[Pulsed Doppler findings of the pulmonary vein flow in mitral stenosis].

Left atrial function was assessed by transesophageal echocardiography in 8 patients with mitral stenosis and sinus rhythm (MS group), and 16 age-matched normal controls (C group). Pulsed Doppler findings of left upper pulmonary vein flow were classified into first and second forward waves during systole (S1, S2), forward wave during diastole (D), and backward flow during left atrial contraction (PVA). Peak velocity (P) and time-velocity integral (TVI) of each wave and acceleration and deceleration slope of S2 wave (S2-Ac, S2-Dc) were obtained. TVI-S2 and P-D in the MS group were significantly lower than those in the C group (TVI-S2: 4 +/- 2 vs 12 +/- 3 cm, P-D: 27 +/- 13 vs 41 +/- 12 cm/sec). S2-Ac and S2-Dc in the MS group were significantly higher than those in the C group (S2-Ac: 456 +/- 116 vs 323 +/- 118 cm/sec2, S2-Dc: 380 +/- 102 vs 165 +/- 48- cm/sec2). There were no differences in S1 and PVA. Lower TVI-S2 in the MS group suggests decreased reservoir function of the left atrium. Lower TVI-S2 in the MS group was caused by increased S2-Ac and S2-Dc which suggests increased left atrial preload and deterioration of left atrial compliance. Decreased P-D in the MS group reflects dysfunction of mitral valve opening.

[Changes in blood flow and skin reaction following radiation therapy].

Skin blood flow (SBF) during radiation therapy was measured and compared with skin reaction to examine the relationship between skin reaction and blood flow. The SBF of 50 patients including 16 with head and neck (H&N) cancer, 13 with breast cancer and eight with lung cancer were analyzed. In the H&N patients, SBF was increased according to the treated dose, while in the breast cancer patients SBF was slightly increased at 10 Gy, and did not change up to 40 Gy. In the lung cancer patients SBF was not increased until 20 Gy, and thereafter increased moderately at 30 Gy. Among H&N, breast and lung cancer patients, the most severe skin reaction was observed at the earliest time in breast cancer patients, while blood flow was only slightly increased. This must have resulted in a relatively higher skin dose than the reference point dose in breast cancer patients. Therefore, differences in the relationship between SBF and skin reaction were observed among different types of cancer patients. However, in H&N cancer patients, SBF was increased as skin dose increased. Skin blood flow could be a good indicator by which to estimate skin reaction quantitatively when the daily skin dose is around 1.6 Gy.

[A new cellulose-barium gel for computed tomography of the esophagus].

A new oral contrast agent for CT of the esophagus was evaluated for its ability to coat the esophageal lumen in 12 patients with esophageal diseases. This agent is made of carboxymethyl cellulose sodium, potato starch and low-density barium, and has remarkably high viscosity of 13600 centipoise. The esophagus was divided into proximal, mid, and distal segments to analyze the effectiveness of lumenal opacification. The average percent opacification of these segments was 78.7, 86.4, and 63.4%, respectively, for all patients. This agent is expected to play a possible role in the CT diagnosis of esophageal cancer by helping to determine the indications for radical surgery and/or radiotherapy.

The roles of the N-linked carbohydrate chain of rice alpha-amylase in thermostability and enzyme kinetics.

The thermostability and kinetics of starch hydrolysis were compared between a rice alpha-amylase isozyme Amy1A and its mutant enzyme that lacks an N-linked carbohydrate chain. Elimination of the N-glycosylation site in Amy1A reduced the thermostability of the enzyme. The temperature dependence of the kinetic parameters (Vm and Km) and substrate recognition of the enzymes were also affected by elimination of the N-glycosylation site. These results suggest that the N-linked carbohydrate chain of Amy1A has important roles in the thermostability and reaction kinetics of the enzyme.

[New development of cellulose-ferric ammonium citrate solution as a contrast-enhancement agent for magnetic resonance imaging of esophageal diseases].

