Coffee and serum lipids: findings from the Olivetti Heart Study.

The relationship between coffee consumption and blood lipids was analyzed in a sample of 900 male workers of southern Italy participating in the Olivetti Heart Study. In the univariate analysis, coffee drinkers (n = 856) had higher values for body mass index (P < or = 0.05) and number of cigarettes smoked per day (P < or = 0.001) and lower levels of serum high-density-lipoprotein cholesterol (P < or = 0.05), compared to noncoffee drinkers (n = 44). In addition, coffee consumption (cups/d) was positively related to serum triglyceride levels (r = 0.105, P < or = 0.01) and cigarette smoking (r = 0.491, P < or = 0.01), and was inversely related to age (r = -0.122, P < or = 0.01). After multivariate adjustment, coffee consumption remained significantly related to age, cigarette smoking, and body mass index (data not shown). After stratification for smoking status, a significant positive linear trend between coffee consumption and serum total cholesterol was observed only in smokers. No significant trend was observed for serum triglycerides and high-density-lipoprotein cholesterol with coffee intake according to smoking status. This finding suggests that the relationship between coffee consumption and serum total cholesterol may change with the smoking status.

Food sources of nutrients in the diet of elderly Italians: I. Macronutrients and lipids.

The major sources of macronutrients in the Italian diet were assessed. Elderly Italians, aged > or = 60 recorded their weighed intake for a 7-day period. A total of 945 subjects from 14 centres throughout Italy participated during the period 1983-1987. These data were used to quantify the contribution of individual foods to the total intake of and variability in intake of kilocalories, protein, carbohydrate, total fat, saturated fat, oleic acid, polyunsaturated fatty acids, linoleic acid, and cholesterol. The foods ranked one to five for Italy were compared to the top five foods contributing to these nutrients in the diet of the elderly in the US based on the NHANES II Survey (1976-1980). These data provide information for the design of an Italian data-based quantitative food frequency questionnaire. They are also of interest in their description of an industrialized, well-nourished population with a dietary pattern markedly different from the US--lower in saturated fat, higher in complex carbohydrate and higher in alcohol.

Food sources of nutrients of the diet of elderly Italians: II. Micronutrients.

The foods which contribute most to the intake of selected vitamins and minerals were evaluated for the elderly in Italy and compared to food sources of nutrients in the US. A sample of 945 Italians, aged > or = 60, from 14 centres throughout Italy, each completed weighed 7-day food records during the period 1983-1987. These data were used to quantify the contribution of individual foods to the total intake of and variability in intake of vitamin A, thiamin, riboflavin, vitamin C, calcium, and iron. In addition, those foods contributing most to micronutrient intake for the Italians were compared to those foods contributing most for the elderly in the US as recorded by the NHANES II Survey (1976-1980). Overall, there was a preponderance of vegetable rather than animal sources, and of less processed food sources in Italy. In Italy, wine was an important source of several nutrients, including iron and riboflavin. These data are useful for characterizing diet in a well-nourished population with marked differences in diet from the US. They may also form the basis for a quantitative food frequency questionnaire for epidemiological studies in Italy.

The relationship of erythrocyte sodium-lithium countertransport to blood pressure and metabolic abnormalities in a sample of untreated middle-aged male workers.

OBJECTIVES: To investigate the relationship between erythrocyte sodium-lithium countertransport and blood pressure in a randomly selected sample of untreated male workers and to evaluate the influence of a set of metabolic abnormalities commonly associated with hypertension on this relationship. DESIGN: A cross-sectional investigation of a randomly selected sample of untreated male workers (n = 216, age range 21-59 years) at the Olivetti factory in Pozzuoli, Naples. METHODS: Standardized measurements of anthropometric and metabolic parameters, blood pressure and Na(+)-Li+ countertransport were performed. RESULTS: In a simple linear correlation analysis Na(+)-Li+ countertransport was directly related to plasma triglycerides and uric acid concentrations, body mass index (BMI) and systolic and diastolic blood pressure. Significantly higher values of Na(+)-Li+ countertransport were observed in the two upper quintiles of the serum triglycerides and uric acid distributions, and of the BMI distribution. Na(+)-Li+ countertransport accounted for approximately 2% of the blood pressure variation in this study population, but its contribution to the effect of metabolic covariates was not statistically significant. Hypertensive individuals with one or more metabolic abnormality had a significantly higher mean level of Na(+)-Li+ countertransport than those hypertensives who were free of such alterations. CONCLUSIONS: The results of the present study suggest that a high level of Na(+)-Li+ countertransport is more common in those hypertensive individuals who have concomitant metabolic abnormalities than in hypertensives who are free of such abnormalities.

