Endoscopic resection of large sessile colorectal polyps using a submucosal saline injection technique.

BACKGROUND/AIMS: Endoscopic removal of sessile colorectal polyps 2 cm or greater in diameter is very difficult. We evaluated the safety and usefulness of submucosal saline injection in endoscopic resection of these polyps. MATERIALS AND METHODS: Under colonoscopic observation, 0.9% NaCl was injected submucosally through needle forceps to elevate polyps, which were then resected by electrocoagulation. Before 1989, the depth and healing status of ulcers produced by endoscopic resection with (N = 12) and without (N = 16) submucosal saline injection were examined in specimens obtained at subsequent colectomies. Between 1990 and 1993, patients were assigned by weighted randomization to undergo colonoscopic polypectomy with (N = 24) or without (N = 5) submucosal saline injection. RESULTS: All 12 ulcers after submucosal saline injection were confined to the submucosal layer while 7 (44%) of 16 ulcers without submucosal saline injection reached the muscle layer or deeper. Seventeen of 24 (71%) polyps were completely removed (14 piecemeal and 3 en bloc) without serious complications after saline injection. Resection without submucosal saline injection was complete, but piecemeal, for two of five polyps; postresection bleeding occurred in one case. CONCLUSIONS: Endoscopic resection after submucosal saline injection is a safe and effective treatment for large, sessile colorectal polyps.

Effect of marzulene on the restitution of rat gastric mucosa after NaOH-induced injury.

BACKGROUND/AIMS: The purpose of this study was to assess the effect of marzulene (L-glutamine plus azulene) on the repair of NaOH-induced gastric mucosal injury in rats. METHODOLOGY: Gastric mucosal injury was induced with intragastric instillation of 3.0 ml of 5% NaOH for 1 minute. From 2 days after the operation, the rats were orally given chow pellets containing 0%, 0.25%, or 0.5% of marzulene for 25 weeks. RESULTS: Oral administration of marzulene at both dosages significantly increased the mucosal heights of the fundic and antral mucosa at week 25. Marzulene also increased the labeling indices of the fundic and antral epithelial cells, but not the mucosal blood flow. CONCLUSIONS: These findings indicate that marzulene stimulates repair mechanisms of rat gastric mucosa after NaOH injury. This effect of marzulene may be associated with a stimulation of gastric epithelial cell proliferation.

Oxygen desaturation during sedation for colonoscopy in elderly patients.

BACKGROUND/AIMS: Conscious sedation increases the tolerance of patients to endoscopic examination, but arterial oxygen desaturation during endoscopy has often been reported. Elderly patients are considered to be at special risk. However, sedation-induced changes in cardiovascular function in the elderly have not been well investigated. Therefore, this study examined the effects of sedation on pulse rate, systolic blood pressure, and arterial oxygen saturation in elderly patients. METHODOLOGY: Twenty patients aged less than 60 years and 20 patients aged 60 years or more, scheduled to undergo colonoscopic removal of colorectal polyps, received 0.05 or 0.025 mg/kg body weight, respectively, of midazolam intravenously. Before, during, and after endoscopy, pulse rate, systolic blood pressure, and arterial oxygen saturation were recorded every 5 min with a pulse oximeter. RESULTS: Administration of midazolam had no significant effect on pulse and arterial systolic blood pressure in either group. In contrast, midazolam significantly reduced the levels of arterial oxygen saturation during and after endoscopy in both age groups. However, although patients aged 60 years or more received midazolam at half dosage, arterial oxygen saturation during endoscopy was significantly lower and the incidence of 5% or more oxygen desaturation was significantly higher than in the patients aged less than 60 years. CONCLUSIONS: Midazolam increases the tolerance of elderly patients to endoscopy, but it significantly lowers the level of arterial oxygen saturation and increases the incidence of desaturation. Continuous monitoring is especially valuable during endoscopy in the elderly.