For magnetic resonance imaging (MRI) of esophageal diseases, we have developed a new contrast-enhancement agent, cellulose-ferric ammonium citrate solution. Using this agent, the esophageal lumen is easily identified. This agent is expected to play a role in the MRI diagnosis of esophageal cancer and to help determine the indications for radical surgery and/or intracavitary irradiation.

The IgM antigen receptor of B lymphocytes is associated with prohibitin and a prohibitin-related protein.

The two major classes of antigen receptors on murine B lymphocytes, mIgM and mIgD, are both contained in a complex with two additional molecules, Ig-alpha and Ig-beta, which permit signal transduction. Accordingly, early biochemical events after antigen binding to either receptor are similar; biological effects, however, are different. Here, we describe three newly discovered intracellular proteins of 32, 37 and 41 kDa molecular mass, that are non-covalently associated with mIgM, but not with mIgD. These proteins coprecipitate with mIgM in Triton X-100 and Nonidet P-40, but not in digitonin lysates. In addition, mIgM is to some extent associated with 29 and 31 kDa proteins that are predominantly associated with mIgD (see accompanying paper). Amino acid sequencing of p32 and p37 identified p32 as mouse prohibitin; this was corroborated by Western blot analysis with antibodies specific for rat prohibitin. p37 is a newly discovered protein. cDNA clones for both proteins were isolated and sequenced. The deduced amino acid sequence of p32 is identical to that of rat prohibitin. p37 is highly homologous to p32. Since prohibitin was identified as an inhibitor of cell proliferation, its association with mIgM, but not mIgD, could explain the different biological events elicited after engagement of each receptor.

Two new proteins preferentially associated with membrane immunoglobulin D.

The IgM and IgD classes of antigen receptor can perform different functions on B cells. However, so far no class-specific components communicating with the cytoplasm have been found in the two antigen receptors. We have employed a new biotinylation protocol to search for intracellular membrane Ig-associated proteins. Here we describe two proteins of 29 and 31 kDa that are associated with membrane IgD and to some extent with membrane IgM. The membrane IgM molecule is associated specifically with three proteins of 32, 37 and 41 kDa. The purification and sequencing of the two mIgD-associated proteins revealed that they are novel proteins which are related to each other. These proteins may be the missing link between the antigen receptor and the cytoskeleton and may contribute to functional differences between membrane IgM and membrane IgD.

Purification of secreted alpha-amylases by immunoaffinity chromatography with cross-reactive antibody.

Two isozymes of rice alpha-amylases expressed and secreted by recombinant yeast were purified by immunoaffinity chromatography by using cross-reactive antibody. Antibodies raised against partially purified barley alpha-amylase adsorbed rice alpha-amylases in fermentation broth by a cross-reaction. By use of these antibodies as ligands, rice alpha-amylases were concentrated and purified to a high degree in one-step immunoaffinity chromatography. Because of the differences in the contaminating impurities between the barley alpha-amylase (antigen) from barley malt and rice alpha-amylases (target protein) secreted from yeast, the high purity of eluted alpha-amylases was attained without the use of highly purified antigen for immunization. Utilization of cross-reactive antibodies in immunoaffinity chromatography is useful for the purification of recombinant proteins in the absence of a sufficient amount and high enough purity of the target proteins to be purified.

[Hemodynamics during hypervolemic hemodilution (HH) technique].

In order to avoid blood transfusion in patient, we have been carrying out hypervolemic hemodilution (HH) with HES and Ringer's lactate without blood withdrawal. We studied the hemodynamics during hypervolemic hemodilution in adult mongrel dogs. The dogs were divided into two groups; control group (group C) and nitroglycerin group (group N) in which nitroglycerin was administrated at 1 microgram.kg-1.min-1. During HH, CVP, PAP, PCWP increased remarkably, and they were lower in group N than in group C, and CO increased in group N more than in group C. Though by the acute hemodilution Hct decreased to 30%, PaO2/FIO2, pH, and SvO2 remained within normal ranges. We conclude that HH is a very useful technique to avoid blood transfusion. However, during HH the excessive volume loading is associated with significant increase in CVP and PCWP, and the vasodilating therapy is effective to reduce the volume overloading.