Earthquake and coronary heart disease risk factors: a longitudinal study.

The longitudinal association between a number of coronary heart disease risk factors and the experience of a natural disaster (earthquake) was analyzed in a group of workers participating in a longitudinal epidemiologic investigation. The 5-year follow-up examination was interrupted by a major earthquake, and examinations were resumed 2 weeks after the quake. Participants screened after the quake had, on average, higher heart rates, serum cholesterol, and triglycerides than participants examined before the quake; these differences were independent from the coronary heart disease risk factor values measured 5 years previously during the baseline examination. The data collected during the 12-year examination indicated that the observed short-term increase in serum lipids and heart rate was not present long-term (7 years after the quake). These longitudinal data indicate that exposure to a natural disaster can be associated with short-term increases in heart rate, serum cholesterol, and triglycerides but that there is no apparent long-term effect on these coronary heart disease risk factors.

Sodium-lithium countertransport and body fat distribution.

The relationship between erythrocyte sodium-lithium countertransport (Na-Li CT) and body fat distribution is analyzed in a sample (n = 101) of normotensive and untreated hypertensive men participating in an epidemiological study of coronary heart disease risk factors. Na-Li CT is significantly and positively associated with both subscapular skinfold and waist to hip ratio, but not with triceps skinfold. The univariate correlation between Na-Li CT and blood pressure is diminished when adjusted for body mass index and waist to hip ratio. These findings support the existence of an association between Na-Li CT and central body fat distribution and suggest that the metabolic abnormalities associated with centrally distributed body fat could explain, at least in part, the association between Na-Li CT and blood pressure. The maximal velocity of the sodium-lithium countertransport (Na-Li CT) in erythrocytes has been reported to be directly associated with blood pressure and hypertension in numerous reports from both clinical and epidemiological studies. In most of these studies, indices of weight and/or adiposity (body mass index, in particular) have been shown to be among the most important correlates of Na-Li CT. Adiposity is an important determinant of blood pressure, and there is evidence suggesting that the patterning of the fat cells in the body is linked to a number of metabolic disturbances that could lead to hypertension and an increase in other CHD risk factors. The present report analyses the relationship between Na-Li CT and body fat distribution in a sample of normotensive and untreated hypertensive men participating in an epidemiological study.

Effects of angiotensin converting enzyme inhibitors and calcium antagonists on atrial natriuretic peptide release and action and on albumin excretion rate in hypertensive insulin-dependent diabetic patients.

Abnormalities in sodium homeostasis and in atrial natriuretic peptide (ANP) behavior could play a role in determining and accelerating the development of glomerular hypertension, hypertension, and microalbuminuria in insulin-dependent diabetes. The aim of the present study was to investigate in 32 hypertensive insulin-dependent diabetic patients (HD) with an altered albumin excretion rate the natriuretic response and ANP release to saline load (2 mmol/kg 90 min, and the effects angiotensin converting enzyme inhibitor therapy 2.5 to 5.0 mg cilazapril, once daily), and calcium antagonists (sustained release verapamil: 120 to 240 mg Isoptin Press, once daily, and long acting nifedipine: 20 to 40 mg Adalat AR, twice daily) on sodium homeostasis and albumin excretion rate. Eight normal subjects matched for sex, age, and weight served as controls. The 32 HD patients showed a blunted response in ANP release and sodium excretion during saline infusion in comparison with controls. The cilazapril and verapamil treatments were tested in 16 of the 32 HD patients and were both effective in ameliorating natriuretic and ANP response to saline load and in decreasing albumin excretion rate. The combined cilazapril and verapamil treatment further improved both these parameters in these patients, although blood pressure levels were comparable. The other 16 HD patients underwent sequential verapamil and nifedipine treatment. Verapamil was more effective than nifedipine in improving natriuresis and ANP release to saline load and in lowering the albumin excretion rate. The results of the present study demonstrate that sodium homeostasis and ANP release are altered in hypertensive nephropathic patients, and both cilazapril and verapamil are more effective than nifedipine in ameliorating natriuresis, ANP release, and albumin excretion rate.

Hematocrit, blood pressure, and hypertension. The Gubbio Population Study.

