The concept of internal decapitation comprises lesions within the skull base, upper spinal column with partial or complete destruction of the cervical part of the medulla. By means of casualties due to internal decapitation we demonstrate that, besides the known causes, a rotational damage at the juncture of the upper spinal column to the skull base as well as fractures by axial stretching may cause such traumas. A decrease of such accident's consequences is hardly possible because of the amount of energy involved causing the damage. Due to a steady increase of traffic, particularly by motor-bicycle, we shall have to cope with more cases of this kind of damage.
Symptoms, complications, and pathoanatomic findings of three cases, in which the esophageal perforation originally caused the death, are demonstrated. The first case deals with an pressure-errosion as a result of gavage, the second an incised wound by a razor blade, and the third a through-and-through gunshot wound of the upper part of the esophagus.

It is well established that in sera of totally duodenopancreatectomized dogs after complete insulin deprivation an immunoreactive glucagon can be found, which is indistinguishable from the pancreatic glucagon, and which can be stimulated by oral glucose and i.v. arginine as well. In four duodenopancreatectomized patients we found, after 26 h insulin deprivation, normal and elevated basal levels of IRG which were not stimulated by intravenous arginine but by oral glucose. Dilution tests of the sera before glucose stimulation showed immunological properties different from pancreatic standard glucagon, whereas in sera after stimulation immunoidentity with pancreatic standard glucagon could be demonstrated. Gel chromatography of the sera after stimulation yielded at least three immunoreactive peaks, the first over 30 000 daltons, a second eluting in the region of insulin, and a third of about 2000 daltons. True glucagon (3500 MW) could not be clearly demonstrated.
The effects of cholecystectomy upon bile salt kinetics were studied in normal guinea pigs. After cholecystectomy, bile salt pool size decreased, fractional daily turnover rate increased, and the rate of bile salt synthesis was unchanged. These data indicate that an increased frequency of bile salt enterohepatic cycling is sufficient to produce alterations in bile salt kinetics. Abnormalities of bile salt synthesis need not be present in order for a reduction in pool size to occur.

INTRODUCTION: SAFYRE is a new readjustable and minimally invasive sling for the treatment of stress urinary incontinence (SUI). Attempts to restore the normal suburethral hammock using an anatomical approach have been made in recent years. The authors report their experience with this device, which associates the efficacy of slings with readjustability. MATERIAL AND METHODS: A total of 100 consecutive patients with clinical and urodynamic diagnosis of SUI underwent SAFYRE sling procedure. The age range was from 40 to 71, mean age 63 years. Seventy-five patients (75%) presented previous failed anti-incontinence procedures. Physical clinical examination, stress test, pad use and a urodynamic study were performed before the surgery. All the patients presented symptoms of SUI and 30% also reported mild urgency. RESULTS: The average follow up period was 14 months (12-30 months). The mean operative time was of 25 minutes. Dystopia repair was performed whenever necessary, during the same procedure. The average hospital stay was 24 hours. In 3% of the implants, bladder perforation occurred. During the postoperative period, 26 patients developed urgency symptoms. During that follow up period, 92% were found to be continent, 3% reported an improvement and 5% were dissatisfied. CONCLUSION: SAFYRE is a safe and quick procedure that allows for postoperative readjustment. This technique may be an attractive alternative if the good result obtained so far proves to be long lasting.
BACKGROUND/AIMS: Degradation of adenine nucleotides to adenosine has been suggested to play a critical role in ischemic preconditioning (IPC). Thus, we questioned in patients undergoing partial hepatectomy whether (i) IPC will increase plasma purine catabolites and whether (ii) formation of purines in response to vascular clamping (Pringle maneuver) can be attenuated by prior IPC. METHODS: 75 patients were randomly assigned to three groups: group I underwent hepatectomy without vascular clamping; group II was subjected to the Pringle maneuver during resection, and group III was preconditioned (10 min ischemia and 10 min reperfusion) prior to the Pringle maneuver for resection. Central, portal venous and arterial plasma concentrations of adenosine, inosine, hypoxanthine and xanthine were determined by high-performance liquid chromatography. RESULTS: Duration of the Pringle maneuver did not differ between patients with or without IPC. Surgery without vascular clamping had only a minor effect on plasma purine concentrations. After IPC, plasma concentrations of purines transiently increased. After the Pringle maneuver alone, purine plasma concentrations were most increased. This strong rise in plasma purines caused by the Pringle maneuver, however, was significantly attenuated by IPC. When portal venous minus arterial concentration difference was calculated for inosine or hypoxanthine, the respective differences became positive in patients subjected to the Pringle maneuver and were completely prevented by preconditioning. CONCLUSION: These data demonstrate that (i) IPC increases formation of adenosine, and that (ii) the unwanted degradation of adenine nucleotides to purines caused by the Pringle maneuver can be attenuated by IPC. Because IPC also induces a decrease of portal venous minus arterial purine plasma concentration differences, IPC might possibly decrease disturbances in the energy metabolism in the intestine as well.
