Transcatheter arterial chemoembolization (TACE) in the treatment of unresectable liver cancer.

Basis, techniques, and recent strategies and results of interventional treatments for hepatoma were reviewed. The basic experimental researches indispensable to justify clinical technique and dosage have been accomplished. A total of 1310 unresectable cases underwent transcatheter arterial chemoembolization (TACE) and/or related therapies. The related therapies are several modified or strengthened TACE such as Lipiodol-TACE and subsegmental TACE. The therapies for unembolizable cases are balloon occluded arterial infusion, Lipiodol-SMANCS infusion and percutaneous direct injection chemotherapy, that reinforces the effect of TACE. As for the resectable cases, postoperative TACE was proved to be beneficial because it increased survival rates; however, preoperative TACE was not beneficial. The TACE and related interventional radiologic therapies for the hepatoma can be an equal or superior therapy by virtue of targeting chemotherapy.

Post-TIPS hepatic encephalopathy treated by occlusion balloon-assisted retrograde embolization of a coexisting spontaneous splenorenal shunt.

A 51-year-old man with posthepatitis cirrhosis underwent a transjugular intrahepatic portosystemic shunt (TIPS) for bleeding of recurrent esophageal varices. The patient had a coexisting, spontaneous, splenorenal shunt. He subsequently developed hepatic encephalopathy, presumably due to excessive portosystemic shunting. Since medical management resulted in no significant improvement, the splenorenal shunt was embolized from the jugular vein approach via renal vein access during temporary balloon occlusion. Within a few days, the patient's hepatic encephalopathy resolved. Twelve months later the patient showed no recurrence of encephalopathy and had maintained a patent TIPS.

Percutaneous transluminal angioplasty in segmental obstruction of the hepatic inferior vena cava: long-term results.

Percutaneous transluminal angioplasty (PTA) was performed in 8 patients with segmental obstruction of the hepatic inferior vena cava. Using Grüntzig balloon catheters, canalization and dilatation were initially successful in all 8 patients without any complications. In long-term observation of 12-48 months, recurrence of obstruction was confirmed in 5 patients. These were recanalized easily by repeat PTA. Long-term patency was obtained in 3 patients in whom the lumen was opened to 13 mm or more in diameter as seen in lateral views of the vena cavogram.

Efficacy of Doppler ultrasonography for assessment of transjugular intrahepatic portosystemic shunt patency.

PURPOSE: To assess the efficacy of Doppler ultrasonography (US) as a noninvasive method for monitoring patency of the transjugular intrahepatic portosystemic shunt (TIPS). METHODS: Twenty-nine patients who had received TIPS for bleeding esophagogastric varices and/or refractory ascites with portal hypertension underwent Doppler US studies within 2 weeks after TIPS. Further studies were performed in 15 of them at 6 months, in 9 at 1 year, and in 4 at 2 years for a total of 57 US studies. The US findings were compared with the angiographic findings obtained at the same time. RESULTS: In 45 of the 57 studies, shunt patency was found by Doppler US, correlating to 44 patencies and one occlusion on angiography. Doppler signal in the shunt could not be detected in 12 studies resulting in the diagnosis of shunt occlusion. This correlated with angiographic occlusion in 8 studies and patency in the remaining 4. All angiographically patent shunts that were occluded by Doppler US had various degrees of stenosis. A number of technical factors were found to be responsible for Doppler US false-positive or false-negative diagnoses, some related to the type of stent used. The Doppler US sensitivity was therefore 92%, the specificity 89%. CONCLUSION: Doppler US is a reliable noninvasive method to evaluate patency of TIPS.

Peripancreatic arteries in thin-section multislice helical CT.

BACKGROUND: The normal and pathologic anatomies of the peripancreatic arteries were demonstrated using thin-section multislice computed tomography (CT). METHODS: Triple-phase dynamic CT was performed in 304 consecutive patients without pancreatic pathology and in 22 patients with pancreatic carcinoma. CT angiographies images (2.5-mm collimation, multihelical pitch 3, HQ mode) and a 1.25-mm reconstruction interval were obtained with a GE Lightspeed Qx/i at 25, 40, and 80 s after bolus injection of 130-150 mL of iodinated contrast medium at a rate of 3-5 mL/s. The images were assessed mainly by the manual cine paging method. RESULTS: The anterior and posterior arcades and the dorsal pancreatic artery were frequently visualized. However, small-caliber arteries smaller than 1.5 mm in diameter, such as the pancreaticomagna, caudal pancreatic, and transverse pancreatic arteries, were infrequently visualized. In some cases, the peripancreatic arteries were very useful for differentiating the origin of tumors. CONCLUSION: Multislice helical CT enables the recognition of small peripancreatic arteries, and evaluation of these arteries should be considered when assessing tumors in the pancreaticoduodenal region.

Treatment of a malignant esophageal fistula with a Gore-Tex-covered flexible nitinol stent.

In order to treat fistulated esophageal cancer using a flexible stent, a covered flexible stent was constructed by wrapping a nitinol stent with a thin sheet of Gore-Tex, preserving the stents original advantages of flexibility and a low-profile introducer system. This stent was used to perform standard radiotherapy in a case of fistulated esophageal cancer.

Balloon-occluded retrograde transvenous obliteration for gastric varices: a feasibility study.