Baseline data from the Gubbio Population Study in north central Italy were used to investigate the relation of hematocrit to blood pressure and hypertension among 2,809 men and women aged 25-74 years. Independent of gender, age, and other confounders, the hypertensive group had a higher hematocrit than the nonhypertensive group (p less than 0.001). In comparison with the untreated hypertensive group, the hypertensive group being treated with diuretics or with other drugs only had similar mean hematocrit levels despite significantly lower blood pressures. Hematocrit was positively correlated with systolic pressure (r = 0.085, p less than 0.01 and r = 0.264, p less than 0.001 for men and women, respectively) and diastolic pressure (r = 0.214, p less than 0.001 and r = 0.266, p less than 0.001). In both sexes, whether or not the treated hypertensive group was included, age-adjusted prevalence of hypertension and average blood pressure were higher for persons in higher quintiles of hematocrit (p less than 0.001). The association of hematocrit with blood pressure and hypertension was significant and independent of several confounders. The regression coefficient of blood pressure on hematocrit ranged between 0.410 and 0.620 mm Hg per unit of hematocrit for systolic pressure and between 0.371 and 0.581 for diastolic pressure, depending on gender and whether the treated hypertensive group was included in multiple regression analysis. Based on exponentiation of the multiple logistic coefficient, prevalence of hypertension was at least two times greater for persons whose hematocrit levels were higher by 10 units.

The Gubbio data. Epidemiology and pathophysiology. Gubbio Study Research Group.

The relation between the maximal velocity of red blood cell sodium-lithium countertransport and blood pressure/hypertension has been studied in 2,748 men and women aged 25-74 years who participated in the 1983-85 baseline examination of the Gubbio Population Study, an epidemiologic investigation performed in a hill town of north central Italy. Men had a higher sodium-lithium countertransport velocity than women at all ages, and, in both sexes, a higher velocity was observed at successive ages. Hypertensives of both sexes had a higher sodium-lithium countertransport velocity than normotensive individuals, the difference remaining significant after control for age, body mass index and plasma uric acid concentration. Individuals with high sodium-lithium countertransport velocity had significantly greater prevalence of hypertension in both sexes. The data show the existence of a cross-sectional association between sodium-lithium countertransport velocity and hypertension in general population. Prospective study of the Gubbio population is now in progress to investigate the relation between baseline sodium-lithium countertransport velocity, its change over the years and the incidence of hypertension.

Coping and blood pressure.

The relationship between psychological coping strategies and BP was analysed in a group of 158 southern Italian men and women aged 20-59, randomly selected from an epidemiological study on coronary heart disease risk factors. Coping strategies were evaluated by use of a self-administered questionnaire (Jalowiec Coping Scale) listing 40 frequently used strategies. Factor analysis revealed the reported use of 5 different coping strategies. Males, on average, reported seeking less external support than females. In both sexes, increased years of education were associated with increased use of solution-oriented coping strategies and decreased use of resignation and sympathy seeking. After adjustment for age, alcohol use, body mass index, education and smoking, increased use of resignation was associated with lowered BP in both sexes. Emotional coping was inversely related to BP in males, but not in females. The search for external support was associated with a higher BP only in males. These data provide some evidence to suggest that psychological coping modalities are related to blood pressure.

Serum selenium and coronary heart disease risk factors in southern Italian men.

The association between serum selenium concentration and a number of coronary heart disease risk factors is studied in 364 males from southern Italy participating in the Olivetti Heart Study. Selenium correlates positively and significantly with serum cholesterol (r = 0.120; P = 0.022), and this positive association persists after adjustment for age and body mass index. Selenium levels in heavy smokers are lower than both light smokers and current non-smokers, but these differences do not reach statistical significance. Selenium is not significantly associated with any of the other CHD risk factors (e.g., triglycerides, HDL cholesterol, blood pressure, age, and body mass index). It is hypothesized that the association between selenium and serum cholesterol reported in this and previous studies could be due to dietary interrelationships between selenium intake and foods that affect serum cholesterol concentrations.

Correlates of high-density lipoprotein cholesterol in a sample of healthy workers.

METHODS: Correlates of high-density lipoprotein cholesterol are analyzed in a sample of 797 male workers in southern Italy participating in the Olivetti Heart Study. At the univariate level high-density lipoprotein cholesterol concentrations are positively related to alcohol consumption (r = 0.127; P less than or equal to 0.001) and sport activity (r = 0.074; P less than or equal to 0.05) and inversely related to body mass index (r = -0.160; P less than or equal to 0.001), serum triglycerides (r = -0.349; P less than or equal to 0.001), cigarette smoking (r = -0.227; P less than or equal to 0.001), and coffee consumption (r = -0.153; P less than or equal to 0.001). RESULTS: In the group as a whole, body mass index, alcohol consumption, cigarette smoking, and serum triglycerides remain significantly related to high-density lipoprotein cholesterol in the multivariate model, while the association with coffee intake and sport activity loses statistical significance. A significant negative interaction is reported between physical activity and cigarette smoking, and a positive significant linear trend between high-density lipoprotein cholesterol and sport activity is observed only in nonsmokers. CONCLUSION: These findings suggest that body mass index, alcohol consumption, cigarette smoking, serum triglycerides, and sport activity are important correlates of high-density lipoprotein cholesterol but that the positive significant association between sport activity and high-density lipoprotein cholesterol is absent in smokers.

