[Obstructive syndrome caused by trichobezoars: historical disease or disease still current? Description of a case in adolescence].

Trichobezoars are masses of entangled material, found in the stomach and intestines, composed of hair ingested by the patient. When the mass grows, symptoms of intestinal occlusion can appear. Trichobezoars in pediatrics are usually found in adolescent females presenting personality disorders and trichophagia. This work describes a case of trichobezoar diagnosed in a 13 year-old girl, wearing a brace for serious scoliosis but absolutely normal from the emotional and psychical point of view, with normal scholastic and familiar situation. Already hospitalized three months before for pneumonia from mycoplasma, the girl comes to our observation for the appearance of vomiting and constipation. The clinical examination reveal an epigastric mass as big as an orange. General conditions and hematochemical tests are good. Lab tests are performed (abdomen echography and upper abdomen MNR) but is the oesophago-gastroscopy which allows the diagnosis. A big trichobezoar is then surgically removed and the gastroenteric symptoms completely disappear.

Nasal polyps: a comparative study of morphologic and etiopathogenetic aspects.

16 nasal polyps belonging to 10 male subjects aged between 40 and 70, have been removed during the same number of ethmoidectomies; the polyps have been sectioned and specimens of mucosa corresponding to the pedicle and to the different parts of the body have been collected. During the operation parts of the mucosa of the inferior turbinates were removed. All the specimens have been prepared and observed at the scanning electron microscope. The epithelium of the inferior turbinates, of the small polyps and of the apical part of all polyps were almost normal. Marked alterations of the epithelial layer have been observed in bigger polyps. Surface characteristics of nasal polyps seem to be influenced by their relationship with neighbouring structures and by their position in the nasal cavity, but mostly by their size. On the basis of these findings the authors discuss the most recent etiopathogenetic theories of nasal polyps. They finally describe some peculiar findings concerning the mucosal layer of the polyps and of the inferior turbinates of three confirmed allergic subjects and discuss their possible clinical implications.

Benzoyltropeine, an unusual substance in street heroin samples.

Benzoyltropeine, an isomer of tropacocaine, was found in samples of heroin seized in Florence in 1981. Since there were no analytical data on benzoyltropeine, it was difficult to differentiate this substance from tropacocaine. In an effort to make the differentiation easier, the authors tried out various techniques and found thin-layer chromatography and high-resolution gas chromatography to be the most suitable for this purpose. A specific identification of isomers of these substances was obtained by using nuclear magnetic resonance and gas chromatography/mass spectrometry techniques.

Heroin in the Florence area, Italy.

The authors report data on the analysis of heroin samples seized in the Florence area between 1975 and the first half of 1981. They consider that the Florentine situation, and the Italian market in general, is different from other non-Italian markets with respect to heroin content and to types of adulterant. No dangerous substances were found in the samples and strychnine, if present, was found in very low concentrations. The concluded that the cause of death in heroin addicts or users is due to the amount of heroin injected (overdose).