[Changes of cytokine levels in ascitic fluid after intraperitoneal administration of OK-432 or CDDP].

In order to evaluate the biological response after intraperitoneal administration of OK-432 and CDDP, we studied the changes of cytokine levels in ascitic fluid. A total of 53 gastric cancer patients were included in this study. OK-432 20KE was administered in 7 patients and CDDP was administered in 4 patients intraperitoneally during operation. The IL-6, IL-8, TNF-alpha and sTNF RI levels in ascitic fluid were measured from 0 to 5 postoperative day. These results were compared with those obtained from the control groups (42 patients). The ascitic level of IL-6 increased immediately after operation, then gradually decreased. The elevations of IL-6 from 0 to 5 postoperative day were remarkably higher in the OK-432 treated group, and those on 0 and 1 postoperative days were remarkably lower in CDDP treated group than in the control group. Similarly, the ascitic level of IL-8 elevated soon after operation and then decreased gradually. In the OK-432 treated group, the ascitic level of IL-8 was significantly higher than in the control group from 0 to 2 postoperative day. Ascitic TNF-alpha was detectable only in the ascites soon after operation in the OK-432 treated group. The ascitic level of sTNF RI peaked on 2 postoperative day in the control and the OK-432 treated group and 1 postoperative day in the CDDP treated group. There were no significant differences between these groups.

[A case of squamous cell carcinoma of negative tumor shadow arising from pulmonary cyst].

We report a case of 53-year-old man who performed resection of infected cystic lung lesion seen from his childhood. Chest X-ray showed no remarkable change in the wall of the cyst, and tumor shadow was negative. As the chest cavity was severely adhesive, operation was ended in partial resection including the cyst. Pathological study revealed moderately differentiated squamous cell carcinoma replacing the entire cystic wall. Right upper lobectomy and dissection of lymph nodes were performed as second operation. Microscopically, carcinoma was limited in the wall of the cyst.

[Changes in urine oxygen tension (PuO2) during anesthesia with isoflurane and sevoflurane].

Leonhardt and coworkers have suggested that PuO2 may indicate degree of renal medullary perfusion and function. The changes in PuO2 during N2O-O2-isoflurane anesthesia (GOI) and N2O-O2-sevoflurane anesthesia (GOS) were studied in 20 patients (GOI group: 10 cases, GOS group: 10 cases) who underwent elective surgeries. In the GOI group, PuO2 decreased significantly 30 and 60 minutes after the beginning of surgical incision, and it returned to the previous value immediately after the cessation of isoflurane. In the GOS group PuO2 decreased starting 30 minutes after the beginning of surgical incision until 10 minutes after the cessation of sevoflurane. These results suggest that isoflurane and sevoflurane administration caused a reduction in renal medullary perfusion, and that the low perfusion during sevoflurane anesthesia lasted longer than during isoflurane anesthesia. Measurement of PuO2 has a number of problems, but we believe that PuO2 is useful as an indicator of renal medullary blood flow.

Clinical effects of intramuscular administration of a small dose of polymyxin B to patients with endotoxemia.

The effects of intramuscular injections of minute amounts of polymyxin B were studied in 42 patients with endotoxemia. Plasma endotoxin was measured by means of an endotoxin-specific Endospecy test (Seikagaku Corp., Tokyo, Japan) after pretreatment of the plasma with a new perchloric acid method that we developed. The normal value of plasma endotoxin is less than 9.8 pg/mL. Polymyxin B was administered at a dose of 12,500 U every 6 hours. Plasma endotoxin rapidly decreased to the normal range in 40 of the 42 patients. Body temperature fell significantly. APACHE II scores were also significantly improved. Tumor necrosis factor-alpha and interleukin-6 (IL-6) decreased in survivors, while tending to persist in high values in patients who died. No side effects were observed in any of the patients. In conclusion, intramuscular injections of small doses of polymyxin B were useful in the treatment of endotoxemia.

