Geographical variation in the seasonality of suicide.

This paper reports the results of a study on the seasonal variation of suicides occurred in Italy during the years 1969-1981. We found evidence for a cyclical pattern with one cycle per year for North/Central as well as for South Italy with no difference between the two parts in the variance explained by it. However, urban regions showed a higher seasonality than the rural parts of Italy, independently of their geographical location. To account for these findings, the hypothesis of a strong seasonal related urban rhythm has been put forward.

Gender, marital status and treated affective disorders in South Verona: a case register study.

Data from the South Verona Psychiatric Case Register were used to test hypotheses about the relationship between age, gender, marital status and the incidence of treated affective disorders. Analysis of the 5-year period 1983-1987 yielded overall rates of affective disorder per 10(5) of 57.7 for males and 78.4 for women. Incidence increased with age both for depressive neurosis and for affective psychosis. Married women had high rates compared with their single counterparts whereas the reverse was true for men. The very low values for incidence are likely to reflect the major role in the treatment of affective disorders carried out by Italian general practitioners, rather than a low population rate of these disorders.

[For a more rational funding system of mental health services. Analyses of costs for services supplied by community mental health services during a 7-year period].

AIMS: In the last years, in Italy as well as in many other developed countries, there has been a growing interest for health economics by researchers. As for as the psychiatric care is concerned, more recently, many research's groups have pointed their attention on new possible funding systems for mental health services and on their effects on services' functioning. The aim of this study is to define a new list of services' costs based on services actually delivered by a Community Mental Health Service (CMHS). METHODS: All psychiatric contacts recorded by the South-Verona Psychiatric Case Register during a 7-year period (1992-1998) have been included in the study (125,623 contacts made by 2,819 patients). Contacts were grouped into 19 type of services. The cost function methodology was used to describe, also reporting elasticity values, costs' behaviour in the South-Verona CMHS. The cost of each service includes expenses for professionals involved (directly or indirectly) in the contacts with the patients and capital costs. RESULTS: For each service were reported a) the cost of the service as it is actually supplied in our CMHS, b) the cost per minute, c) an estimate of the cost of service delivered with standard modalities (duration equal to the mode value registered; staff composition take into account either the actual functioning of the CMHS either indication about a good clinical practice) and, finally, d) cost of the eight services included into the reimbursement system currently in use in Italy. CONCLUSIONS: Our results showed that services' definition used in this study allow to describe different types of psychiatric care supplied from the South-Verona CMHS. The national list currently adopted for the reimbursement in Italy should allowed to describe only 28% of the registered psychiatric contacts (35,230 vs. 125,632). The urgent need for a new list of psychiatric services, accepted at a national level, was confirmed. Cost values obtained clearly show that the funding system currently used underestimates the true costs of care delivered by the CMHS. The cost function makes available a tool to test a prospective per-capita funding system as provided in the Act No. 229 of the Italian Government.

[Monitoring the impact on international literature of scientific production of Italian researchers, in the disciplines of "psychiatry" and "psychology." The period of 1995-1999 compared with the period of 1985-1989].

AIM: The aim of this paper is to present the results of a citation analysis concerned with the impact on the international literature of Italian researchers in psychiatry and psychology and of sub-fields, within these two disciplines, in 1995-1999 compared to 1985-1989. METHOD: We used a database prepared by the Institute for Scientific Information (ISI) that included all papers, with at least one Italian author, working in Italy, which appeared in two periods (1995-1999 and 1985-1989) in psychiatric or psychological journals listed in Current Contents. A citation analysis concerning the following years, up to 1999, was performed. RESULTS: In 1995-1999 the most cited researchers were those involved in clinical psychology, psychiatry and psychopharmacology research. Neuropsychology, which dominated the period 1985-1989, underwent a clear-cut decline. CONCLUSIONS: Citation analysis, concerning relatively short periods of time (five years), appears to be a very helpful tool for monitoring the development of scientific research in specific disciplines and the trend of the scientific production of researchers.

Detection and management of mental distress and psychiatric disorders in primary care settings.

OBJECTIVE: Epidemiological and clinical studies indicate that 10-50% of primary care patients suffering from clinically relevant psychiatric distress are not diagnosed by their physician and only a minority of them receive an appropriate treatment. The improvement of physicians' ability to detect mental distress and psychiatric disorder, in their routine clinical activity, represents a crucial point to reduce the social impact of mental illnesses, prevent their worsening and chronicity and, eventually, relieve mental health services of an excessive burden of care and costs. The aim of this article is to examine a number of factors which intervene in the process of detection of mental distress by the physician. Then, we will examine factors related to the management of psychiatric disorders most commonly co-occurring with physical illness in general health care sector. METHOD: The method used for this review was essentially a recension of the literature concerning detection and treatment of psychiatric disorders in primary care settings, having in view to see the factors connected with these processes. RESULTS: Among factors intervening in the process of identification of mental distress in primary care settings, both the characteristics of the physician and the characteristics of the patient should be taken into account. Primary care physicians and psychiatrists are being asked to work together more frequently in this era of community care. The principal aim of such invoked collaboration is the amelioration of quality of care and reduction of costs for mentally ill patients. An important issue within this collaboration is the referral by primary care physicians to specialist services.

[The impact on the international literature of the scientific production of Italian researchers in the disciplines "psychiatry" and "psychology". A bibliometric evaluation].

OBJECTIVE: The aim of the study was to present the results of a citation analysis concerned with the impact of Italian researchers and institutions in psychiatry and psychology upon the international scientific community. METHOD: The analysis has been performed using a database of the Institute for Scientific Information (ISI): All scientific papers which were published between 1981 and 1998 in psychiatric and psychological journals included in the Science Citation Index (SCI) and the Social Sciences Citation Index (SSCI) were considered. The most cited Italian papers, authors and institutions are reported, as well the most frequently utilised journals. RESULTS: Publications concerned with neuropsychology, psychopharmacology and biological psychiatry were the most cited. This prevalence also affected the ranking of the most cited authors, even though, in this case, research groups in disciplines such as clinical psychology and epidemiological psychiatry appeared to be strong. The four most productive Italian Universities were characterized by the presence of both a School of Medicine and a School of Psychology. The Journal of Neurology, Neurosurgery and Psychiatry and Psychopharmacology were the most frequent vehicles of scientific communication. CONCLUSIONS: The results entail important implications for Italian research in psychology and psychiatry. On a general level, these analyses appear to be helpful for monitoring scientific production by granting agencies and for comparing different individual researchers. On a more specific level the analysis has identified the leading trends in research of Italian psychiatry and psychology.

[En epidemiologic approach for the analysis of the demand and supply of psycho-psychiatric care in general medicine and in specialized services].

In this paper we present data collected in the area of Verona concerning indicators of demand and of supply of care in two settings, the general practice and the South-Verona Community Psychiatric Service. These data, obtained from the epidemiological studies conducted by our research group during the past ten years, emphasize similarities and differences existing in the two levels of care. In general practice the demand for care for psychological disorders is difficult to identify, because of the frequent somatic expression of these disorders; moreover at the level of psychiatric services the demand is frequently aspecific. As far as the supply of care is concerned, in both settings drug prescription is the most frequent treatment, however other interventions are also offered. For example, in general practice counselling and referral to the psychiatrist are often provided; in psychiatric services monitored during our studies a large range of treatments were supplied, often in an integrated way.
