Platelet insulin receptor determination in non-insulin dependent diabetes mellitus.

The platelet membrane insulin receptors of healthy and non-insulin dependent (type 2) diabetic patients were studied. Receptor number and affinity proved to be decreased in type 2 diabetes mellitus. The changes in platelet insulin receptor characteristics are in good correlation with the alterations reported in other tissues or cells. The possible role of these phenomena in the pathogenesis of disturbed platelet function in diabetics needs further investigation.

Haemostasis in diabetics.

Haemostasis was studied in 34 diabetic patients with and without detectable vascular complications (micro- and macroangiopathy). Information on platelet functions was obtained by beta-thromboglobulin determination, and of heparin-thrombin coagulation time, platelet aggregation in vivo, and on the condition of the vessel walls by estimation of factor VIII-protein (VIIIR:Ag). The results were suggestive of an increased platelet activity, the most marked abnormalities having been found in cases of angiopathy. Attention is drawn to the therapeutic possibilities offered by studies of the pathogenetic role of the abnormalities of haemostasis in diabetes.

Hepatic actions of progestogenic steroids.

The effects of two oral progestogens, norethisterone and d-norgestrel, were studied on hepatic functions in CFY female rats. In healthy animals both progestogens inhibited cytochrome-P-450 dependent nonspecific mixed function monooxygenases. Serum bilirubin and cholylglycin RIA values were normal. Protein and glycogen content of the liver remained intact after norethisterone, d-norgestrel reduced glycogen. The pathologic values characteristic for chronic carbon-tetrachloride damage improved on a highly significant grade when these progestogens were simultaneously administered. The progestogens inhibited mixed function monooxygenases. We may presume that in this way they inhibited the formation of hepatotoxic trichloromethyl free radical and improved several liver functions.

Plasma levels of beta-thromboglobulin and factor VIII-related antigen in diabetic children and adults.

In order to investigate the relationship between the in vivo platelet activation in diabetes mellitus and the endothelial damage connected with the diabetic micro- and/or macroangiopathy, plasma levels of beta-thromboglobulin (B-TG) and of factor VIII-related antigen (VIII R:Ag) were studied (1) in juvenile-onset (Type I) diabetics without clinical signs of angiopathy (age under 12 years) and (2) in mostly maturity-onset (Type II) diabetics with and without overt angiopathy (age between 14 and 60 years). Normal controls and nondiabetics with atherosclerosis were also studied. Plasma levels of both proteins were found to be elevated in all the groups of diabetic and atherosclerotic patients in comparison with the controls. Highest levels were found in adult diabetics with angiopathy and in atherosclerotics even without diabetes, but values of the diabetic children were also elevated. The data suggest a causal relationship between the vascular damage and the enhanced platelet reactivity in which the former may play the primary role.

Serum alpha-fetoprotein in chronic liver disease.

The serum alpha-fetoprotein level was measured by radioimmunoassay in 20 normal subjects and in 118 patients with chronic liver disease. The highest concentrations were found in the group of hepatocellular carcinoma. Compared with the controls, higher values were demonstrable in the other groups of liver disease as well. This is attributed to the more or less marked regeneration of liver tissue, which is never absent in any type of liver disease studied, and in the case of fatty degeneration, to the alpha-fetoprotein inducing effect of alcohol. The importance of alpha-fetoprotein estimation in the diagnosis of hepatocellular carcinoma and monitoring of its therapy, as also in the assessment of activity and regeneration in other forms of chronic liver disease, are pointed out. Countercurrent electrophoresis has been found unreliable for the measurement of alpha-fetoprotein.