Patterns of hepatitis B virus DNA integration in liver tissue of children with chronic infections.

Although an integration of hepatitis B virus (HBV) DNA in children with chronic HBV infection has been documented at early phases of the disease, the incidence of this process is not known. Therefore we examined nine liver DNA specimens from chronic HBV carriers ages 5-14 years and one sample from a neonate delivered of a carrier mother, in order to determine the HBV DNA patterns of these patients at different ages and phases of chronic infection. The integrated HBV DNA was detected by Southern blot hybridization and analyzed by molecular cloning. Southern blot showed a smear pattern of HBV DNA integration in four of six chronic hepatitis patients (ages 5-14), as well as in one asymptomatic carrier (age 12). Multiple and random integrations occurred during chronic infections in childhood. The neonate did not, however, show signs of any integrations, suggesting that integration starts after HBV multiplication. A band pattern that suggested clonal growth of integrated liver cells was found in a chronic active hepatitis patient (age 9) and in one of two hepatocellular carcinoma patients (age 11). Molecular cloning in two cases with chronic active hepatitis showed that the HBV genome structure was preserved in five of six HBV DNA inserts. Our findings confirm that HBV DNA integration can occur at early stages of chronic HBV infection. In Japanese children, the process of integration seems to be common regardless of HBeAg/anti-HBe status.(ABSTRACT TRUNCATED AT 250 WORDS)

Primary gastric carcinoma cells frequently lose heterozygosity at the APC and MCC genetic loci.

Loss of heterozygosity (LOH) at APC and MCC gene loci (both mapped to 5q21) was investigated in 24 surgical specimens of primary gastric carcinomas using the polymerase chain reaction after tumor cell enrichment by cell sorting based on differences in DNA content. LOH at APC and/or MCC was detected in 87% (13/15) of the cases; at the APC in 86% (12/14) and at the MCC locus in 100% (7/7). LOH at the APC locus was always accompanied by LOH at the MCC locus. LOH at the APC and/or MCC was found in both differentiated and undifferentiated types in both early and advanced stages of gastric carcinoma. Thus, LOH at APC and/or MCC is considered to be one of the most prevalent genetic alterations in human gastric carcinoma and occurs at an early stage of the carcinogenesis.

Benign, non-parasitic hepatic cyst causing obstructive jaundice.

A 71-year-old woman with an hepatic cyst that led to obstructive jaundice is reported. Percutaneous intracystic fluid aspiration was useful in transiently decreasing the jaundice. Thereafter, the cyst was extirpated, and was found to be large (nearly 10 cm), benign and non-parasitic. This patient and 15 similar cases reported in the literature, indicate that the risk of jaundice induced by a simple hepatic cyst is related to its size (larger than 10 cm) and location near the common hepatic duct.

[B cell malignant lymphoma complicated with partial Addison's disease, report of a case].

A 58-year-old man was admitted in May 1988, because of high fever, skin, pigmentation and body weight loss. Abdominal ultrasonography and generalized computed tomography examinations showed swelling of general lymph nodes and bilateral adrenal glands, splenomegaly, and lesion in the liver. Serum cortisol, urinary 17-OHCS and 17-KS level were within the normal range, while the ACTH level was elevated (189.9 pg/ml). ACTH overload test showed a non-reactive pattern, leading to a diagnosis of partial Addison's disease. He was also diagnosed as non-Hodgkin lymphoma, diffuse, large cell type (B) by a biopsy of the left supraclavicular lymph node. After combination chemotherapy, swelling of the supraclavicular lymph node diminished, followed by normalization of ACTH level and improvement of symptoms. Abdominal lymphadenopathy, hepatosplenomegaly and swelling of adrenal glands also decreased in size. Although involvement of tumor cells in adrenal glands sometimes occurs in malignant lymphoma, it is reported that more than 90% destruction of adrenal gland tissue is necessary to develop Addison's disease. It was suggested that the involvement of many lymphoma cells in both adrenal glands resulted in the development of partial Addison's disease in this case.
