[Four-year follow-up of thyrotoxic patients treated with antithyroid drugs bases on thyroid suppressibility-Prognostic value of the 20-min 131-I thyroid uptake (author's transl)].

Out of 77 thyrotoxic patients treated with Carbimazole (30-15 mg/day) in combination with 1-triiodothyronine (75 micron/day) for 5-48 months applying 20-min 131I-thyroid uptake test at the end of treatment as a parameter for thyroid suppressibility, 39 cases (51%) have maintained euthyroid state for more than 4 years, while 20 (26%) relapsed within 3 months and the remaining 18 (23%) recurred within 6 years after antithyroid-drug (ATD) therapy. The predictable value of 20-min 131I-thyroid uptake was useful for distinguishing relapses from others, but there was no statiscally significant difference between recurrent cases and remittent ones. Since the negative correlation between the 20-min 131I-thyroid uptake and the recurrent phase was noted, the continuation of ATD therapy until the uptake reaches below 8% postpones this phase of recurrence.

[Prognostic evolution of Graves' disease treated by synthetic antithyroid (author's transl)].

A follow-up of at least 4 years after withdrawal of a medical treatment of 77 patients with Graves' disease is presented. The treatment consisted in Carbimazole (15-30 mg/d), LT3 (75 ug/d), Propranolol (30-60 mg/d), Hydroxyzine (75 mg/d). A normal thyroid suppressibility (20 mm 99m Tc uptake test) is obtained in patients without goiter faster than in those with goiter. A normal early uptake at the sixth month therapy is a good index for cure. Despite this result the authors recommend a treatment of 12 to 18 months duration because of the probable immunodepressive effect of the antithyroid drugs.

Crevicular fluid in the osseointegrated implant sulcus: a pilot study.

The purpose of this study was to clarify the relationship between crevicular fluid volume in the osseointegrated implant sulcus and peri-implant condition. Crevicular fluid volume was measured in 63 osseointegrated implant sulci, and Gingival Index, Plaque Index, and probing depth were evaluated around the implants. The crevicular fluid volume in the sulci of osseointegrated implants with high Gingival Indexes was greater than that in those with low Gingival Indexes. The crevicular fluid volume in the sulci of the implants with high Plaque Indexes was greater than that in those with low Plaque Indexes.

Microbiological evaluation of a newly designed dental air-turbine handpiece for anti-cross contaminations.

The effectiveness of a newly developed anti-cross contamination device for a dental air-turbine handpiece was tested. The handpiece with or without the anti-cross contamination device was contaminated with two bacterial strains, Staphylococcus aureus and Streptococcus mutans, as well as two bacteriophage strains, T2 and MS2. After contamination with these microorganisms, the handpieces were disinfected with glutaraldehyde or replaced with newly autoclaved ones. Residual microorganisms inside the handpiece or an air/water supply hose line were collected and counted after overnight cultivation. The anti-cross contamination device effectively reduced the contamination level of an air-turbine handpiece to that of the negative control. No microbial contamination in the air/water supply hose line was detected with this device.

[Rotavirus, adenovirus, astrovirus, calicivirus and small round virus particles in feces of children with and without acute diarrhea, from 1987 to 1988, in the greater São Paulo].

Between 1987 and 1988, 193 faecal specimens from children, with or without diarrhea, were submitted to enzyme immunoassay, polyacrylamide-gel electrophoresis and electronmicroscopy tests for virus detection. The positivity for Rotavirus, Adenovirus, Astrovirus, Calicivirus and Small Round Virus Particles (SRVP) was 11.3%, 3.1%, 2.1%, 1.0% and 4.1%, respectively, for the 97 children with acute diarrhea. Of the 96 children without diarrhea, 4.2% were positive for Rotavirus, 1.0% for Calicivirus and 7.3% for SRVP. Of 15 positive specimens for Rotavirus, 14 showed electrophoretic patterns proper to group A and 1 specimen of group C Rotavirus. The analysis of electrophorotypes demonstrated great heterogeneity of electrophoretic patterns and predominance of subgroup 2, "long". The association of virus, bacteria and parasites was present both in children with or without acute diarrhea.