PURPOSE: To evaluate the clinical feasibility of balloon-occluded retrograde transvenous obliteration (BORTO) for gastric varices. METHODS: BORTO was performed in 14 patients with gastric varices due to liver cirrhosis. The gastric varices were confirmed by endoscopy, and their feeding and draining veins were identified by contrast-enhanced computed tomography (CT) and angiography. A 6 Fr Simmons-shaped balloon catheter was inserted into the gastrorenal shunt. The balloon was inflated, and 5% ethanolamine oleate iopamidol was infused slowly through the catheter. Patients were followed up with endoscopy and enhanced CT at 1 week, 1, 3, and 6 months after the procedure and every 6 months thereafter. RESULTS: The gastric varices completely disappeared in 12 of 14 patients and was partially resolved in the remaining 2 patients. Neither a recurrence nor an aggravation of gastric varices were found. No major complications were experienced. CONCLUSION: BORTO is a safe and effective treatment for gastric varices.

Determination of solubility parameters for poly(3-hydroxyalkanoates).

The three dimensional solubility parameters defined by Hansen are based on dispersion forces between structural units, interaction between polar groups and hydrogen bonding. For polar polymers such as poly(3-hydroxyalkanoates), P(3HA), this approach was used to obtain the three coordinates of a solubility parameter in terms of: a dispersion part, a polar part and a hydrogen bonding part. Thirty-eight different solvents for poly(3-hydroxybutyrate), PHB, which are mentioned in the literature are examined by this method and the theoretical predictions are compared with the experimental reports. Another overall comparison between PHA polymers provides their Hansen and Hildebrand parameters for side chain lengths up to C13. In this series a linear progression in calculated solubility parameters with side chain length was found. An Appendix provides information and data on calculation of the solubility parameters. While the solubility information is limited and only covers homopolymers, it should help to highlight some of the contradictions regarding PHB solubility. This semi-empirical approach is only valid for amorphous polymers hence crystallinity effects, which are important with PHB, as well as molecular weight effects still require analysis.

Simultaneous determination of methamphetamine and its metabolites in monkey urine by gas chromatography/mass spectrometry with selected ion monitoring.

A sensitive and specific gas chromatography/mass spectrometry (GC/MS) method has been developed for the simultaneous analysis of methampthetamine and its metabolites (amphetamine, p-hydroxymethamphetamine, and p-hydroxyamphetamine) in monkey urine. The method has been applied to the analysis of urine samples from monkeys singly and repeatedly treated with methamphetamine. We have shown that the repeated administration of methamphetamine causes the reduction of urinary excretion of aromatic hydroxylated metabolites. We propose that the method described here can be used for biological monitoring.

Acquired expression of hst-1 in an autonomous subline (Chiba subline 2) derived from androgen-responsive mouse mammary tumor (Shionogi carcinoma 115).

Since growth of Shionogi Carcinoma 115 (SC 115) and its autonomous subline (CS 2) were regulated by fibroblast growth factor-like peptide, expression of int-2 and hst-1 was examined in these cell lines. Hybridization of genomic DNA with long terminal repeat of mouse mammary tumor virus (MMTV) revealed the same pattern of restriction fragments, showing the same integration of MMTV. Although weak expression of int-2 was noticed in the two cells, clear expression of hst-1 was seen only in CS 2 cultured with/without testosterone. It is suggested that autonomous growth of androgen-unresponsive CS 2 is connected with expression of hst-1.

Amplification of DAT1 (human dopamine transporter gene) 3' variable region in the Japanese population.

Amplification of the human dopamine transporter gene (DAT1) was performed by the polymerase chain reaction, and polymorphism of the VNTR locus was analyzed in a population sample of 176 randomly selected Japanese individuals. Genomic DNAs were extracted from whole blood, and the 3' untranslated region of the human DAT1 gene was amplified. The amplification products were distinguished by agarose gel electrophoresis, and visualized directly by ethidium bromide staining. In this way, four alleles were visualized directly and designated A, B, C, and D in decreasing order according to size. The relative frequencies of DAT1 allels A through D were 0.0171, 0.0625, 0.9119, and 0.0085, respectively. Evidence of significant deviations from the Hardy-Weinberg equilibrium was observed from these Japanese population data. The DAT1 locus was estimated to have a heterozygosity index of 13% and polymorphic information content of 0.16.

Erythroid differentiation and the cell cycle: some implications from murine foetal and erythroleukemic cells.

On the basis of observations with (1) erythropoietin induced erythroid differentiation of foetal mouse liver proerythroblasts and (2) chemically induced expression of the erythroid program in MELC, it appears that DNA replication plays a critical role in the transition to haemoglobin formation. Erythropoietin acts selectively on proerythroblasts to stimulate first housekeeping RNA species (rRNA, tRNA), then cell proliferation and differentiation. In erythro-leukemia cells expression of the erythroid program is induced by a variety of polar compounds. DNA synthesis appears requisite to this transition to haemoglobin formation, The molecular site of action of inducing compounds is not established but it is suggested that one critical effect is on the structure of chromatin which occurs during DNA replication and results in the transcription of the erythropoietic gene program.

Short-term intrathrombotic injection of ultrahigh-dose urokinase for treatment of iliac and femoropopliteal artery occlusions.

Thirty-two patients with iliac and femoropopliteal artery occlusions were treated with direct intrathrombotic injection of ultrahigh-dose urokinase (10,000 IU/min). Occlusions ranged from 3 to 25 cm (average 13.3 cm) in length and the total urokinase dose from 120,000 to 1,410,000 IU (average 688,000 IU). The procedure was usually followed by balloon angioplasty to dilate the underlying stenoses. The initial recanalization rate was 96.9%, the two week patency rate 81.3%, and the cumulative one-year patency rate 66.3%. Complications associated with this procedure included three episodes of minimal bleeding, two distal embolisms, one local hematoma, and one low-grade fever. All were well controlled by palliative treatment. Results indicate that this procedure can provide rapid recanalization at a higher success rate and with fewer complications than previously reported.
