Transsphenoidal pituitary surgery for the treatment of Cushing's disease: results in 64 patients and long term follow-up studies.

Transsphenoidal pituitary surgery was performed in 64 patients with Cushing's disease in search of a corticotroph adenoma. In 4 patients, profuse local bleeding prevented completion of the exploration. Of the 60 patients who had an adequate exploration, 4 could not be followed after surgery. Short term assessment of the surgical outcome (3-6 months postoperatively) was performed on 60 patients, including the 4 who had incomplete pituitary exploration. Forty-two patients (70%) were judged as immediate successes [urinary cortisol excretion, less than 90 micrograms (less than 248 nmol)/day]; the mean urinary cortisol excretion and plasma ACTH level fell from 463 +/- 70 (+/- SE) to 26.7 +/- 3.6 micrograms/day (1277 +/- 193 to 74 +/- 10 nmol/day; n = 33) and from 111 +/- 33 to 36 +/- 14 pg/mL (24 +/- 7 to 8 +/- 3 pmol/L; n = 23) in patients who had both measurements pre- and postoperatively. Eighteen patients (30%) were judged as immediate failures; neither urinary cortisol excretion nor plasma ACTH levels changed significantly in patients who had both measurements pre- and postoperatively. The preoperative epidemiological, clinical, hormonal, and radiological characteristics of the 2 groups were similar. Histological examination of pituitary fragments removed in 58 of the 60 patients evaluated postoperatively revealed the presence of tumoral tissue in a higher percentage of patients in the immediate success group (72%) than in the immediate failure group (24%; P less than 0.01). The 42 patients in the immediate success group were followed from 6 months to 7 yr (median, 2 yr); 6 patients had recurrences from 2-3 yr after operation. Actuarial analysis indicates that the probability of a patient remaining well 6 yr after surgery is 72 +/- 20% (95% confidence limit). Most of the patients in the immediate success group had transient ACTH deficiency preceding a progressive return to normal pituitary-adrenal function.

[Medulloadrenal secretion and peripheral sympathomimetic receptors in familial dysautonomia].

A case of familial dysautonomia in a 30-year old woman with severe postural hypotension is reported. A study of adrenal medulla secretion showed that it was inadapted to the fall in blood pressure, with no increase in dopamine secretion. Insulin-induced hypoglycaemia had no effect on adrenal medulla secretion. Changes in blood pressure and heart rate were studied after administration of alpha- and beta-adrenergic drugs combined with fludrocortisone. The increase observed in blood pressure and heart rate suggests that peripheral catecholamine receptors are present in dysautonomic patients. The best treatment of postural hypotension in these patients seems to be a dihydroergotamine-fludrocortisone combination.

Separation of basic drug substances by reversed-phase high-performance liquid chromatography on dynamically modified silica and on bonded-phase materials.

The chromatographic behaviour in the reversed-phase mode of various drug substances, which are nitrogen-containing bases, has been investigated. Three different types of column materials were used, ordinary chemically bonded ODS-silica, deactivated ODS-silica specifically designed for the separation of bases and bare silica dynamically modified by adding long-chain quaternary ammonium compounds to the eluent. Various brands were tested for all three types of column materials. When using ODS-silica as the column material, considerable tailing of peaks and prolonged retention was observed. Also the deactivated columns exhibited tailing problems, but less pronounced. For both types of bonded-phase material it was possible to reduce the tailing considerably by the addition of an anti-tailing agent to the eluent. On bare silica used in the dynamic modification mode the tailing was almost negligible.

Increased urinary excretion of the retinol-binding protein in insulin-dependent diabetes mellitus in the absence of microalbuminuria.

Urinary excretion of the retinol-binding protein (RBP) was studied in 36 insulin-dependent diabetics with serum creatinine concentrations within the normal range and no microalbuminuria. The excretion was significantly (p less than 0.05) higher in the diabetics as compared to a group of apparently healthy persons. The diabetes control indices HbA1 and serum fructosamine showed no correlation to the rate of urinary RBP excretion. The present data support the idea that tubular dysfunction may be an early symptom of diabetic nephropathy.