Effects of collagen concentration on epithelial differentiation: in vitro analysis with a mucosal tissue model.

Using collagen, epithelial cells, and fibroblasts isolated from the rabbit oral mucosa, a mucosal tissue model was prepared in vitro. Using this model, the effects of the collagen concentration on growth and differentiation of epithelial cells were studied. A positive correlation was found to exist between the collagen concentration and growth as well as the differentiation of epithelial cells. Further, above a 8.6% concentration of collagen, formation of the epithelial process and the basement membrane at the border between the epithelial layer and the connective tissue was noted. A conclusion was drawn that the collagen concentration in the extracellular matrix is deeply related to the growth and differentiation of mucosal epithelial cells. At the same time, it can be said that the successful completion of this research project endorsed the reliability of our methodology for the study of mucosa.

Visual findings and histologic diagnosis of pelvic endometriosis under laparoscopy and laparotomy.

OBJECTIVE: To assess the diagnostic value of visual findings in the diagnosis of endometriosis from the histological point of view. STUDY DESIGN: 212 specimens from 107 patients with benign (74.8%) or malignant (25.2%) disease were obtained by biopsy or resection under laparoscopy (65 patients) or laparotomy (42 patients). Ages ranged from 19 to 62 (mean age 36.4). Visual findings were classified according to the criteria established by the Endometriosis Committee of the Japan Society of Obstetrics and Gynecology in 1993. Specimens were stained with hematoxylin-eosin, and 15 cases with periodic acid-Schiff stain or silver impregnation stain. RESULTS: Among pigmented lesions, endometriosis was found in 73.0% of specimens from the pelvic peritoneum and in 56.4% of those from the ovaries. Blueberry spots in the pelvic peritoneum as well as ovarian chocolate cysts showed the highest positive rate. In the presence of multiple or complex pigmented lesions of the pelvic area, the rate was still higher (88.6%). Those rates were due to our inclusion of inaccurate and incomplete biopsy specimens. Endometriosis of nonpigmented lesions was found in only 11 patients (12.0%) who also had pigmented lesions and/or adenomyosis. CONCLUSION: The laparoscopic diagnosis of endometriosis can be made only when multiple complex pigmented lesions are observed, but, otherwise, histopathological confirmation is necessary.

Mandibular lengthening by intraoral distraction using osseointegrated implants.

Mandibular lengthening by distraction osteogenesis is a new method for use in treating congenital deformities or postsurgical bone defects. However, the use of extraoral transcutaneous pins in the mandible has disadvantages, such as facial scars and facial nerve or inferior alveolar nerve injury. The purpose of this study was to establish a new approach to distraction osteogenesis in the mandible by using osseointegrated implants and an intraoral device. Ten adult canines were used for this experiment. After extraction of the teeth and placement of two titanium implants in the left mandible, connection of the intraoral distraction device to the abutments, and corticotomy in the medial portion between implants were performed. Distraction was done at the rate of 1 mm per day to elongate 10 mm in length. Radiographic and histologic examinations showed that successful mandibular lengthening was achieved. New bone was primarily formed by intramembranous ossification and partial endochondral ossification. Titanium implants placed for anchorage of the device remained stable during the course of mandibular lengthening. Study results suggest that the intraoral device using osseointegrated dental implants can be used as a mechanism for distraction osteogenesis in the mandible.

Osseointegrated implants in a patient with osteoporosis: a case report.

Osteoporosis is a relative contraindication for osseointegrated implants. This article describes a patient with severe osteoporosis who underwent implant therapy in the mandible. The treatment was carried out in a conventional manner, except for a longer healing period (6 months). All implants had clinically osseointegrated. Furthermore, there was no problem after the superstructure was placed.

