Synthesis and antiscorbutic activity of vitamin C analogue: L-threo-hex-2-enaro-1,4-lactone ethyl ester in the guinea pig.

L-threo-hex-2-enaro-1,4-lactone ethyl ester (II) was synthesized by the modified Fisher's esterification and its sodium salt was obtained almost quantitatively. Confirmation of the compound was made by elementary analysis, as well as IR, UV, MS and NMR spectra. The antiscorbutic activity was compared to that of ascorbic acid and the result showed that (II) did not have Vitamin C activity. The results of this investigation indicate that a close relationship exists between the chemical structure of the C-6 position of ascorbic acid analogues and the development of vitamin C activity.

[Stereotactic evacuation of massive hypertensive intracerebral hemorrhage].

Six patients with massive hypertensive intracerebral hemorrhage and showing progression of consciousness-disturbance were treated by CT-guided stereotactic surgery. Serious complications or the age of these patients prevented evacuation of the hematomas by craniotomy under general anesthesia. The increase in the size of the intracerebral hematoma suggested by the progression of the consciousness-disturbance on admission was stopped by controlling the blood pressure. Stereotactic evacuation of the hematoma was performed using Komai's CT stereotactic apparatus 1-4 days after the onset. On the CT slice showing the maximum size of the hematoma, two target points showing each center or two circles which cover the greater part of the hematoma were determined, and then two drainage tubes were inserted into the two target points of the hematoma through two burr holes, and the hematoma was aspirated with a syringe. Postoperatively, every 12 hours, a solution of 60,000IU urokinase in 100 ml saline was irrigated into the hematoma cavity with aspiration of the hematoma, and finally 10 ml urokinase solution was left in the hematoma cavity. By 2-4 repetitions of this procedure, 83-91% of the estimated hematoma volume was evacuated using urokinase (120,000-240,000IU) for 1-2 days. Therefore, all of the cases showed improvement in the consciousness level without rebleeding or progression of serious complications. For large hypertensive intracerebral hematomas in aged patients or patients with serious complications, this stereotactic surgery can be carried out safely and rapidly through two drainage tubes using urokinase after 24 hours from the onset.

[Surgical treatment of aged patients with ruptured cerebral aneurysm; evaluation of the operations performed without using retractors].

Sixteen patients over 70 years of age with ruptured cerebral aneurysms in the anterior circulation were surgically treated via the pterional approach. Self-retaining brain retractors (SRBRs) were used in seven patients (Group A), but not in nine other patients (Group B). On admission there were no significant differences between the groups in terms of the neurological grading of Hunt & Kosnik or the CT grading of Fisher. The timing of the operation was decided on the basis of the neurological grading of Hunt & Kosnik and the systemic complications present in each case. Early operation (within 2 days) was performed in 10 cases (5 cases in each group), whereas delayed operation (after 15 days) was carried out in 6 cases (Group A: 2 cases; Group B: 4 cases). Symptomatic vasospasm occurred in 43% of Group A and in 33% of Group B. Postoperative CT scans showed new or aggravated brain edema in the fronto-temporal region (the site of craniotomy) in 3 Group A cases, but not in any Group B cases. By means of technetium-99m-labeled hexamethyl-propyleneamine oxime single photon emission computed tomography (SPECT) the regional cerebral blood flow (CBF) in the fronto-temporal region of the operated side was studied in 6 patients (3 cases in each group) one day after and three months after the operation. One day after the operation, the regional CBF in the fronto-temporal region had decreased in the three Group A cases, but no changes were observed in the three Group B cases. Three months after the operation the regional CBF impairment had improved in 2 of the Group A cases.(ABSTRACT TRUNCATED AT 250 WORDS)

Studies on the ascorbic acid 2-sulfate biosynthesis in rat.

The possible biosynthesis of ascorbic acid 2-sulfate (AsS) in rat was investigated. A in vivo test was conducted according to Mumma and Mohamram. 10 mumoles ascorbic acid (AsA) and 25 mu Ci35SO4(2-) were injected intraperitoneally into rats once a day for four days. 24 hours after the final administration, the animals were sacrificed and the liver, kidneys and urine were collected. The result did not confirm the biosynthesis of AsS under the test conditions, though there appeared the possibility of the biosynthesis of a trace amount of AsS too small to permit detection.

