantly reduced in patients with advanced CaP as compared to patients with localized CaP or normal controls. Obtained results indicated that the depressed NK cell activity in advanced CaP was not due to a reduced number of active NK cells but due to the decreased recycling capacity of the individual NK cells.

Sympathetic response to oral glucose load is altered in hypertensive subjects.

Blood pressure, plasma norepinephrine (NE) and epinephrine (E) levels in responses to oral glucose load (OGTT) and to head-up tilting (Tilt) were measured in 14 hypertensive (HT) and 17 normotensive subjects (NT). In response to OGTT, NE significantly increased within 30 minutes (+25 +/- 8%; p less than 0.05) in NT, followed by a decline to the basal level, while it remained unchanged in HT (+5 +/- 8%). E decreased similarly during OGTT in both groups. Mean blood pressure, however, fell only in HT (-3.3 +/- 1.1%; p less than 0.05) but did not change in NT. Blood pressure, NE and E responses to Tilt did not differ between NT and HT. These results suggest that hypertensive subjects have some defects in sympathetic activation in response to glucose loading.

[Study on thermal pulp testing of immature permanent teeth].

The duration of reaction to cool stimulation was investigated in 185 normal maxillary right and left central and lateral incisors from 55 pediatric patients aged from 6 to 15 years, who were brought to the Department of Pedodontics of Nippon Dental University Hospital, and from 9 students aged from 19 to 20 years in the School of Dental Technology and Hygiene affiliated with Nippon Dental University by using Dichloro-Difluoro-Methane coolants [PULPER (G-C Dental Industrial Corp.) and PULPOFLUORANE (Septodont Co.)] in order to determine the efficacy of thermal pulp testing for mature and immature permanent teeth. The results obtained are as follows: 1) The mean duration of reaction to cool stimulation according to the type of teeth was 0.93 sec for the maxillary left lateral incisors, 0.91 sec for the maxillary left central incisors, 0.88 sec for the maxillary right central incisors and 0.95 sec for the maxillary right lateral incisors, showing no statistically significant difference in the duration between the tooth types. 2) The duration of reaction to cool stimulation according to age was shortened with aging except for the 6-year-old patients. 3) The patients were divided into those 10 years old or younger and those older than 10 years because the tooth roots of the maxillary anterior teeth are developed at 10 years of age. The mean duration of reaction to cool stimulation was 1.02 sec in the group with immature permanent teeth, which consisted of patients aged 10 years or younger, while it was 0.82 sec in the group with mature permanent teeth, which consisted of patients aged 11 years or older.(ABSTRACT TRUNCATED AT 250 WORDS)

Microcirculatory disturbances of oral mucosa and periodontal disease in patients with liver cirrhosis.

This study showed: (1) The bone score in cirrhotic patients was significantly lower than that in the control group. (2) In liver cirrhosis, capillaries were decreased in number and they were dilated compared to that in the control. In particular, the capillaries in cirrhotic patients with ascites or oesophageal varices were dilated compared to those without. (3) Blood flow of the mucogingival junction was significantly decreased in patients with liver cirrhosis in comparison with the control group. These results suggest that microcirculatory disturbances of oral mucosa are important in the progress of periodontal disease in patients with liver cirrhosis.

[Osteoblastic skull metastasis of lung cancer].

A case of osteoblastic skull metastasis of lung cancer is reported. A 56-year-old female patient was admitted to our hospital with complaints of headache and tumor of the right parietal bone. A plain skull X-ray showed hyperostotic feature of the right parietal bone. CT scan displayed that right parietal bone became thick and osteoblastic. Soft tissue was shown in the hyperostotic bone under MRI. An external carotid angiogram showed that the skull tumor was fed by the middle meningeal artery. The skull tumor and 2 solid intracerebral tumors were extirpated. Histological examination revealed adenocarcinoma in the skull and intracerebral lesions. The present case indicates that osteoblastic stimulating factor may be secreted by lung cancer.

[Basic study on hepatic artery chemoembolization and tumor selective drug targeting by temperature-sensitive liposome with local hyperthermia].

Temperature-sensitive liposome entrapping adriamycin (L-ADM) was administered into the hepatic artery of hepatic tumor-bearing rats. The embolization of the hepatic artery with liposome and bio-distribution of ADM were examined. ADM concentration in blood showed a peak at 30 min after local heating on tumor (the heating had been performed for 6 min at 41-42 degrees C 2 hr. after injection). The value at the peak was about 3 times higher than that just before heating. ADM administered in liposomal form showed a high accumulative property to tumor with heating; ADM concentration in tumor 8 hr. after administration of ADM in liposomal form was about 5 times higher than that in liver and about 30 times higher than that in the heart, and about 20 times higher than that in tumor after administration in free form.

[The examination about agreement or disagreement among the dental findings on personal identification (Part 2)--Especially about aging factors].