A Japanese multicenter study of osseointegrated implants placed in irradiated tissues: a preliminary report.

A survey was undertaken to analyze implants placed in irradiated tissues. It was found that nine centers had placed 118 implants in 24 patients in Japan. Of 118 implants, 39 were in the maxilla, 71 were in the mandible, and 8 were in the orbital region. Seven patients underwent adjunctive hyperbaric oxygen treatment. The treatment decreased implant loss only in the maxilla. (The success rate without hyperbaric oxygen treatment was 62.5%, and that with hyperbaric oxygen treatment was 80.0% for the maxilla.) Implants 7 and 10 mm in length were at a greater risk of being lost than longer implants in the maxilla.

Studies of the pathogenesis and management of thrombotic thrombocytopenic purpura.

We have studied the pathophysiological signs and methods of management of thrombotic thrombocytopenic purpura (TTP) in our TTP patients. We showed that anti-platelet GP II b- III a monoclonal antibodies bound to human vascular endothelial cells (HUVEC). The binding rate of 125I-anti-platelet GP II b- III a monoclonal antibodies to HUVEC treated with TTP patients sera was decreased, compared with the value observed for HUVEC treated with normal sera. These findings showed that GP II b- III a like substances are expressed on HUVEC and that TTP patients sera appear to contain anti-platelet GP II b- III a antibodies which may attack and injure the endothelial cells. PGI2 stabilizing activity of plasma is measured by PGI2 inhibitory activity attenuation on normal platelet aggregation induced by ADP. PGI2 stabilizing activity decreases in acute phase, increases in remission, suggesting that there may be a relationship between pathogenesis of TTP and decreases of PGI2 stabilizing activity. We also showed that PGI2 analogue (beraprost sodium) was useful for prevention of relapse of TTP. Plasmapheresis has emerged as the treatment of choice of TTP. We found the effectiveness of the high molecular weight fraction (HMW-F) of plasma in chronic TTP patients. HMW-F of plasma may contain the main factor necessary for improvement of TTP.

Effects of steroid-induced osteoporosis on osseointegration of titanium implants.

The purpose of this study was to clarify the effects of steroid administration on the osseointegration of pure titanium implants. Twelve female New Zealand white rabbits, 8 weeks of age, were divided into two groups: a prednisolone-treated group (Group P) and a control group (Group C). In each rabbit, two implants were placed into the mandible and two into the tibial metaphyses with bone tapping. The six steroid-treated rabbits received three courses of 4 days of prednisolone injections (10 mg/kg per day) before implant placement, 1 month and 2 months after implant placement. The six control rabbits received no administration of prednisolone. Three months after implant placement, all rabbits were sacrificed. Bone density of the femur and removal torque of the implants placed in the tibia were significantly lower in Group P than in Group C. In addition, there were significant correlations between the bone density of the femur and the removal torque of the implants placed in the tibia. There was no significant difference in removal torque of the implants placed in the mandible between Group P and Group C, and there was no significant correlation between the bone density of the femur and the removal torque of the implants placed in the mandible. These results suggest that steroid administration could have less effect on the osseointegration of titanium implants in the mandible than in the skeletal bone.

Experience with osseointegrated implants placed in irradiated tissues in Japan and the United States.

A survey was undertaken to analyze osseointegrated implants placed in irradiated maxillae and mandibles. It was found that nine centers in Japan and two centers in the United States had placed 228 implants in 44 patients. Of 228 implants, 59 were placed in the maxilla, 169 in the mandible. Three of 169 implants placed in irradiated mandibles were removed; 17 of 59 were removed from maxillae. One hundred nineteen implants were placed in the mandible without adjunctive hyperbaric oxygen treatment, of which only 2 were removed. From these results, it is suggested that osseointegrated implants can be placed in the irradiated mandibles of selected patients without hyperbaric oxygen treatment.