MRI demonstration of cortical laminar necrosis and delayed white matter injury in anoxic encephalopathy.

We performed serial radiological examinations on a patient with anoxic encephalopathy. In the early term after the anoxic insult, T1-weighted MRI revealed high signal intensity areas distributed laminarly in the cerebral cortex and diffusely in the putamen, which were thought to reflect the cortical necrosis and necrosis in the putamen. Single photon emission computed tomography using I-123 isopropylamphetamine showed persistent hypoperfusion in the arterial watershed zones. T2-weighted MRI performed several months after the anoxic episode revealed diffuse high-intensity lesions in the arterial water-shed zones. These delayed-onset white matter lesions continued to extend over several months.

[Treatment of symptomatic vasospasm with GIK (glucose-insulin-potassium) infusion].

GIK (glucose-insulin-potassium) solution has been administered to myocardial infarction patients as a polarizing therapy, but the effects of GIK administration on vasospasm after subarachnoid hemorrhage have never been reported. We used GIK solution to treat 7 cases of symptomatic vasospasm with congestive heart failure due to hypervolemia-hypertensive treatment. The GIK solution, composed of 200cc of 50% glucose solution, 250cc of water, 40 mEq of KCl, and 20 units of actrapid insulin, was administered continuously through a central venous catheter. The GIK therapy improved congestive heart failure following elevation of cardiac output in 7 cases, and simultaneously stabilized the serum glucose level within the range of 88-175 mg/dl. After GIK administration, remarkable improvement in the consciousness level was achieved in all cases, and cerebral infarction due to vasospasm appeared in only one case in spite of severe subarachnoid hemorrhage. It is thought that GIK therapy will be effective in the treatment of symptomatic vasospasm with congestive heart failure through the normalization of hemodynamics, the improvement of hyperglycemia and protection against cerebral ischemia.

Immunosuppressive acidic protein in true histiocytic lymphoma.

Serum immunosuppressive acidic protein (IAP) was determined in patient with true histiocytic lymphoma. The change of serum level of IAP correlated well with the clinical course, i.e., increased at relapse and decreased during remission. Immunohistochemistry showed IAP was present in the cytoplasms of normal granulocytes and histiocytes but not in neoplastic histiocytes.

[Effect of radiation therapy on extracerebral cavernous hemangioma in the middle fossa].

We report three cases of extracerebral cavernous hemangiomas in the middle fossa all of which had received radiotherapy. Follow-up study with serial computed tomography during and after irradiation were presented. In Case 1, radiotherapy after partial removal of the tumor reduced the tumor's size and CT number, and facilitated total removal. In Case 2 as well, irradiation was started before the operation. The tumor responded very well to irradiation and showed significant reduction in the size and CT number of the tumor with a dose of about 3,000 rads. Subtotal removal was possible. In Case 3, the tumor responded very well clinically to irradiation, and vertigo were much improved with a dose of about 3,000 rads, and CT scan at that time showed significant reduction in the size and CT number of the tumor, therefore she only received radiation therapy, no surgical intervention. It is concluded that in cases of extracerebral cavernous hemangioma with massive hemorrhage, radiation of up to 3,000 rads was a method of choice. The treatment results in an increased probability of total removal of the tumor, and moreover the possibility of eliminating surgery.

Studies on the excretion of ascorbic acid 2-sulfate and total vitamin C into human urine after oral administration of ascorbic acid 2-sulfate.

The excretion of AsS and total vitamin C into urine after oral administration of AsS to humans was investigated. When 10 mmol of AsS was administered to the subjects, the excretion of AsS into urine continued for 60 hr in males and 48 hr in females. The average amount excreted per hour was less than 5 mg. These results differed from those for AsA and DAsA orally administered to humans. The determination of vitamin C after oral administration of AsS to the subjects consisting of ten males and six females showed no vitamin C effect in humans, similarly to the case with the guinea pig and the rhesus monkey.