Opportunities for identifying unidentified corpses from dental findings have been increasing in recent years. In finding the person in question promptly from the dental charts submitted by dental clinics or the dental findings stated in police investigation cards, it is effective to narrow down the extent of verification by preliminary screening. While dental findings are quite diversified in detailed examinations, statements in dental charts and police investigation cards are often simplified, thereby resulting in the same findings even for different subjects. On the other hand, due to progress of tooth decay, errors in statement, etc, there may be cases where information on the same subject does not necessarily agree completely. Furthermore, as is well known, the trends of dental diseases are different and the treatment of conditions depends on the kinds of teeth. Miyazawa et al. in this department analyzed samples comprising 1,000 dental cards of subjects in their twenties. The states of each tooth were broken down into 12 classes ranging from healthy teeth to dentures, and an examination was made on the rate of agreement with dental findings in different subjects. The report can be summarized as follows: 1. The rate of complete agreement in dental findings of all the 28 teeth between any different subjects was 0.03%. 2. Further examination of the dentition, which was divided into 6 parts, revealed that the rate of agreement between any different subjects was the lowest for mandibular molars (2.68%) and the highest for mandibular anterior teeth (75.35%). 3. The probability that 25 or more of the 28 teeth correspond in findings between any different subjects was 0.90%.(ABSTRACT TRUNCATED AT 250 WORDS)

Stereotactic evacuation of hypertensive cerebellar hemorrhage using plasminogen activator.

Treatment for hypertensive cerebellar hemorrhage still remains controversial as to whether direct surgical procedure is indicated or not. This is so even after the introduction of CT scan which easily demonstrates the location and size of the hematoma and the presence of hydrocephalus. In this paper, we present our experience of 20 patients with cerebellar hemorrhage treated by stereotactic evacuation using Komai's CT-stereotactic apparatus. All the patients had vertigo, cerebellar symptoms, dysfunction of brain stem or consciousness disturbance. The hematomas on CT scan were more than 28 mm in diameter. Acute obstructive hydrocephalus occurred in 90% of the patients with hematoma 40 mm or larger in size. The patients with consciousness disturbance were immediately operated on after the attack, and a drainage tube was placed in the hematoma cavity to drain cerebrospinal fluid and liquefied hematoma for one to eight days. On the other hand, when patients with hematoma around 30 mm in diameter complained vertigo for about two weeks, they also were operated on stereotactically. After the operation, their symptoms improved rapidly. The stereotactic operation could aspirate about 85% of the estimated hematoma volume and improved the hydrocephalus, except in one case in which the patient rapidly deteriorated to coma level with a large cerebellar hemorrhage and brain stem damage. This stereotactic evacuation of cerebellar hematoma using a plasminogen activator is effective for not only the removal of hematoma, but also for the treatment of secondary hydrocephalus following obstruction of the fourth ventricle by cerebellar hemorrhage.(ABSTRACT TRUNCATED AT 250 WORDS)

Anti-ischemic effect of the novel anti-ischemic agent 2-(4-(p-fluorobenzoyl)-piperidin-1-yl)-2'-acetonaphthone hydrochloride and possible mechanism of its action.

The protective effect of E-2001 (2-(4-(p-fluorobenzoyl)-piperidin-1-yl)-2'-acetonaphthone hydrochloride) was examined in various ischemic models, and the mechanisms of its action were investigated in vitro and in vivo. 1. Pretreatment with E-2001 ameliorated the degeneration of pyramidal neurons in the hippocampal CA1 sector following transient ischemia in Mongolian gerbils. 2. E-2001 improved stroke symptoms induced by permanent unilateral carotid artery ligation in gerbils. 3. E-2001 prolonged the survival time following permanent bilateral carotid artery ligation in gerbils and mice. E-2001 also prolonged the survival time following intravenous injection of KCN into mice. 4. E-2001 suppressed the high potassium-evoked release of glutamate from rat hippocampal slices. Furthermore, E-2001 prevented the excessive accumulation of extracellular glutamate induced by a brief ischemia in gerbils. 5. E-2001 exerted calcium antagonistic action, i.e., a relaxing effect on high potassium-induced contraction of rat aorta. 6. E-2001 exerted inhibitory action on the lipoperoxide production in vitro and in vivo, and exhibited a radical scavenging effect against O- and N-radicals. These results suggest that E-2001 might be effective as a novel anti-ischemic agent.

[A long-surviving case of gastric cancer with peritoneal metastasis (P3) responding to short-term high-dose chemotherapy and long-term immunotherapy].

A 37-year-old female was admitted to our hospital for further examination of epigastralgia. She was diagnosed as having multiple metastases due to advanced gastric cancer (Borrmann type 3). The operative findings showed bilateral ovarian (Krukenberg), Schnitzler and widespread peritoneal metastases involving the appendix (P3H0N2S2). She underwent total gastrectomy, splenectomy, bilateral oophorectomy, and appendectomy with CDDP (100 mg intraperitoneal administration). After operation, CDDP (50 mg/body, twice i.p. and once i.v.) and PSK (3.0 g/day) were administered. She has been followed in our outpatient department for 3 years without any recurrence. The findings suggest that combination therapy using short-term high-dose chemotherapy and long-term immunotherapy can be effective for such cases.

[Combined valvular and coronary artery surgery].

Combined valvular and coronary surgery were performed on 9 patients between 1973 and 1986. Valve replacements were consist of 5 AVR (including 1 translocated AVR and 1 Bentall operation), 2 MVR and 2 double valve replacement. Coronary surgery were of 6 aorto-coronary bypass using great saphenous veins, 1 innominate artery-coronary bypass using a Dacron prosthesis, 1 punch-out of coronary ostium and 1 dilatation of the left coronary ostium with a vein patch. Retrograde infusion of cardioplegic solution from the coronary sinus were done in patients with coronary ostial stenoses. One emergency case with acute cardiac failure died immediately after operation, and one with double valve and one with mitral valve replacement died of hepatic failure and multiple organ failure respectively within 1 year after surgery. Combined valvular and coronary surgery are necessary for patients with both diseases. Although the results of double valve replacement were not satisfactory, they would be improved using intra-aortic balloon pump and ventricular assist devices actively.