Prevalence and correlates of angina pectoris in the Italian nine communities study. Research Group ATS-RF2 of the Italian National Research Council.

We investigated the prevalence and correlates of angina pectoris in 6573 men and women aged 20-59 participating in the Italian National Multicenter Study on Risk Factors for Arteriosclerosis. In both sexes, the prevalence of angina pectoris, as assessed by the Rose questionnaire, increased sharply with age (from 0.8% to 5.1% for men and from 1.7% to 6.9% for women). In all age groups, there was a higher prevalence of angina pectoris for women than for men. In men, a strong positive association was found between angina pectoris and myocardial infarction (both by self-report and electrocardiographic documentation) and self-reported dyspnea. In women, myocardial infarction (self-reported), electrocardiographic-documented myocardial ischemia, intermittent claudication, and dyspnea were all associated with angina pectoris. In both sexes, angina pectoris was positively associated with body mass index. Males with diabetes had two times the prevalence of angina pectoris as males without diabetes; in females, diabetes was only weakly associated with angina pectoris. None of other major ischemic heart disease risk factors (blood pressure, serum lipids, or smoking) was associated with angina pectoris.

Correlates of sodium-lithium countertransport. Findings from the Gubbio Epidemiological Study. The Gubbio Collaborative Study Group.

BACKGROUND: Numerous reports have presented evidence for a positive association between the maximal velocity of the sodium-lithium countertransport (Na-Li CT) in erythrocytes and hypertension. The nature of this association remains to be clarified. METHODS AND RESULTS: This report presents correlates of Na-Li CT in a population sample of 3,591 people aged 5-74 years. Males had higher mean age-specific levels of Na-Li CT than females except for the 5-14-year age stratum. In adults aged 25-74, for both men (n = 1,044) and women (n = 1,192), body mass index, plasma uric acid and glucose, alcohol consumption, and red blood cell mean corpuscular volume were positively related to countertransport in multivariate analyses; plasma high-density lipoprotein (HDL) cholesterol and plasma potassium were inversely related. Plasma non-HDL cholesterol was independently and directly related to Na-Li CT in women, and plasma sodium was inversely associated with Na-Li CT in men. These relations prevailed for men when persons with hypertension were excluded from the analyses and prevailed in part for women. When stepwise regression analyses were done for all men and women combined (n = 2,236), sex ceased to be significantly related to countertransport with plasma uric acid and alcohol intake in the model. In adults of either sex, no independent association was detected between Na-Li CT and age, heart rate, or the ratios of sodium to potassium or of sodium to creatinine in overnight untimed urine. CONCLUSIONS: In both sexes, Na-Li CT is significantly and independently associated with a number of metabolic variables (plasma uric acid, plasma glucose, body mass index, plasma potassium, and life-style habits [e.g., alcohol intake]). Further research is needed to elucidate the meaning of the significant associations between Na-Li CT and the foregoing variables (all of them also related to blood pressure).

Calcium intake and blood pressure in blacks and whites.

This study focuses on 1790 residents of Erie County, New York, over age 19, who participated in a study of blood pressure in 1986. Subjects were selected using a highly stratified sampling procedure to obtain a sample that included approximately equal numbers of blacks and whites in three educational strata (less than 12 years, 12 years, and more than 12 years of education). Calcium intake was estimated using questions regarding usual frequency of intake of foods high in calcium and of calcium-containing supplements and antacids. Calcium intake by normotensives tended to be higher than intake by hypertensives; differences were significant for females but not for males. Calcium intake was significantly inversely associated with systolic blood pressure for black females and diastolic blood pressure in black and white females after control for age, education, body mass index, smoking, and alcohol. For other sex-race groups, there was generally a trend (although not significant) toward decreased blood pressure with increased calcium intake. Diastolic blood pressure was inversely associated with calcium from milk and yogurt (but not cheese) for white females. A similar association (although not significant) was found for black females; no association was found for males. No differences were found between blacks and whites in response to calcium. This cross-sectional study provides some additional evidence of a protective effect of calcium intake on blood pressure in both black and white females.
