Maculopathy and visual acuity in newly diagnosed type 2 diabetic patients and non-diabetic subjects: a 10-year follow-up study.

PURPOSE: The long-term evolution of diabetic maculopathy and visual acuity and their risk factors in patients with newly diagnosed Type 2 diabetes and in control subjects. METHODS: A 10-year prospective study consisting of a representative group of 133 (70 men, 63 women) newly diagnosed Type 2 diabetic patients diagnosed at health centers between 1979 and 1981 and 144 (62 men, 82 women) non-diabetic control subjects recruited from the population register. The frequency of maculopathy was determined by grading of 45 degrees fundus photographs and fluorescein angiograms. The subjects were studied at baseline and after 5 and 10 years. RESULTS: The frequency of maculopathy in Type 2 diabetic patients was low at the time of diagnosis but increased sharply after 5 years of the disease and at the 10-year examination 21% of diabetic patients had signs of maculopathy. By the 10-year follow-up the visual acuity declined more markedly in the diabetic patients than in the control subjects. Poor glycaemic control was the most important predictive factor for the development of maculopathy as well as deterioration of visual acuity. CONCLUSIONS: In newly diagnosed patients with Type 2 diabetes one-fifth had developed maculopathy which deteriorated their visual acuity. Poor glycaemic control was the most important predictor of maculopathy.

Leucine 7 to proline 7 polymorphism in the neuropeptide y gene is associated with retinopathy in type 2 diabetes.

In this study we tested the hypothesis that the Leu7Pro7 polymorphism in prepro neuropeptide Y (NPY) gene could be a risk marker for the development of diabetic retinopathy and analyzed a well characterized cohort of patients with Type 2 diabetes followed-up for 10 years from the time of diagnosis. The frequency of Leu7/Pro7-polymorphism was 9.3% (8 out of 86). At baseline, the frequency of retinopathy in patients with the Leu7/Pro7-polymorphism was 25% (2 out of 8) and in those without it 6.4% (5 out of 78) (p=0.126). At 10-year the respective figures were 88% and 50% (p=-0.040). The odds ratio for Leu7/Pro7-polymorphism in logistic regression analysis adjusted for age, gender and HbA1c was 8.97 (95% confidence intervals 1.09-98.0; p=0.049). Our finding based on elderly Finnish Type 2 diabetic subjects suggests that the Leu7Pro7-genotype in preproNPY gene is associated with the development of diabetic retinopathy.

Build your own multipurpose, economically priced portable stereo microscope.

Binocular loupes and stereo microscopes are expensive items of equipment. However, inexpensive operating microscopes (ophthalmic) with coaxial illumination have been constructed for use in developing countries, where there is little medical funding. We describe one method of transforming economically priced pocket binoculars into a portable stereo microscope with nearly coaxial illumination.

The effect of some treatment variables on long-term results of argon laser trabeculoplasty.

Eighty-five eyes of 75 patients were retrospectively followed up for 5 years after argon laser trabeculoplasty. The effects of some treatment and patients variables on the results of trabeculoplasty were assessed. The laser power level or visible burn effect on the chamber angle did not have any statistically significant impact on the success rate or degree of intra-ocular pressure (IOP) decrease. A statistically significant level was reached by the degree of chamber angle pigmentation and the number of pre-operative antiglaucomatous medication. In eyes where trabeculoplasty was given as primary therapy, the IOP decrease was greatest.

A prospective study of clinical and metabolic associates of proteinuria in patients with type 2 diabetes mellitus.

Urinary protein excretion rate and clinical and metabolic associates were investigated in a group of 108 patients with Type 2 diabetes mellitus at the time of diagnosis and after 5 years, and also 121 control subjects. The presence of coronary heart disease, neuropathy and retinopathy, cardiovascular risk factors and 24-h urinary excretion rate of albumin, beta-2-microglobulin, and IgG were examined. At the 5-year examination, urinary excretion rate of albumin was higher in diabetic patients than in control subjects (39 +/- 75 vs 16 +/- 28 mg 24 h-1 for men, p < 0.05; 38 +/- 57 vs 22 +/- 42 mg 24(-1) h for women, p < 0.01). Furthermore, increased beta-2-microglobulin excretion rate, a marker of tubular impairment, was observed in diabetic men as compared to control men (0.17 +/- 0.15 vs 0.14 +/- 0.21 mg 24 h-1, p < 0.05). Diabetic patients with increased albumin excretion rate (> 30 mg 24 h-1) showed poorer metabolic control than those with normal albumin excretion rate, but no significant differences in body mass index or in the frequencies of smoking, hypertension, coronary heart disease or retinopathy and neuropathy were observed between the groups. Baseline hyperinsulinaemia was closely associated with increasing albuminuria at the 5-year examination.

Contrast sensitivity in ocular hypertension and glaucoma.

Forty-eight patients aged from 60 to 69 years (58 eyes) with ocular hypertension (OHT) or primary open-angle glaucoma (POAG) and a control group of 16 persons of the same ages (26 eyes) were studied with a clinical contrast sensitivity test, the Vistech 6500 test. No significant differences could be found between the normal and OHT or POAG eyes in any of the spatial frequencies. The group of newly found POAG eyes showed the greatest difference from the normals, but this difference did not reach the level of significance in any of the spatial frequencies.

Observations on color vision testing in ocular hypertension and glaucoma.

