[Acute and subacute intracranial hepatoma with hemophilia A--two cases report (author's transl)].

Two cases of traumatic intracerebral hemorrhage in hemophiliac are presented. Case 1 was a 3-year-old boy, who had a mild trauma and became semicomatous. The carotid angiogram showed the findings of intracerebral hematoma and extravasation of contrast medium from the temporal branch of the middle cerebral artery. Evacuation of hematoma was successfully performed under human AHF administration. Postoperatively, the human AHF was administrated to maintain 30% of the plasma concentration of the VIII factor for 3 weeks. He discharged without any neurological deficit. Case 2 was a 14-year-old boy who had experienced several episodes of hemorrhage in nose and knee joints. He had a minor head trauma. Then, 12 days later, he became drowsy and the left hemiparesis developed. The right carotid angiogram revealed the presence of thin subdural hematoma and intracerebral hematoma. From clinical state, however, the conservative treatment was selected and human AHF was administered. The concentration of plasma VIII factor was kept in 30% with human AHF for 2 weeks like case 1. He completely recovered. The authors believe that the human AHF is very effective and 30% of plasma VIII factor is enough for treatment of intracerebral hemorrhage in hemophiliac.

Acute Disease↗

[Intracranial invasion of ethmoid cancer --a report of two cases (author's transl)].

Two cases of intracranial invasion of the ethmoid cancer were reported. With additional six cases reported in Japan, the precise clinical course and pathophysiological findings of the disease were reviewed, and the following several points became clear; 1) The incidence of the ethmoid cancer was rare. 2) These were divided into main groups according to the onset of intracranial signs, such as the frontal lobe symptoms and I-VII cranial nerve involvement. The first group showed the intracranial symptoms early, and the second group late. 3) The tumor invaded the cranial cavity along the perineural sheeth of each cranial nerve mainly. 4) It was very difficult to identify the paranasal sinus of origin after invading the cranial cavity. 5) The prognosis was extremely poor. All cases were died within one and half year.

Medulloblastoma in two brothers.

The occurrence of medulloblastoma in two brothers is presented. The striking similarities between two cases in the sex, age of onset of symptoms, clinical manifestations and histological findings suggested a possible role of heredity in the etiology of this tumor.

Effects of myocardial ischemia on ventricular compliance. Protective role of hydrocortisone.

The effects of hydrocortisone on left ventricular force-velocity relations, systolic length-tension curves, and diastolic pressure-volume curves (compliance) were determined before and 30 and 60 minutes after a 1 hour period of normothermic ischemic cardioplegia. Five dogs were given no hydrocortisone, 5 dogs 150 mg. per kilogram of hydrocortisone immediately after aortic unclamping, and 5 dogs 150 mg. per kilogram of hydrocortisone 20 minutes before clamping. Thirty minutes after unclamping, there was a 45 per cent decrease in contractility (p less than 0.005). At 60 minutes, there was a significant improvement (p less than 0.025), although a 32 per cent depression persisted. There were no differences among the three groups (p less than 0.05). Dogs not given hydrocortisone before clamping, compliance decreased 18 per cent (p less than 0.05), and in those given hydrocortisone immediately after unclamping, compliance decreased 28 per cent (p less than 0.025). The decreases in compliance in the hydrocortisone-treated dogs were significantly less (p less than 0.05) than the decreases observed in the dogs not given hydrocortisone.