Comparison of sodium hyaluronate and methylcellulose in extracapsular cataract extraction.

Extracapsular cataract surgery with posterior chamber lens implantation was performed in a total of 30 patients. These were randomly divided into two groups. In one group sodium hyaluronate 1% (Healon) was used as a tool in surgery, in the other group methylcellulose 2% was used. The viscous substances were irrigated out of the eyes at the conclusion of surgery. Central corneal thickness, intraocular pressure, and aqueous flare were recorded until one month post-operatively. There was no difference in corneal thickness between the two groups. The mean intraocular pressure was lower than pre-operatively in both groups with a lower pressure in the methylcellulose group compared to the sodium hyaluronate group during the first part of the observation period.

The total number of burn injuries in a Scandinavian population--a prospective analysis.

During the period 1 September 1982 to 31 August 1983 the total number of burn injuries in the municipality of Copenhagen was recorded prospectively. The social changes and the constitution of the population are illustrated. The total number of burn injuries showed a decrease of 33 per cent compared to the investigation in 1974/75. Scalds are still the most frequent cause of burn accidents, small children are especially vulnerable. Seventy-two per cent of all accidents occurred at home. Only a few patients required 'antishock' therapy. No patients with work-related burn injuries died. Preventive measures have eliminated or reduced the number of some types of thermal injury.

A new HLA-D specificity associated with DR blank: D-BON.

Since 1980, a DR blank specificity undefined by serology but known to segregate in families linked to DQw1 has been recognized. We describe a Caucasoid family, BON... where 4 among 8 children are homozygous for such a specificity; their cells can be used as homozygous typing cells and the specificity defined is provisionally called D-BON. There is no known consanguinity in this family. The homozygous D-BON cells carry DR molecules as shown by their reactivity with monomorphic anti-DR monoclonal antibodies. In a random family material consisting of 210 haplotypes, the gene frequency of D-BON is between 1 and 2%, which corresponds to about 40% of the DR blank frequency. The D-BON specificity seems associated with DQw1 and also with B35 and C4B2.

Alloimmune neonatal thrombocytopenia and hydrocephalus. Platelet antigen Zwa, ABO-antigens and HLA-antigens in mothers to infants with hydrocephalus.

In 27 mothers to infants with hydrocephalus determinations of platelet antigen Zwa, HLA-typing and ABO-typing were performed in order to evaluate whether undiagnosed alloimmune neonatal thrombocytopenia (AINT) could be an aetiological factor in hydrocephalus. All mothers were Zwa-positive, and the frequency of HLA-antigens and ABO-antigens was as in the normal population. Though sporadic cases of hydrocephalus following AINT are reported, this is not a common cause of intracranial haemorrhage and hydrocephalus.

[Hashimoto's thyroiditis with intra-thyroid production of a monoclonal antithyroglobulin autoantibody].

A 46-year old man had an Hashimoto's thyroiditis. The disease was particular by the voluminous goiter, associated with pressure symptoms and by the presence of a monoclonal Ig.G Kappa. Because of a possibly associated lymphoma of the thyroid, a total thyroidectomy was performed. Post-operative course showed a rapid and total disappearance of monoclonal Ig.G and antithyroglobulin autoantibodies. There was no pathological feature of thyroid malignancy. We could show that the monoclonal Ig.G bore an antithyroglobulin activity and that this monoclonal antithyroglobulin autoantibody was produced with in the thyroid. This case demonstrate that a monoclonal proliferation can arise from autoreactive B lymphocytes infiltrating the thyroid during Hashimoto's thyroiditis. Such a mechanism could explain, in some cases, the well-known association between lymphomas of the thyroid and Hashimoto's thyroiditis.