Forty-eight patients aged from 60 to 69 years (58 eyes) with ocular hypertension (OHT) or primary open angle glaucoma (POAG) and a control group of 16 persons (31 eyes) were studied with six color vision tests: Standard Pseudoisochromatic Plates Part 2, Farnsworth Panel D 15, Farnsworth-Munsell 100-hue (FM 100) test, Lanthony Desaturated Panel, Nagel (red-green) anomaloscope, and Besançon (blue) anomalometer. In the color vision tests, the newly diagnosed OHT eyes without treatment differed significantly from the control group in the blue anomalometer. The long-term OHT eyes with treatment had no significant difference from the normals in any of the tests. The newly diagnosed POAG eyes without treatment were significantly different from the normals in the FM 100 test as well as in the boxes I, II, III and IV of the test, in the Lanthony Desaturated Panel and in the blue anomalometer. The long-term POAG eyes with treatment only differed significantly from the normal eyes in the blue anomalometer. The box IV of the FM 100 test and blue anomalometer were observed to be the most useful of these six tests in finding the possible early beginning of the blue color vision defect in the group of newly diagnosed OHT.

Presence of pseudoexfoliation on clear and opacified crystalline lenses in an aged population.

Pseudoexfoliation (PEX) of the lens capsule is a well-known risk factor for open-angle glaucoma. Its prevalence is known to increase by age in the same way as prevalence of age-dependent cataract. In the present paper the prevalence of PEX varied from 8.5 to 13.2% increasing by age. Lens opacities were statistically significantly more common in PEX-positive than PEX-negative eyes. The importance of detecting exfoliation from the lens surface is discussed.

Correlation of some ocular and hematologic factors and intraocular pressure in an aged population.

Ocular status including gonioscopy was performed on 833 volunteers in the Kuopio Eye Survey (KEYS). Some systemic factors including arterial blood pressure, body mass index, blood hemoglobin and glucose were also screened. In regression analysis only systolic blood pressure, grade of trabecular meshwork pigmentation and sex had a statistically significant correlation to intraocular pressure.

Pigmentation of the anterior chamber angle in normal and pseudoexfoliative eyes.

Anterior chamber angle pigmentation was gonioscopically graded in 833 persons recruited for the KEYS (Kuopio Eye Survey) during 1989. Pseudoexfoliation was observed in 8.5% in either eye from those born in 1924, the figure being 13.2% for those born in 1914. Anterior chamber angle pigmentation was found to be significantly heavier in eyes with pseudoexfoliation.

Incidence of open-angle glaucoma and screening of the intraocular pressure with a non-contact tonometer.

New equipment for measuring intraocular pressure have been introduced lately. One of these is the Keeler Pulsair non-contact tonometer which uses pressurized air in measurement. It is found to be safe and easy to use in practice, but it seems to give 1.5-2.0 mmHg lower reading than the Goldmann applanation tonometer. This was confirmed by the present study, where non-contact tonometry was controlled by applanation tonometry with a 2-3 week delay between the measurements. However, for screening procedures the accuracy of the apparatus can be considered as sufficient.

Kuopio eye survey (KEYS).

Mass health screenings for the elderly have been undertaken in Kuopio since 1983. Three different age groups, 65, 70 and 75 years old are called for examination so that each group is examined at 5-year intervals. In 1989 an ophthalmologic examination was included in the protocol for those born in 1914 and 1924. A total number of 1133 persons were recruited, 833 (73.5%) of which actually participated in the study. Cataract was found to be the most frequent visual handicapper. Of participants with visual acuity less than 0.3 at least in one eye, cataract was observed in 41.7% of the younger and in 91.1% of the older group. Age-dependent macular degeneration and glaucoma were less frequent than cataract. Visual acuity of less than 0.3 in the better eye was observed in 5.5% of the older group.

Co-existence of empty sella syndrome and low tension glaucoma.

In 15 low-tension glaucoma patients computed tomography of the head was re-evaluated in order to find the possible empty-sella syndrome. Fourteen CT-scans were normal, and empty sella was found in one patient with progressive low tension glaucoma. The possible role of empty sella syndrome in glaucomatous visual field defects is discussed.

Topical indomethacin in extracapsular cataract surgery. A photographic study.

A photography-based single-blind study was conducted to evaluate the effect of topical indomethacin on the per-operative pupil of 52 consecutive patients undergoing an extracapsular cataract extraction with or without an intraocular lens. Each operation was done by the author. Per-operative miosis was significantly smaller in the group receiving indomethacin as compared with the control group (P = 0.06-0.13). A reproducible photographic method for evaluating the per-operative behavior of the pupil is described.

Progressive hearing loss in Usher's syndrome.

In 18 patients with Usher's syndrome, progressive hearing loss was verified audiologically in eight cases. Despite poor auditory threshold values and low speech discrimination scores, there was only one patient who could not communicate with speech. The possibility of hearing impairment being mainly progressive in Usher's syndrome is discussed.

Repeated 50 burn/180 degree argon laser trabeculoplasty.

Thirty-five repeated 50-spot/180 degree argon laser trabeculoplasties (ALT) are prospectively compared to as many first ALT's. The distribution of the treatment variables possibly affecting the results was almost identical between the two groups. The postoperative intraocular pressure decrease was statistically significant in both groups but it was significantly smaller in the re-ALT group as compared with the pressure reduction taking place in the group receiving their first ALT.

Low power argon laser trabeculoplasty.

Sixty-seven consecutive argon laser trabeculoplasties to as many patients were retrospectively followed-up for 3 to 12 (mean 7) months. The laser power used was substantially lower than originally proposed by Wise & Witter (1979) ranging from 100 mW to 500 mW. Factors influencing the outcome of low power trabebuloplasty were evaluated. The statistical analysis of the data was performed using multiple regression analysis, analysis of variance, t-test and chi 2-test. The mean success rate remained relatively low (33%). In the high power capsular glaucoma group it was 50%.
